The remaining colon after radical surgery for colorectal cancer. The first three years of a prospective study.

This study investigates the possible gain and limitation by performing colonoscopy and double-contrast enema immediately after, and every si … in none of 40 patients with recurrence diagnosed by other means than colonoscopy and enema. Radical colorectal surgery should be followed by colonoscopy and double-contrast enema, but how often and for how long remains to be established.

A randomized evaluation of single dose chemoprophylaxis in elective colorectal surgery--a comparison between metronidazole and doxycycline.

In a prospective, randomized blind study either 400 mg doxycycline or 1500 mg metronidazole were given intravenously in a single preoperative dose to 147 consecutive patients before elective colorectal surgery. Septic complications mainly caused by a mixed flora of aerobes and anaerobes occurred in 13% of the patients in the doxycycline group. Septic complications occurred in 29% of the patients in the metronidazole group and were caused by E. coli and coliform rods. Bacteriological studies showed a marked reduction of anaerobes in peroperative samples in this group. Most postoperative infecti…

Early implementation of exercise to facilitate recovery after elective colorectal surgery-a randomized clinical pilot trial.

BACKGROUND: Early mobilization is a key component of enhanced recovery after elective colorectal surgery. This pilot trial evaluated the safety and feasibility of an early supervised postoperative exercise intervention in addition to standard care. METHODS: In this single-center randomized controlled pilot trial, 31 patients undergoing elective colorectal surgery were randomized to an intervention group (n = 16) or control group (n = 15). One patient withdrew after randomization, leaving 30 patients for modified intention-to-treat analysis. The intervention consisted of individualized, physio…

Frailty phenotype and long-term mortality after curative colorectal cancer surgery: a retrospective multicenter cohort study.

BACKGROUND: Frailty is associated with poor short-term surgical outcomes, but whether the Fried frailty phenotype is associated with long-term mortality after curative colorectal cancer (CRC) surgery, independently of age and tumor stage, is uncertain. We examined this association using restricted mean survival time (RMST), which does not require proportional hazards. METHODS: This retrospective multicenter cohort study included 304 patients aged ≥60 years who underwent curative CRC surgery at three centers in China in 2018-2019. The preoperative Fried phenotype was analyzed as robust (n=131)

Interaction between advanced age and ASA physical status on early postoperative mortality after colorectal cancer surgery.

BACKGROUND: It is unknown whether the preoperative anesthesiological risk assessment interacts with patient age in colorectal cancer (CRC) to increase the risk of early postoperative mortality beyond that explained by the independent effects of age and anesthesiological risk assessment. METHODS: We conducted a nationwide cohort study (2014-2023) of all Danish non-metastatic CRC patients (UICC pathological TNM I-III) aged 70 or more years at time of surgery (n = 15,673). To analyse perioperative risk, we used the American Society of Anesthesiologists (ASA) Physical Status Classification system

Anastomotic Leakage After Colorectal Surgery: Exploratory Analysis of Perioperative Risk Factors in a 672-Patient Cohort.

Background Anastomotic leakage (AL) is a serious complication of colorectal surgery, associated with re-intervention, prolonged hospitalization, and impaired long-term outcomes. Although patient- and procedure-related risk factors are well established, the impact of perioperative conditions, thermal management, fluid balance, intraoperative blood loss, and vasopressor-supported hemodynamics remains incompletely defined, with heterogeneous and confounded evidence. Methods In this retrospective single-center cohort study, 672 colorectal procedures with primary anastomosis at the University Hospi…

Low preoperative adiponectin levels are associated with postoperative systemic inflammatory response following major colorectal cancer surgery.

BACKGROUND: Adiponectin is an adipokine secreted by adipose tissue that plays an important role in colorectal oncogenesis and the response to surgical stress through its anti-inflammatory effects. However, it remains unclear whether perioperative alterations in circulating adiponectin in patients undergoing colorectal cancer surgery are associated with early postoperative systemic inflammation. The aim of this study was to investigate the predictive value of perioperative adiponectin levels, interleukin-6 (IL-6), and standard inflammatory blood biomarkers for the development of postoperative s

Impact of CT-Based Body Composition Analysis on Postoperative Survival in Patients Undergoing Colorectal Cancer Surgery.

BackgroundColorectal cancer (CRC) remains a leading cause of cancer-related mortality worldwide, underscoring the need for reliable prognostic biomarkers. Emerging evidence suggests that body composition parameters may influence cancer outcomes. This study aimed to evaluate whether low skeletal muscle mass, high visceral fat, and their coexistence predict long-term outcomes in patients with CRC.MethodsThis retrospective study included 103 patients with pathological stage III CRC who underwent curative resection. Skeletal mass index (SMI) and visceral fat index (VFI) were calculated from preope

Double trouble: incidental finding of a duplicated ureter using intraureteral indocyanine green during laparoscopic colorectal surgery.

Ureteric injury is a feared complication in colorectal surgery. Anomalous genitourinary anatomy is common, with duplication of the ureter present in up to 6% of patients. This may increase the risk of inadvertent injury. We describe a laparoscopic anterior resection performed for recurrent sigmoid diverticulitis, where there were difficulties visualizing the left ureter. Intraureteral indocyanine green (ICG) was instilled via a temporary ureteric stent to assist with its identification. This unexpectedly revealed a previously unrecognized narrow-caliber bifid ureter, and enabled both branches …

Outcomes of laparoscopic reoperation for anastomotic leak-associated peritonitis following colorectal cancer surgery: a comparative study.

BACKGROUND: Anastomotic leakage after colorectal cancer surgery can lead to severe peritonitis requiring urgent surgical reintervention. Although laparotomy has traditionally been the standard approach in this setting, the role of laparoscopy for reoperation remains uncertain. This study aimed to evaluate the feasibility and safety of a laparoscopic-first strategy for reoperation in patients with anastomotic leak-associated peritonitis. METHODS: This retrospective study included patients who underwent reoperation for anastomotic leakage after colorectal cancer surgery between 2006 and 2025. P
