Various Technique Modifications of BEAR ACL Restoration.

BACKGROUND: Anterior cruciate ligament (ACL) injuries are incredibly common, with an annual incidence of 68.6 per 100,000. The Bridge-Enhanced ACL Restoration (BEAR) is a relatively novel technique that combines suture repair of the ligament and a bioactive scaffold implant to bridge the gap between the torn ligament ends. Since the adoption of the BEAR technique, several modifications have been made to the original technique. These modifications are being prospectively studied within the Bridge registry. INDICATIONS: ACL tears amenable to repair. TECHNIQUE DESCRIPTION: The described techniq

Arthroscopic Anterior Labral Repair With Capsulorrhaphy and Interval Closure: Surgical Technique.

BACKGROUND: Traumatic anterior labral injuries (Bankart lesions) lead to anterior shoulder instability. Arthroscopic anterior labral repair (also known as Bankart repair) provides a soft tissue reconstruction to reduce the risk of dislocation. However, the described techniques vary significantly. We present our technique for enhancing the quality and volume of tissue incorporated in the repair and utilizing interval closure to further reduce recurrent instability rates. INDICATIONS: Non-throwing athletes or high-demand patients with recurrent instability after a traumatic event with <20% glen

Open 4-Compartment Fasciotomy for Chronic Exertional Compartment Syndrome of the Lower Extremity.

BACKGROUND: Chronic exertional compartment syndrome (CECS) is an underrecognized cause of lower extremity dysfunction in athletes. Open 4-compartment fasciotomy remains the gold standard for surgical management. INDICATIONS: Operative intervention is indicated for patients with clinically and objectively confirmed CECS who fail to respond to conservative management. Contraindications include inadequate nonoperative treatment or unaddressed medical comorbidities. Proper diagnosis requires exclusion of vascular or neurologic causes that can mimic CECS symptoms. TECHNIQUE DESCRIPTION: A 2-incis

Arthroscopic Reduction and Suspensory Button Fixation of Bony Bankart Lesion.

BACKGROUND: Bony Bankart lesions (anteroinferior glenoid avulsion fractures) often occur after anterior shoulder dislocations, with an incidence up to 33% in first-time dislocaters. Detachment of the labrum and underlying glenoid bone piece can lead to recurrent instability, especially with increasing glenoid bone loss. Treatment is generally arthroscopic bony Bankart repair in acute injuries with viable bone fragments and minimal glenoid bone loss (<13.5%). However, there is no gold standard for arthroscopic fixation. INDICATIONS: Given the acuity of the lesion with a sizeable bony Bankart f

Patellar Shell Osteochondral Allograft Transplantation for Full-Thickness Patellar Cartilage Lesions.

BACKGROUND: Large cartilage defects of the patella in young, active patients present a significant treatment challenge, with a risk of progression to patellofemoral arthritis. Shell osteochondral allografts are a treatment option for large, irregular, or uncontained lesions, particularly in cases involving subchondral bone pathology or the failure of previous cartilage repair surgeries. CASE PRESENTATION: We present the case of a 24-year-old female with persistent anterior knee pain and mechanical symptoms after 5 previous knee surgeries. Preoperative imaging confirmed diffuse, full-thickness

Core Decompression for Osteonecrosis of the Femoral Head.

BACKGROUND: Osteonecrosis, also known as avascular necrosis (AVN), of the femoral head occurs due to a reduced blood supply, leading to bone death, increased intraosseous pressure, and collapse. Primary risk factors include prolonged corticosteroid use, excessive alcohol consumption, and trauma-among various other causes. Core decompression is an effective treatment for relieving pressure and stimulating healing in the early stages of osteoarthritis. INDICATIONS: Core decompression for AVN is indicated for precollapse stages, such as Association Research Circulation Osseous (ARCO) 1 and 2. Ho

TELEnutrition and KIdNey hEalth Study: protocol for a randomised controlled trial comparing the effect of digital health to standard care on serum phoSphate control in patients on dIalysiS (TeleKinesis Study).

INTRODUCTION: Diet and nutrition play a vital role in all stages of chronic kidney disease (CKD) prevention, treatment and management. In particular, dietary interventions are essential to manage hyperphosphataemia, a common metabolic complication in CKD consistently associated with an increased risk of cardiovascular disease and all-cause morbidity and mortality. Unfortunately, dietary management of any kind in this cohort of patients also comes with the added challenge of limited and variable access to renal dietitians, logistical difficulties and multiple medical appointments. Given the com

Femoral and Tibial Derotational Osteotomies With MPFL Reconstruction and Lateral Retinacular Lengthening.

BACKGROUND: Many anatomic factors contribute to patellar instability, including excessive femoral anteversion and external tibial torsion. Derotational osteotomies may be performed concomitantly with medial patellofemoral ligament (MPFL) reconstruction to address multiple factors and improve patellofemoral engagement. INDICATIONS: Proper identification of axial plane abnormalities is important as recurrence of instability may be higher in patients when these factors are not addressed. Femoral anteversion or tibial torsion of >20° can generate abnormal lateral forces in the patellofemoral join

Elbow Arthroscopy for Treatment of Osteochondritis Dissecans of the Capitellum.

BACKGROUND: Osteochondritis dissecans (OCD) of the elbow is an idiopathic lesion of subchondral bone resulting in delamination and sequestration with or without articular cartilage involvement; for patients with persistent symptoms, surgical treatment is often recommended. While traditionally performed in the supine or prone position, arthroscopic debridement and microfracture of OCD of the capitellum in the lateral decubitus position permits strategic arm positioning and placement of portals to optimize visualization and reduce the risk of damage to neurovascular structures. INDICATIONS: Art

Ulnar Nerve Decompression With Subcutaneous Transposition.

BACKGROUND: Medial-sided elbow injuries are becoming increasingly common among throwing athletes due to overuse and increased specialization at early ages. High valgus stress and repetitive elbow flexion/extension during throwing not only affects the ligaments and dynamic support of the elbow, but commonly affects the ulnar nerve. INDICATIONS: Management of cubital tunnel syndrome is initially rest, therapy, and functional training; however, if conservative measures do not appropriately address the ulnar neuropathy, surgical decompression with subcutaneous transposition is a reliable treatmen

Stage I Revision ACL Reconstruction With Allograft Bone Dowels.

BACKGROUND: When evaluating a patient with a failed anterior cruciate ligament (ACL) reconstruction, a detailed history and physical examination is paramount. The position and size of original femoral and tibial tunnels are critical in the surgical decision-making in the setting of revision ACL reconstruction. This video presents a case of stage I revision ACL reconstruction with the use of allograft bone dowels due to increased size and poor position of original fixation tunnels. INDICATIONS: Indications for staging revision ACL reconstruction include significant tunnel osteolysis or dilatio

A New Algorithm for Extra-Articular Reinforcement in ACL Injury Based on Rotational Instability.

BACKGROUND: The results of surgical reconstructions of the anterior cruciate ligament (ACL) are improving with the association of extra-articular reinforcements, such as anterolateral ligament reconstruction and Lemaire tenodesis. However, ACL injury can occur through various mechanisms, and when there are valgus and external rotation of the tibia, anteromedial rotational instability (AMRI) may develop. Articular reinforcement in the anteromedial quadrant, in the topography of the anterior oblique ligament (AOL), can be an important tool for patients with ACL injury and external rotation insta

Arthroscopic-Assisted Core Decompression of Humeral Head Osteonecrosis With Bone Marrow Aspirate Concentrate Augmentation.

BACKGROUND: The proximal humerus is the second-most common anatomic site for osteonecrosis, a condition of disrupted blood supply leading to death of bone tissue, with resultant shoulder pain and dysfunction. Core decompression is a viable joint-preserving surgical intervention that aims to improve local blood supply and restore bone health prior to bony collapse. INDICATIONS: Humeral head osteonecrosis has numerous underlying causes, most commonly the sequelae of prior trauma, chronic use of systemic corticosteroids, sickle cell disease, and alcohol abuse. Core decompression may be indicated

A New Algorithm to Treat Chronic Combined ACL/MCL Injuries: Let's Come Back to the "Rotatory Instability Test".

BACKGROUND: Chronic combined medial collateral ligament (MCL) and anterior cruciate ligament (ACL) injuries are frequent. Medial residual laxity is a risk factor for ACL rerupture. It should be treated at the same time as the ACL reconstruction (ACLR) if necessary, but there are still questions surrounding the indications for abstention or surgery of the medial plan, especially for grade 2 MCL injuries of the Fetto and Marshall classification. INDICATIONS: The purpose is to come back to a simple test, the "Rotatory Instability Test" as described by Slocum and Larson in 1968 for systematic cli

Posterior Shoulder Instability in the Military and Athlete: An Evidence and Experience-Based Treatment Approach.

BACKGROUND: Recognition of posterior glenohumeral instability has increased in young, athletic populations, leading to evolution in operative approaches to management. As with anterior instability surgery, successful treatment for these challenging injuries is dependent on understanding the key principles of pathology and restoration of the functional anatomy. INDICATIONS: Operative management of posterior glenohumeral instability is indicated for recurrent instability events or persistent pain refractory to physical therapy in the setting of posterior labral pathology with or without bone lo

Ulnar Collateral Ligament Reconstruction with Hamstring Autograft.

BACKGROUND: Ulnar collateral ligament (UCL) reconstructions have become increasingly common, particularly in youth overhead throwing athletes. These injuries are most commonly due to overuse and repetitive trauma on the inner elbow. Throwers with a deficient UCL often report decreased pitching speeds in addition to elbow pain and instability. INDICATION: The indications for this procedure include symptomatic valgus elbow instability during overhead throwing motions and a verified rupture of the UCL on advanced imaging. TECHNIQUE DESCRIPTION: The ipsilateral gracilis tendon is harvested and p

Arthroscopic Onlay Biceps Tenodesis Utilizing 2 Knotless All-Suture Anchors.

BACKGROUND: Tenodesis is an effective surgical technique for the management of a pathologic long head of the biceps tendon. Tenodesis is performed with either an onlay (tendon opposed to cortical surface) or inlay (tendon docked into a reamed intra-cortical tunnel). Both techniques can be performed either from a subpectoral location, which is typically done through an open approach, or from a suprapectoral location, which is performed arthroscopically. The arthroscopic suprapectoral approach, when compared with its subpectoral counterpart, has been shown to report similar patient outcomes and

Modified Macintosh Extra-Articular Anterior Cruciate Ligament Reconstruction in a 6-Year-Old Patient.

BACKGROUND: Anterior cruciate ligament (ACL) injury in children is an increasingly common occurrence. Historically, nonsurgical treatment has been the main treatment option; however, the indication for surgical reconstruction is increasing, as the histological characteristics of the immature skeleton are better known. INDICATIONS: The extra-articular surgical technique for ACL reconstruction is a good option for Tanner I patients, aged up to 8 years, with knee instability and recurrent pain. Its advantage in skeletally immature patients is due to the fact that it avoids the bone growth plates

Scapulothoracic Arthroscopy for Snapping Scapula Syndrome.

BACKGROUND: Snapping scapula syndrome (SSS) is a disruption of the normal smooth motion of the scapulothoracic joint leading to clicking or "snapping" which can be painful or painless. There are a variety of etiologies with first-line treatment being conservative. If patients fail extensive nonoperative treatments, then surgery may be considered. Although technically challenging, arthroscopic treatment is recommended due to its decreased morbidity and improved cosmesis. INDICATIONS: We present a 21-year-old man with a 2-year history of mechanical popping and crepitus with overhead and scapulo
