The extended microbiology of group A streptococcal pharyngitis. Observations during a double-blind controlled study of cephalexin twice versus four-times daily.

In a double-blind controlled study we compared the effectiveness of cephalexin b.i.d. versus q.i.d. in the treatment of group A streptococcal pharyngitis in 65 children. Clinical improvement was noted in 64 patients (98%) and bacteriologic cure in 60 (92%). Despite good compliance, three bacteriologic failures were noted in the q.i.d., and two in the b.i.d. treatment groups. Two of these five were carriers. Significant antibody responses were observed in 61% of the patients by at least one of three tests (ASO, anti-DNase B, Streptozyme). We also investigated the extended microbiology of strept

Protocol for a parallel-group superiority randomized controlled trial comparing Sage- a couple psychotherapy- to supportive individual psychotherapy for borderline personality disorder.

BACKGROUND: Although borderline personality disorder (BPD) is associated with substantial mortality and morbidity, frontline BPD interventions are difficult to access and have suboptimal efficacy. Therefore, there is a pressing need to develop and test brief, optimized BPD interventions. BPD is associated with disrupted intimate relationships, and recent theoretical models suggest that such disruptions may play a central role in driving BPD pathology. This led to the development of Sage, a protocolized, conjoint psychotherapeutic intervention delivered to people with BPD and their intimate par

Dose-response evaluation of intravenous carbetocin for prevention of postpartum haemorrhage during elective caesarean delivery: A parallel-group randomised controlled trial.

BACKGROUND AND AIMS: Postpartum haemorrhage (PPH) is a major cause of maternal morbidity and mortality. Although 100 μg intravenous carbetocin is commonly administered for uterotonic prophylaxis at caesarean delivery, the optimal dose remains uncertain. The primary objective was to evaluate the dose-response relationship of intravenous carbetocin in achieving adequate uterine tone during elective caesarean delivery. METHODS: In this randomised, parallel-group, dose-response trial, 100 parturients scheduled for elective caesarean delivery were assigned into five groups to receive intravenous c

Physical Activity and Recreation for Kids Study (PARKS): Park-intensive community outreach for child physical activity group-randomized controlled trial.

BACKGROUND: Park use among urban children can contribute to higher child physical activity (PA), yet most children do not use parks or park sports programs. The PARKS (Physical Activity and Recreation for Kids Study) group randomized controlled trial evaluated the effect of park-level intensive community outreach on change in child MVPA, light and sedentary activity. It was hypothesized that children in the intervention parks would increase their MVPA and decrease their sedentary activity compared to children in the usual parks. METHODS: Twenty urban parks were randomized to intensive park ou

Evaluation of a transdiagnostic mental health intervention in German primary care: a parallel-group, two-arm, cluster randomised controlled pilot study.

INTRODUCTION: Adequate mental health treatment in primary care (PC) is often hindered by structural factors, such as time constraints, and the high comorbidity of mental health conditions. A transdiagnostic approach may help address these challenges, yet evidence on interventions delivered by general practitioners (GPs) remains limited. This pilot study therefore assessed the feasibility, acceptability and potential effectiveness of a GP-led transdiagnostic intervention. METHODS: PC practices in Munich (Germany) were randomised (1:1) to an intervention group (IG) or a control group (CG). In t

Inspiratory Muscle Training in Older Patients With Acute Heart Failure: Design and Rationale of a Multi-Arm Parallel-Group Randomized Controlled Trial.

BACKGROUND: Approximately 70% of older patients with acute heart failure (AHF) exhibit inspiratory muscle weakness (IMW). Inspiratory muscle training (IMT) may improve respiratory function and functional capacity, yet evidence regarding its efficacy, safety, and optimal dosing in this population remains limited. The IMT-AHF trial aims to verify the efficacy and safety of IMT in older patients with AHF. METHODS AND RESULTS: The IMT-AHF trial is a single-center, multi-arm, parallel-group randomized controlled trial enrolling older patients aged ≥65 years with AHF who have IMW and can ambulate i

Controlling FDR in selecting group-level simultaneous signals from multiple data sources with application to the National COVID Collaborative Cohort data.

One challenge in exploratory association studies using observational data is that the associations between the predictors and the outcome are potentially weak and rare, and the candidate predictors have complex correlation structures. False discovery rate (FDR) controlling procedures can provide important statistical guarantees for replicability in predictor identification in exploratory research. In the recently established National COVID Collaborative Cohort (N3C), electronic health record (EHR) data on the same set of grouped candidate predictors are independently collected in multiple diff

Th17 cells require the DNA repair sensor xeroderma pigmentosum complementation Group C to control oxidative DNA damage in a murine model.

T helper 17 cells play essential roles in mucosal immunity and autoimmunity, yet the mechanisms that protect these cells from oxidative DNA damage remain poorly defined. Here we show, in a murine model, that the nucleotide excision repair sensor Xeroderma Pigmentosum Complementation Group C preserves genomic stability and metabolic fitness during T helper 17 cell differentiation. Loss of this factor reduces interleukin 17 production and increases mitochondrial reactive oxygen species and oxidative DNA damage, resulting in altered metabolic programs. Mechanistically, Xeroderma Pigmentosum Compl

A digital therapeutic for people with borderline personality disorder in Germany (EPADIP-BPD): a pragmatic, assessor-blind, parallel-group, randomised controlled trial.

BACKGROUND: Fewer than 25% of patients with borderline personality disorder (BPD) receive guideline-adherent psychotherapy. Digital therapeutics can help reduce this gap. Therefore, we tested the effectiveness and safety of priovi, a digital therapeutic for BPD. METHODS: We conducted a pragmatic, assessor-blind, parallel-group, randomised controlled trial, recruiting patients with BPD, according to DSM-5 classification, of at least moderate severity on the 23-item Borderline Symptom List (BSL-23) via online advertisements in Germany. Patients were aged 18 years or older, agreed on an emergenc
