[Structure and function of the microcirculatory bed in the preserved heart].

The morphological analysis of the state of the heart during hypertermal perfusion with different conservants reveals clear dependence of the microcirculation and the activity of the heart upon the type of the conservant. Perfusion with a salt solution and hemodilution is accompanied by pronounced disorders in microcirculation and unsatisfactory parameters of the cardiac activity. Conservation with cryoprecipitated plasma is characterized by comparatively less microcirculatory disorders, but fails to give a reliable safety of the heart. When using medium 199, changes in microcirculation were fo

Coronary Spasm Testing with Acetylcholine: A Powerful Tool for a Personalized Therapy of Coronary Vasomotor Disorders.

Coronary vasomotor disorders (CVD) are characterized by transient hypercontraction of coronary vascular smooth muscle cells, leading to hypercontraction of epicardial and/or microvascular coronary circulation. CVDs play a relevant role in the pathogenesis of ischemia, angina and myocardial infarction with non-obstructive coronary arteries. Invasive provocative testing with intracoronary Acetylcholine (ACh) administration is the gold standard tool for addressing CVD, providing relevant therapeutic and prognostic implications. However, safety concerns preclude the widespread incorporation of the…

Isolated aortic valve stenosis in the eighth decade.

Aortic valve stenosis was evaluated in 26 patients in the eighth decade of life, representing 20% of all patients referred for aortic valve stenosis. There were no clinical features which distinguished this older group from younger patients. Coronary artery disease was present in 46% of the older group, 30% in the younger. Angina pectoris or an infarction pattern on the electrocardiogram was not useful in predicting coronary artery disease in either group. A dominant left coronary circulation was more frequent (22%) in the younger than the older group. This was related to the higher incidence …

Renal vascular system in right heart overload with varying circulatory compensation.

To determine the characteristics of renal vascular system adaptation to venous congestion, the right heart overload was induced in 32 puppies by surgical narrowing of the pulmonary trunk. The animals were followed up over the period as long as 2 years. Twenty-three puppies showed compensation of the functionally overloaded right heart ventricle. The signs of circulatory decompensation with exudative processes in the pericardium, thoracic and abdominal cavities were recorded in 9 animals. Ten dogs of the appropriate age were used as control. During experiments, all group animals underwent measu

Microcirculatory dysfunction in the heart and the brain.

Coronary microcirculatory dysfunction (CMD) is a major cause of myocardial ischemia that influences the outcomes of patients with coronary artery disease. The mechanisms of CMD are heterogeneous and may result from a spectrum of biological and cardiovascular risk factors that may affect also the microcirculation of other vital organs. Microcirculatory dysfunction of the brain, known as cerebral small vessel disease, is increasingly being recognized as a cause of cognitive decline and neurodegenerative disorders. Despite microvascular dysfunction of the heart and the brain may share underlying

Isolated anomalous origin of the right coronary artery from the pulmonary artery in an asymptomatic 12-year-old girl: role of MRI in depicting the anatomy, detecting the ischemic burden, and quantifying the amount of left-to-right shunt.

Asymptomatic 12-year-old girl with a heart murmur underwent echocardiogram and suspicious continuous flow was detected in her interventricular septum. She underwent cardiac magnetic resonance (CMR) for further investigation. The CMR images demonstrated anomalous origin of the right coronary artery from the pulmonary artery. Adenosine stress perfusion scan showed an inducible perfusion defect in the right coronary artery and left circumflex artery territories. Flow quantification showed a left-to-right shunt with pulmonary to systemic blood flow ratio (Qp/Qs) of 1.25.

The role of collateral blood supply in ventricular tachycardia after myocardial infarction.

The hypothesis that collateral or anterograde blood supply to an infarcted area maintains blood supply to cells responsible for ventricular tachycardia after myocardial infarction was studied in six patients. All patients had suffered a myocardial infarction and developed spontaneous episodes of sustained monomorphic ventricular tachycardia. The arrhythmia was paroxysmal in three patients and incessant in the other three. During ventricular tachycardia iced isotonic saline (10 ml in approximately 4 s) was injected first in the coronary artery ostia and thereafter superselectively in the corona

Clinical and angiographic correlates of dobutamine-induced wall motion patterns after myocardial infarction.

The ability of different dobutamine-induced wall motion patterns to define the anatomic status of the infarct-related artery (IRA) was evaluated in 159 patients who underwent dobutamine stress echocardiography (DSE) and coronary angiography 10 +/- 2 and 18 +/- 3 days, respectively, after hospital admission. The DSE result was classified as: (1) biphasic: improvement with a low dose followed by deterioration with a high dose; (2) worsening: direct deterioration at low or high doses; (3) sustained improvement: improvement with a low dose that was maintained at high dose; and (4) no change: no ch

Significance of reduced regional myocardial blood flow in asynergic areas evaluated with intervention ventriculography. Results of studies combining washout of xenon-133 and postextrasystolic potentiation.

Nineteen patients with coronary artery disease were studied to determine the significance of reduced regional myocardial blood flow (50 ml/min per 100 g or less) in areas of abnormal wall motion. Regional myocardial blood flow was measured in four regions of the left ventricle with an Anger camera after the injection xenon-133 into the left main coronary artery. Abnormal wall motion was evaluated with biplane left ventriculography at rest and during postextrasystolic potentiation, a potent inotropic stimulus. Abnormal wall motion was defined as hemiaxis shortening of less than 20 percent. Four

Anaesthetic management of a child with anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) and supravalvular aortic stenosis (SVAS) associated with Williams-Beuren syndrome undergoing combined coronary reimplantation and supravalvular aortoplasty.

A child with genetically confirmed Williams-Beuren syndrome, having moderate supravalvular aortic stenosis, was diagnosed with anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), a rarely described combination. This coexistence created competing physiological stress on coronary perfusion as myocardial oxygen delivery to the left ventricle depended on right coronary collateral circulation in the presence of fixed left ventricular outflow obstruction. The child underwent combined surgical repair with reimplantation of the left coronary artery into the aorta and supra

[Evaluation of coronary artery fistula using a color Doppler flow imaging technique].

Two unusual cases with coronary artery fistula were reported. The diagnoses were first made by color Doppler flow imaging and confirmed by selective coronary angiography and operation. In the first case, color Doppler revealed a continuous flow signal within the dilated left coronary artery and a turbulent jet within the right atrium. In the second case, color Doppler showed turbulent flow signals which could be traced from the proximal left coronary artery, along the left ventricular posterior wall and into the left ventricle. The high velocities at the site of drainage of fistula were record

Therapeutic interventions for coronary microvascular obstruction after acute myocardial infarction: Developments and challenges.

ST-segment elevation myocardial infarction (STEMI) remains a major global cause of mortality and heart failure, even in the era of primary percutaneous coronary intervention (PCI). While timely epicardial reperfusion is routinely achieved, a substantial subset of patients experiences impaired myocardial perfusion due to coronary microvascular obstruction and dysfunction (MVO). MVO is a complex and multifactorial process driven by ischemia-reperfusion injury, endothelial dysfunction, distal embolization, inflammation, and emerging mechanisms such as pericyte-mediated capillary constriction. MVO

Effect of hemodynamics on clot formation and flow cessation in patient-specific coronary bifurcations.

Hemostasis prevents bleeding by forming clots, although disruptions can cause insufficient or excessive clotting. Shear stress is a critical factor influencing the normal function of the blood and can stimulate the formation of unnecessary clots. Stenosed vessels significantly contribute to this phenomenon by increasing shear stress on the vessel walls. This study aims to quantify the effects of shear stress and stenosis severity on thrombus formation in patient-specific left coronary bifurcations. Three-dimensional patient-specific geometries were reconstructed from angiographic data. Blood f

Coronary microvascular dysfunction endotypes: IMR tips and tricks.

Coronary microvascular dysfunction (CMD) is an important clinical disease spectrum which has gained widespread attention due to chronic anginal symptoms, and worse clinical outcomes, with or without obstructive coronary artery disease (CAD). Coronary microcirculatory dysfunction is due to a wide array of mechanisms such as inflammation, platelet aggregation, vessel wall collagen deposition, imbalance of nitric oxide, free radicals, and sympathetic/parasympathetic simulation. As noted in this supplement, CMD can occur as a primary disease or co-exist with multi-array of diverse cardiac conditio

Absolute coronary blood flow measurement and the principle of microvascular resistance reserve.

Measurement of absolute coronary blood flow and myocardial resistance by continuous intracoronary thermodilution was recently introduced and facilitates assessment of the coronary microcirculation. In addition, Microvascular Resistance Reserve (MRR) is a new index that further permits the characterization of coronary microcirculatory function. It can be calculated from resting and hyperemic absolute coronary flow and resistance, measured during continuous thermodilution. By being independent of both epicardial disease and myocardial mass, MRR is highly specific for the microvascular compartmen
