Disease spectrum and genetic profile of high-risk children with severe carnitine deficiency.

BACKGROUND: Severe free carnitine deficiency (C0 ≤5 μmol/L) is frequently observed in pediatric patients suspected of primary carnitine deficiency (PCD), though confirmed diagnoses remain rare. This study aimed to analyze the disease spectrum and genetic profile of high-risk children with severe reduced carnitine levels and to enable targeted treatment. METHODS: We retrospectively analyzed blood acylcarnitine profiles (ACP), urine organic acids, genetic results, and clinical features of high-risk pediatric patients with significantly low C0 levels, which led to a diagnosis. RESULTS: Among 56

Does ACE2 deficiency have a role in Parkinson's disease -exacerbated pulmonary fibrosis?

Pulmonary fibrosis is a common and life-threatening complication of Parkinson's disease (PD), yet the molecular mechanisms linking the two diseases remain unclear, creating a critical gap in targeted therapeutic strategies for comorbid patients. Angiotensin-converting enzyme 2 (ACE2) plays a key role in neuroprotection and lung homeostasis; its deficiency exacerbates PD-related neuroinflammation and α-synuclein aggregation, while also promoting pulmonary inflammation and fibrotic remodeling. Clarifying how ACE2 deficiency drives PD-exacerbated pulmonary fibrosis is therefore an urgent unmet ne

Association between FNDC5 Deficiency and Compromised Mitochondrial Integrity and Biogenesis in Kidney Disease.

BACKGROUND: Mitochondrial dysfunction is a hallmark of acute and chronic kidney diseases. This study investigates the expression landscape and potential association of FNDC5 with renal injury. METHODS: We integrated human scRNA-seq, mouse spatial transcriptomics (ischemia-reperfusion injury model), in vitro high-glucose (HG) models in proximal tubule cells and podocytes, and a constitutive Fndc5 knockout mouse model. Candidate transcription factor occupancy at the FNDC5 promoter was examined by ChIP-qPCR, and associations were assessed in a non-insulin-dependent diabetes mellitus mouse model.

Trafficking Deficiency of TMEM175 Variants in Parkinson's Disease Pathogenesis and the Prospects of Precision Medicine.

The transmembrane protein 175 (TMEM175) is a high-risk gene for Parkinson's disease (PD) and encodes an ion channel in lysosomes. Many PD-associated variants in TMEM175 lead to the loss of ion channel function. In this study, we identified the PD-associated TMEM175-L156P variant is a missense mutation with trafficking deficiency. Using imaging, biochemical, and whole-endolysosomal electrophysiological approaches, we identified that L156P abolished the lysosomal expression of TMEM175; instead, it accumulates in the endoplasmic reticulum (ER). The cytosolic segment of the fourth transmembrane he

The Vicious Cycle of Diabetic Kidney Disease, Vitamin D Deficiency, and Arterial Hypertension.

Diabetic kidney disease (DKD) is a major complication of diabetes mellitus that contributes substantially to chronic kidney failure and increased cardiovascular risk. Beyond progressive deterioration of renal function, DKD is associated with disturbances in endocrine and vascular regulation. Among these, alterations in vitamin D homeostasis and blood pressure (BP) control represent clinically relevant, yet incompletely integrated aspects of DKD pathophysiology. This narrative review synthesizes current evidence on the multidirectional relationships between DKD, vitamin D deficiency, and arteri

Celiac disease in children with refractory iron deficiency anemia: a prospective observational study.

Celiac disease (CD) is an immune-mediated enteropathy triggered by dietary gluten in genetically susceptible individuals. Refractory iron deficiency anemia (RIDA) may be the sole presentation of CD in children. This study aimed to investigate the prevalence of CD in this group of patients. This prospective observational study was conducted over a period of 1 year at a tertiary care center of North India. Children aged 6 months to 14 years with refractory IDA were enrolled. Those with positive serology for CD underwent upper gastrointestinal endoscopy and histopathological examination. Out of 6

Case report: IL12RB1 deficiency in an infant with disseminated Bacille Calmette-Guérin disease.

Disseminated Bacille Calmette-Guérin (BCG) disease is a rare adverse reaction to BCG vaccination. We report a neonate presenting with a progressive left axillary mass 3 months post-vaccination. High-throughput sequencing confirmed Mycobacterium bovis (BCG strain) infection. Genetic testing confirmed inborn errors of immunity (IEI) caused by interleukin (IL) 12 receptor β1 deficiency. An expert panel diagnosed BCG disease (acute hematogenous disseminated BCG disease and axillary lymph node involvement) due to the immunodeficiency. After about 28 months of anti-BCG combination therapy with inter

Transcriptome analysis of liver injury of fatty liver disease induced by ALDH2 deficiency.

Aldehyde dehydrogenase 2 (Aldh2) Glu504Lys mutation, common in East Asians, is linked to various alcohol-related pathologies, notably fatty liver disease. Recent findings suggest that high ethanol-producing Klebsiella pneumoniae(HiAlc Kpn) exacerbates liver injury in non-alcoholic fatty liver disease (NAFLD). Our study investigated the combined effects of Aldh2 deficiency and HiAlc Kpn on NAFLD liver injury, transcriptome analyses to unearth potential mechanisms and therapeutic targets. In our controlled experiment with Aldh2-deficient mice, we induced fatty liver via alcohol and HiAlc Kpn gav

HNF4A Regulated APEH Deficiency Promotes UPR Activation in Diabetic Kidney Disease.

Diabetic kidney disease (DKD) constitutes a severe microvascular complication of diabetes and ranks among the leading causes of end-stage renal disease (ESRD). Endoplasmic reticulum stress (ERS) significantly contributes to the initiation and progression of DKD, as persistent ERS leads to excessive activation of the unfolded protein response (UPR), triggering cellular apoptosis. Protein carbonylation, a prominent oxidative modification caused by reactive oxygen species (ROS), is recognized as a critical initiator of ERS. Acyl peptide enzyme hydrolase (APEH) demonstrates cytoprotective properti

Evaluation of Ficolin-3 deficiency as a risk factor in the development of rheumatic heart disease.

OBJECTIVE: Ficolin-3 is a crucial protein for the activation of the complement system. Previous work has indicated this protein may play a role in the pathogenesis of rheumatic heart disease (RHD), and it has been hypothesised that ficolin-3 has potential as a biomarker for early identification of patients with suspected RHD. This study investigated FCN3 gene polymorphisms rs532781899 (c.349del) and rs4494157 (c.658 + 250 C > A) and ficolin-3 serum concentrations in an ethnically diverse cohort of 53 RHD cases and 45 healthy controls from across Africa. RESULTS: Ficolin-3 was found to be incr

Brain accumulation of lactosylceramide characterizes GALC deficiency in a zebrafish model of Krabbe disease.

Krabbe disease (KD) is an autosomal recessive sphingolipidosis due to mutations of the GALC gene encoding for the lysosomal β-galactosylceramidase (GALC) that removes β-galactose from β-galactosylceramide, β-lactosylceramide (LacCer) and the neurotoxic metabolite β-galactosylsphingosine (psychosine). At present, the accumulation of psychosine is thought to be the main cause of demyelination, neurodegeneration and neuroinflammation that characterize the early infantile KD with a 1.5-2-year median survival. Currently, the standard of care of KD is haematopoietic stem cell transplantation which,
