An update on the management of hay fever in adults.

Allergic rhinitis is a common disorder occurring in about one in four people in Britain with a peak onset during adolescence.1-3 Although not necessarily a serious illness, it can adversely affect quality of life and disrupt normal activities, and is a risk factor for asthma.2,3 The symptoms of seasonal allergic rhinitis/rhino-conjunctivitis caused by an IgE-mediated type 1 hypersensitivity reaction to airborne allergens, particularly pollens, and which typically occur between spring and autumn are commonly referred to as hay fever.3 There are a number of management options available including

Effect of confounding cofactors on responses to pollens during natural season versus pollen challenge chamber exposure.

BACKGROUND: The severity of allergic rhinoconjunctivitis (AR) symptomatology elicited after exposure to pollen in the absence versus the presence of confounding cofactors, such as in a pollen challenge chamber (PCC) and the natural pollinating season, respectively, might differ. OBJECTIVE: We sought to determine the correlation of AR severity in the natural season versus out-of-season PCC exposures. METHODS: Twenty-four Virginia live oak (VLO)-positive, 14 VLO-negative, 16 mountain cedar (MC)-positive, 8 MC-negative, and 26 ragweed-positive participants recorded AR symptoms (total symptom sc

Successful desensitization to docetaxel after severe hypersensitivity reactions in two patients.

PURPOSE: Two cases of successful desensitization to docetaxel after severe hypersensitivity reactions are reported. SUMMARY: Two patients with gynecological malignancies (uterine leiomyosarcoma and ovarian adenocarcinoma) experienced severe hypersensitivity reactions with docetaxel, including flushing, numbness, sharp radiating pain, severe nausea and vomiting, apnea, and unresponsiveness. Both patients received ondansetron before docetaxel. One patient received dexamethasone, diphenhydramine, and famotidine premedication before docetaxel, as she had previously reacted to paclitaxel. Docetaxe

Overview of penicillin allergy.

Allergy to penicillin is the most commonly reported antibiotic allergy. However, most patients who report a positive history of a prior reaction to penicillin are not found to be allergic to penicillin upon skin testing. Often, this history is vague or based on a parent's recollection of an event that occurred in the distant past. Avoidance of penicillin based on self-reported allergic history alone often leads to the use of an alternate antibiotic with greater cost or side effect profile. Patients with a negative skin test to both major and minor determinants may generally be given penicillin

Aspirin desensitization for ASA triad patients--prospective study of the rhinologist`s perspective.

UNLABELLED: OBJECTIVES/PROBLEM: To determine the sinonasal effect of aspirin salicylic acid (ASA) desensitization in patients with nasal polyps, asthma and aspirin intolerance (ASA triad). METHODS OF STUDY: Patients with ASA triad were recruited from the outpatient otolaryngology clinic. They underwent a program of ASA desensitization (2005 - 2008) with prospective assessment of subjective and objective responses. Incremental doses of aspirin were given to reach a target of 625 mg twice daily during a period of 3 - 5 days. A maintenance dose was then given for the study period. The patients a

Improvement of shrimp allergy after sublingual immunotherapy for house dust mites: a case report.

The appropriateness of house dust mite specific immunotherapy in patients allergic to shrimps still remains unclear We present a clinical case as an immunological model for the strong sensitization to tropomyosin with symptoms of anaphylaxis due to shrimps and coexisting asthma due to house dust mite. The improvement in respiratory symptoms for house dust mite and in the food challenge for shrimps during mite immunotherapy with a known and high dosage of tropomyosin suggests the hypothesis that efficacy of mite immunotherapy in food allergy to tropomyosin may be dose dependent.

Frequency of acute systemic reactions in patients with allergic rhinitis and asthma treated with sublingual immunotherapy.

BACKGROUND: Several studies have demonstrated the efficacy and safety of sublingual immunotherapy in the treatment of respiratory allergy. OBJECTIVE: To determine the frequency of systemic adverse reactions in patients treated with standardized extracts of sublingual immunotherapy. METHODS: Allergic patients with rhinitis with or without asthma and sensitized to at least 1 allergen were included. Increasing doses of standardized allergens were administered until reaching an average dose accumulated of 7,200 U after 26 weeks of treatment. Adverse events were graded according to the European A

Management of patients with a history of allergy to beta-lactam antibiotics.

Patients with a history of penicillin allergy pose a treatment dilemma. Unnecessary avoidance of this relatively nontoxic class of drugs exposes the patient to potentially more toxic drugs, increases health care costs, and contributes to the development of antibiotic resistance. Yet for those who truly have allergy or other serious adverse reactions to beta-lactams, the use of alternate drugs is a must. This article reviews current management strategies to determine which patients are good candidates for reintroduction of beta-lactams and which patients should continue avoidance.

Successful administration of a 1-day imported fire ant rush immunotherapy protocol.

BACKGROUND: Young children with a history of systemic reactions to imported fire ant (IFA) stings are at substantial risk of recurrent stings because of their maturational inability to practice appropriate avoidance techniques. OBJECTIVE: To present 3 cases in which patients 36 months or younger completed a 1-day rush immunotherapy (RIT) protocol with IFA whole-body extract (WBE). METHODS: The 1-day RIT protocol used for these patients was modified from the Wilford Hall 2-day rush protocol previously published. A 1:1 vol/vol maintenance vial consisted of 1 mL of IFA WBE and 9 mL of human ser

The dilemma of the negative skin test reactors with a history of venom anaphylaxis: will this always be the case?

The loss of sensitization over time, the involvement of a different pathogenetic mechanism and the poor sensitivity of diagnostic tests have been included among the causes of a non IgE-mediated anaphylaxis triggered by an insect sting. To provide further insight into this topic we describe the case of a patient suffering from urticaria pigmentosa, anaphylactic shock due to a Vespid sting and monosensitive to Polistes dominulus venom, who was previous diagnosed with non-IgE mediated insect sting anaphylaxis in subjects with urticaria pigmentosa. Therefore ultra-rush specific immunotherapy using

Latex allergy: diagnosis and management.

Latex allergy is an IgE-mediated immediate hypersensitivity response to natural rubber latex (NRL) protein with a variety of clinical signs ranging from contact urticaria, angioedema, asthma, and anaphylaxis. Major allergens include dipped latex products such as gloves and balloons. In highest risk for NRL allergy are patients with spina bifida, but health care workers and others who wear latex gloves are also at risk. NRL allergic patients may also react to fruits/foods, especially banana, kiwi, and avocado. Diagnosis is made by a positive latex RAST and/or skin prick test or challenge test t

Incidence of resensitization after tolerating penicillin treatment in penicillin-allergic patients.

The incidence of resensitization to penicillin in patients with a history of penicillin allergy appears to be low but the approach to patients is controversial. We sought to determine the incidence of penicillin resensitization in patients with a history of penicillin allergy who had negative skin tests to penicillin determinants and received an initial uneventful course of beta-lactam antibiotics. We identified 254 patients who had negative penicillin skin testing to the major and minor penicillin determinants from January 1, 1994 to October 1999 and then reviewed their hospital and/or clinic

Immunotherapy with a depigmented, polymerized vaccine of Olea europaea pollen allergens. Significantly reduces specific bronchial and skin test reactivity in sensitized patients after one year of treatment.

Clinical manifestations after the inhalation of Olea europaea pollen are very frequent in Spain. Forty-five patients with a clinical history of asthma and sensitivity to O. europaea pollen were included in a randomized trial to evaluate the safety and the clinical efficacy of immunotherapy with a new chemically modified extract (depigmented and glutaraldehyde-polymerized) of Olea europaea. The study was conducted following good clinical practices and appropriate consent forms were signed. Patients were divided into three groups of 15 individuals: Group A received a maximum concentration of 44

Patients with negative skin tests.

PURPOSE OF REVIEW: The aim of this article is to present and discuss the clinical problem of systemic anaphylaxis to Hymenoptera venoms in patients without detectable immunoglobulin E, as it appears in recent literature. Reported at variable frequency in large series of patients undergoing evaluation, systemic anaphylaxis was previously considered to reflect lost sensitization or to involve non-immunoglobulin E mediated mechanisms. Sporadic case-reports drew attention to the fact that severe or even fatal reactions may occur in patients with negative skin tests. RECENT FINDINGS: A breakthroug

Allergy to insect stings. A need for improved preventive management.

Allergy to insect venom is a major health problem for a significant number of people. Immunotherapy can reduce the risk of subsequent reaction from about 60% to less than 5%. Standard preventive care should include (1) advice concerning avoidance of insects, (2) prescription of an epinephrine kit or syringe for self-administration (unless medically contraindicated), and (3) referral for evaluation. Results of several studies from various regions of the country raise concern about the preventive care and advice given these patients and suggest a need for continuing medical education to improve

[Allergen immunotherapy with an immunochemically characterized grass pollen extract: change in follow-up sensitization parameters].

A perennial hyposensitization treatment over 2 years was performed in 18 pollen-allergic patients (14 children, 4 adults) with a standardized and purified graspollen extract. The selection for therapy was provided by allergen titration in skin prick test, conjunctival test and by the estimation of specific IgE to timothy. The reported decrease of seasonal symptoms was accompanied by reduced sensitivity in skin test and conjunctival provocation test (p less than 0.001, p less than 0.05). No consistent change in specific IgE was observed at the end of the study. Specific IgG antibodies significa
