Does prior authorization of sibutramine improve medication compliance or weight loss?

OBJECTIVE: This study was designed to examine whether prior authorization for insurer reimbursement of weight loss medication affects compliance with taking sibutramine or adherence to a medical weight control program. The underlying hypothesis is that physician advocacy through prior authorization increases patient compliance and treatment outcomes. RESEARCH METHODS AND PROCEDURES: A retrospective review was conducted of 22 subjects who had received a prescription for sibutramine that was reimbursed through their health insurer by prior authorization (PAR) and compared them with 47 randomly

Predictors of low bone density in young adolescent females with anorexia nervosa and other dieting disorders.

OBJECTIVE: To compare the bone density of adolescent patients with anorexia nervosa with adolescent patients with other dieting disorders and to evaluate risk factors for low bone density in these patients. METHOD: Sixty-nine consecutive female patients referred to an adolescent eating disorders clinic were studied by interview, blood sampling, body composition, and lumbar spine bone density measurement using dual energy X-ray absorptiometry. RESULTS: Although patients with anorexia nervosa were more malnourished, their bone density was similar to other dieting patients. Patients were divide

Patients with bulimia nervosa who fail to engage in cognitive behavior therapy.

Although cognitive behavioral treatment is the treatment of choice in bulimia nervosa, patients' response is variable. A minority of patients do not respond at all and some never engage in treatment. This paper concerns the latter group. A case series of six such patients with whom treatment could not be initiated is compared with a group who received a full course of treatment. The group with whom treatment could not begin were found to have a longer history of disorder, to report excessive laxative abuse, to have more severe depressed mood and a greater dissatisfaction with their body weight

Personality and family features of adolescent girls with eating symptoms: evidence for restricter/binger differences in a nonclinical population.

Conflicting evidence exists concerning a "restricter/binger" dichotomy aligned with personality traits of obsessionality versus impulsivity, and family traits of enmeshment versus incohesion. The present study explored relationships among reported personality and family traits, on the one hand, and subtypes of eating symptoms, on the other, in a sample of 715 high-school girls. Symptomatic eaters (most displaying subclinical eating problems) consistently displayed more Mood Problems, Body Concerns, and Self-Criticism than did asymptomatic girls. Differences were observed between restrictive ea

Long-term effects of a psychologically based group programme for women preoccupied with body weight and eating behaviour.

The effects of a 10 session group programme designed to discourage dieting behaviour and reduce preoccupation with body weight, food and eating were assessed in 87 women (mean age 40 years and mean body mass index 31.9) who attended three different agencies in Adelaide, South Australia. The subjects were selected on the basis of eating preoccupation and not body weight. Measurements of various psychological parameters and body weight were made at entry to the programme, at the completion of the programme and at 6 months, 1 year and 2 years after its completion. Eighty women completed the group

[Evaluation and treatment of obesity in clinical practice].

Obesity continues as before to be a widespread condition. Obesity is defined as a body weight of over 120% of the ideal weight, corresponding roughly to the 85th percentile of the weight distribution. According to the "Build Study" (1979), the ideal weight is assumed to be rather higher than formerly; in men it is 8%, in women 6% less than the so-called normal weight. The latter corresponds roughly to the average weight and is defined as: height (cm) minus 100 in kg. In obese subjects both somatic and psychological complications arise; these are related exponentially to the degree of overweigh

Blinding, unblinding, and the placebo effect: an analysis of patients' guesses of treatment assignment in a double-blind clinical trial.

We administered a questionnaire to assess maintenance of patients' blindness at the end of a double-blind clinical trial of Osmotic Release Oral System phenylpropanolamine (PPA) vs. placebo in mild obesity. Seventy-four percent of placebo participants and 43% of PPA participants guessed their treatment correctly. Appetite control was the most frequently reported basis for guessing PPA, even by placebo participants. Lack of adverse drug reactions was the most frequently reported basis for guessing placebo, even by PPA participants. Participants receiving either PPA or placebo and guessing PPA l

Restraint among bulimic women.

In the present investigation, the scores of bulimic women on the Restraint, Social Desirability, and Lie scales were compared with the scores of obese dieters, obese nondieters, and nondieters whose weights were within normal limits. Bulimics attained significantly higher restraint scores than obese nondieters and normals. These results are discussed within the context of models of bulimia nervosa.

Severe energy restriction in treatment of toxemia of pregnancy.

A total of 78 patients with toxemia pregnancy were treated for over a week before delivery. The treatment was characterized by (1) mental and physical rest, (2) disuse of hypotonic, diuretic, and antispasmodic agents, and (3) a diet with reduced salt and energy (200-1200 Cal/day). Except for one fetal death, which occurred during delivery, all mothers and their babies left the hospital in good condition. No fetal or neonatal deaths that were related to the maternal condition or severity of toxemia were encountered. The long-lasting complications of toxemia were lower than those reported by oth

Blunted inflammatory response is associated with a lower response to a weight loss dietary intervention in liver recipients.

BACKGROUND & AIMS: Obesity is associated with chronic low-grade inflammation, and adipose tissue inflammation is required for fatty tissue remodeling. Interestingly, immunosuppressed patients, as liver transplant recipients, often experience excessive weight gain. We investigated how liver recipients' inflammatory response affects body weight loss induced by dietary treatment. METHODS: Overweight liver recipients were paired with non-transplanted subjects to compare their peripheral immune profiles. RESULTS: Transplanted patients had similar profiles of peripheral blood mononuclear cells com

Association between diabetes risk reduction diet score and risk of breast cancer: A case-control study.

PURPOSE: To assess the association between the diabetes risk reduction diet score (DRRD) and the odds of breast cancer (BC). METHODS: During this hospital-based case-control study we included 149 newly diagnosed cases of BC and 150 controls matched by age. All cases were patients with pathologically confirmed BC, with no history of any form of other cancers. The controls were randomly selected from visitors and families of non-cancer patients in other wards of the same hospital who had no health issues including BC. The dietary intakes were evaluated by a validated 147-item semi-quantitative

Dieting and Disinhibited Eating Patterns in Adult Women with Normal Body Weight: Does Rumination Matter?

Dieting and disinhibited eating patterns are presented in both clinical and nonclinical samples. Repetitive negative thinking (i.e., rumination) may lead to maladaptive eating behaviors. While numerous studies have focused on dieting and disinhibited eating behaviors in clinical samples, less is known about these behaviors in nonclinical samples with normal body weight. Therefore, the present study aimed to explore how dieting, uncontrolled eating and emotional eating are related to rumination in adult women with normal body weight. One hundred eighty-eight women (Mage = 29.46 ± 8.94; MBMI = 2

Effect of diet-induced weight loss on cytokeratin-18 levels in overweight and obese patients with liver fibrosis.

AIMS: Liver biopsy is currently the gold standard test for NAFLD diagnosis and staging but has many drawbacks. In addition, other tools such as transient elastography are limited to specialized research centers. To assess the usefulness of CK-18 as a non-invasive biomarker for detecting therapeutic responses in patients with liver fibrosis. MATERIALS AND METHODS: Sixty overweight and obese patients with liver fibrosis were evaluated by a dietitian and given a weight-reducing diet with a calorie deficit of 500-1000 kcal/day over a 6-month period. Controlled attenuation parameter (CAP) and live

Weight loss improves disease activity in patients with psoriatic arthritis and obesity: an interventional study.

BACKGROUND: Obesity is over-represented in patients with psoriatic arthritis (PsA) and associated with higher disease activity, poorer effect of treatment and increased cardiovascular morbidity. Studies on the effects of weight loss are however needed. This study aimed to prospectively study the effects of weight loss treatment with very low energy diet (VLED) on disease activity in patients with PsA (CASPAR criteria) and obesity (body mass index BMI ≥ 33 kg/m2). METHODS: VLED (640 kcal/day) was taken during 12-16 weeks, depending on pre-treatment BMI. Afterwards, an energy-restricted diet wa

A systematic review of commercial weight loss programmes' effect on glycemic outcomes among overweight and obese adults with and without type 2 diabetes mellitus.

OBJECTIVE: We examined the glycemic benefits of commercial weight loss programmes as compared with control/education or counselling among overweight and obese adults with and without type 2 diabetes mellitus (T2DM). METHODS: We searched MEDLINE, Cochrane Database of Systematic Reviews, and references cited by individual programmes. We included randomized controlled trials of ≥12 weeks duration. Two reviewers extracted information on study design, population characteristics, interventions, and mean changes in haemoglobin A1c and glucose. RESULTS: We included 18 randomized controlled trials. Focusing on media body ideal images triggers food intake among restrained eaters: a test of restraint theory and the elaboration likelihood model.

Although research consistently shows that images of thin women in the media (media body ideals) affect women negatively (e.g., increased weight dissatisfaction and food intake), this effect is less clear among restrained eaters. The majority of experiments demonstrate that restrained eaters - identified with the Restraint Scale - consume more food than do other participants after viewing media body ideal images; whereas a minority of experiments suggest that such images trigger restrained eaters' dietary restraint. Weight satisfaction and mood results are just as variable. One reason for these
