Changes in bone marrow lesions in response to weight-loss in obese knee osteoarthritis patients: a prospective cohort study.

BACKGROUND: Patients are susceptible for knee osteoarthritis (KOA) with increasing age and obesity and KOA is expected to become a major disabling disease in the future. An important feature of KOA on magnetic resonance imaging (MRI) is changes in the subchondral bone, bone marrow lesions (BMLs), which are related to the future degeneration of the knee joint as well as prevalent clinical symptoms. The aim of this study was to investigate the changes in BMLs after a 16-week weight-loss period in obese subjects with KOA and relate changes in BMLs to the effects of weight-loss on clinical symptom

Progress in Pediatrics in 2012: choices in allergy, endocrinology, gastroenterology, hematology, infectious diseases, neurology, nutrition and respiratory tract illnesses.

In this review, we summarize the progresses in allergy, endocrinology, gastroenterology, hematology, infectious diseases, neurology, nutrition and respiratory tract illnesses that have been published in The Italian Journal of Pediatrics in 2012. The induction of Treg activity by probiotics might be effective for promoting tolerance towards food allergens. Nasal cytology is useful in patients with rhinitis for diagnosing chronic non-allergic non-infectious diseases. Atopic eczema is associated both with an aberrant skin matrix and impaired systemic immune response. Therefore, isolated topical t

End point prick test: could this new test be used to predict the outcome of oral food challenge in children with cow's milk allergy?

BACKGROUND: Cow's milk allergy (CMA) is the most frequent food allergy in childhood; the trend of CMA is often characterized by a progressive improvement to achieve tolerance in the first 4 to 5 years of life.It has been observed that specific IgE (sIgE) towards cow's milk proteins decrease when the age increases.Although food allergy can be easily diagnosed, it is difficult to predict the outcome of the oral food challenge (OFC), that remains the gold standard in the diagnosis of food allergy, by allergometric tests. METHODS: We considered 44 children with CMA diagnosed through OFC who retur

[Knowledge of the urogenital diseases in the work of Gaspar Casal, "natural and medical history of the Principality of Asturias" (1762)].

OBJECTIVES: Gaspar Casal (Gerona, 1680, Madrid, 1759) made most of his medical work in Oviedo (Asturias, Spain), where he lived for thirty-four years, before his return to Madrid as a doctor for the Royal House. Fruits of this work is the book "Natural and medical history of the Principality of Asturias" (Madrid, 1762), considered the best exponent of the Spanish medicine of the 18th century, and where the "disease of the rose", known later as pellagra or hypovitaminosis B, was first described in. METHODS: Study of the life of Gaspar Casal and analysis of "Natural and medical history of the P

Implementing nutrition guidelines that will benefit homeless people.

The nutritional status of single adult homeless people is often overlooked, with many of the homeless population being malnourished due to a variety of reasons. This paper looks at the government's policies relating to nutrition and at the current research concerning home-lessness and diet. It also offers various strategies for improving the nutritional intake and status of this client group. It discusses the issues surrounding the use of dietary supplements, ways to minimise the risk of misuse, and highlights possible alternatives. A guideline has been devised with the specific aim of identif

Intermittent claudication services in England: insights from a freedom of information request.

BACKGROUND: Peripheral arterial disease (PAD), presenting as intermittent claudication (IC), affects 20% of adults over 60 years of age in the UK. Claudication reduces patients’ mobility, quality of life, and increases their risk of cardiovascular mortality. National guidelines recommend a holistic, multidisciplinary approach including supervised exercise therapy (SET), pharmacotherapy, and lifestyle modifications. However, clinicians raise concerns about inconsistent service provision across NHS England. Freedom of information requests (FOIR) have become an important tool for researchers to a

Nutrition impact symptoms and short-term outcomes in patients with hematological cancers.

BACKGROUND & AIMS: Malnutrition remains common in patients with hematological diseases, where nutrition impact symptoms (NIS) may impair dietary intake and consequently clinical outcomes. However, current tools often assess either symptom presence or intake limitation. This study aimed to determine the prevalence of NIS and discrepancies between NIS presence and intake limitation, assess changes following stem cell treatment initiation, and explore NIS associations with weight change and short-term mortality. METHODS: Observational study with a cross-sectional baseline assessment and longitud

Delivering optimal weight gain advice to pregnant women by lead maternity carer midwives in a real-world setting to optimise health outcomes (DOT study): a case study protocol.

BACKGROUND: Excess gestational weight gain (GWG) occurs in up to 70% of women who birth. Excess GWG increases the risk of adverse outcomes in both mother and baby including gestational diabetes, pre-eclampsia, postpartum weight retention, large for gestational age babies, and adiposity among children. Antenatal lifestyle interventions can limit excess GWG. However, successful translation of interventions into routine antenatal care are seldom, and interventions involving midwives are uncommon. Following a successful feasibility study, the main aims of this study are to determine if the Deliver

Serum Levels of IL-1 RA Increase with Obesity and Type 2 Diabetes in Relation to Adipose Tissue Dysfunction and are Reduced After Bariatric Surgery in Parallel to Adiposity.

BACKGROUND: Excess adiposity leads to a dysfunctional adipose tissue that contributes to the development of obesity-associated comorbidities such as type 2 diabetes (T2D). Interleukin-1 receptor antagonist (IL-1RA) is a naturally occurring antagonist of the IL-1 receptor with anti-inflammatory properties. The aim of the present study was to compare the circulating concentrations of IL-1RA and its mRNA expression in visceral adipose tissue (VAT) in subjects with normal weight (NW), obesity with normoglycemia (OB-NG), or obesity with impaired glucose tolerance or T2D (OB-IGT&T2D) and to analyze

The impact of national lockdown on nutritional status of children with inflammatory bowel disease.

BACKGROUND: The COVID-19 pandemic has had wide-reaching primary and secondary health implications. The UK government implemented a national lockdown to slow the rate of infection at the end of March 2020, lasting until early summer 2020. The results from a UK nationwide survey suggest the majority of inflammatory bowel disease patients were followed up using technology-enabled care services (TECS) during this time. We therefore aimed to explore the impact of the pandemic on nutritional status of children with inflammatory bowel disease, focusing on the effect of national lockdown from March to

Determinants and Trends of Adherence to a Gluten-Free Diet in Adult Celiac Patients on a Long-term Follow-up (2000-2020).

BACKGROUND & AIMS: Data on factors governing long-term adherence to a gluten-free diet (GFD) in celiac disease (CD) are scarce. We aimed to determine trends and clinical predictors of long-term GFD adherence in adult CD. METHODS: Initial and long-term (>3 years) GFD adherence, clinical characteristics at baseline and follow-up were collected retrospectively from celiac patients followed-up over 20 years (2000-2020). Predictors of long-term GFD adherence at diagnosis, and follow-up were evaluated by multivariate logistic regression. RESULTS: 248 patients (37 ± 12 years, 186F, median time on a

[Protein -based diet with respect to the principles of rational nutrition. Menus analysis].

INTRODUCTION: A diet high in protein and low in carbohydrates has four phases, first of them being the attack phase, which eliminates carbohydrates to the highest extent. In subsequent phases the consumption of carbohydrates is gradually allowed but their ratio is limited. The aim of performed studies was to analyze the metabolic effects of protein -based diet. The hypothesis – the analysis based on the composition of the diets should draw the attention to health risks being not only related to too high consumption of protein. MATERIALS AND METHODS: In the study, 40 diets were composed accord

Quality of life, psychological adjustment, and adaptive functioning of patients with intoxication-type inborn errors of metabolism - a systematic review.

BACKGROUND: In recent decades, considerable progress in diagnosis and treatment of patients with intoxication-type inborn errors of metabolism (IT-IEM) such as urea cycle disorders (UCD), organic acidurias (OA), maple syrup urine disease (MSUD), or tyrosinemia type 1 (TYR 1) has resulted in a growing group of long-term survivors. However, IT-IEM still require intense patient and caregiver effort in terms of strict dietetic and pharmacological treatment, and the threat of metabolic crises is always present. Furthermore, crises can affect the central nervous system (CNS), leading to cognitive, b

A prospective study of pre-illness diet in newly diagnosed patients with Crohn's disease.

BACKGROUND: Environmental factors, including diet, seem to participate in the etiology of inflammatory bowel disease. The kind of dietetic habits before the appearance of the illness in patients with Crohn's disease (CD) has not been studied extensively. AIM: To prospectively assess the kind of food consumption in patients with CD exactly at the time of diagnosis and to identify dietary constituents as risk factors for development of CD. PATIENTS - METHODS: Twenty eight patients with a newly established diagnosis of CD (2-4 weeks), (12 men and 16 women), 30 patients with previously (between 2

[Assessment of long-term bone metabolism in patients with relative hypoparathyroidism after changing the calcium concentration of the dialyzate to 3mEq/L].

The existence of a low-bone turnover, non-aluminium related, has been more prevalent in recent years. Factors involved include the increasing number of older and diabetic patients initiating dialysis. Also higher intake of calcium salts and the widespread use of vitamin D derivates may play a role. Demonstration of low PTH secretion allows the recognition of the group of patients with special risk of adynamic bone disease. Little can be done to improve bone remodelling apart from correct management of risk factors previously mentioned. A decrease in dialysate calcium concentration can act in t

The circulating insulin-like growth factor system in children with coeliac disease: an additional marker for disease activity.

BACKGROUND: Chronic undernutrition resulting from coeliac disease (CD) could be associated with changes in the circulating insulin-like growth factor (IGF) system, which may participate in the pathogenesis of growth retardation occurring in these patients. METHODS: We performed a cross-sectional study in CD subjects attempting to (1) document the pattern of serum IGF-I and IGF binding protein (IGFBP) 1 and 3 at diagnosis and (2) assess the response of circulating IGF system to dietary treatments, in comparison with the response of clinical and laboratory findings utilized for the diagnosis of
