Orthonasal and retronasal odor identification in patients with parosmia.

OBJECTIVE: To compare retronasal and orthonasal perception in parosmic COVID-19 patients, in order to determine whether COVID-19 has a differential effect on these functions. METHODS: Using the Sniffin Sticks test battery orthonasal function was examined for odor threshold, discrimination and identification. Retronasal function was assessed using 20 tasteless aromatized powders. Gustatory function was measured using the Taste Strips test. RESULTS: This study included 177 patients (127 women, 50 men; mean age 45 years), of whom 127 (72%) were hyposmic and 50 (28%) normosmic. Compared to patie

Preliminary transcriptomic analysis of peripheral blood from German Shepherd dogs with degenerative joint disease for the identification of diagnostic biomarkers.

OBJECTIVE: Degenerative joint disease (DJD) includes a group of disorders characterised by the deterioration of the articular cartilage. In this study, we investigated the transcriptomic profile of peripheral blood in German Shepherd dogs with DJD to identify putative diagnostic biomarkers. METHODS: Differential gene expression (DGE) and gene ontology (GO) analyses of the bulk RNA-seq experiment were performed in a cohort of 12 adult dogs (five cases and seven controls, classified by clinical and radiographic analyses). RESULTS: Radiographs of cases revealed severe signs of progressive DJD.

Evaluation of Glycosylated Ferritin in Adult-Onset Still's Disease and Differential Diagnoses.

Glycosylated ferritin (GF) has been reported as a good diagnostic biomarker for adult-onset Still’s disease (AOSD), but only a few studies have validated its performance. We performed a retrospective study of all adult patients with at least one GF measurement over a 2-year period in one hospital laboratory. The diagnosis of AOSD was based on the expert opinion of the treating physician and validated by two independent investigators. Patients’ characteristics, disease activity, and outcome were recorded and compared. Twenty-eight AOSD and 203 controls were identified. Compared to controls, the

A Case of Glycogenic Hepatopathy as a Complication of Poorly Controlled Type 1 Diabetes Mellitus.

A 23-year-old African American male with a medical history significant for poorly controlled type 1 diabetes mellitus (T1DM) presented with abdominal pain and vomiting. His laboratory workup was consistent with diabetic ketoacidosis (DKA). An acute elevation of liver enzymes was noted as the DKA resolved, with the alanine transferase and aspartate transferase levels elevated to more than 50 times the normal limit within the next 24 hours. Because abnormal liver function tests are found frequently in patients with type 1 diabetes mellitus, it is important to have a broad differential diagnosis.

Postpartum headaches after epidural or spinal anesthesia.

PURPOSE OF REVIEW: Postpartum headache is a common occurrence with various obstetric, neurologic, and anesthetic etiologies. Post dural puncture headache (PDPH) after neuraxial anesthesia is a culprit that may be less familiar to obstetricians. In this review, authors will discuss the differential diagnosis and management of postpartum headache, review PDPH, and explore evidence suggesting that PDPH may have greater implications than previously considered. RECENT FINDINGS: Emerging evidence suggests that PDPH, previously believed to be benign and self-limited, is associated with significant a

Nucleolar stress in Drosophila neuroblasts, a model for human ribosomopathies.

Different stem cells or progenitor cells display variable threshold requirements for functional ribosomes. This is particularly true for several human ribosomopathies in which select embryonic neural crest cells or adult bone marrow stem cells, but not others, show lethality due to failures in ribosome biogenesis or function (now known as nucleolar stress). To determine if various Drosophila neuroblasts display differential sensitivities to nucleolar stress, we used CRISPR-Cas9 to disrupt the Nopp140 gene that encodes two splice variant ribosome biogenesis factors (RBFs). Disruption of Nopp140

Pheochromocytoma and paraganglioma: genotype versus anatomic location as determinants of tumor phenotype.

To date, germline or somatic genetic events can be detected for at least 60% of paragangliomas. Strong genotype-phenotype associations have been recognized and become increasingly refined. Characteristics closely linked with genotype include syndromic presentation, age of onset, risk of metastatic disease and predominant anatomic site. In contrast, profiles of catecholamine secretion appear to be largely determined by anatomic location or cell type of origin. This review summarizes current knowledge of genotype-phenotype correlations for paragangliomas in different locations and scrutinizes pr

Wolbachia spread dynamics in multi-regimes of environmental conditions.

Mosquito-borne diseases such as dengue fever and Zika kill more than 700,000 people each year in the world. A novel strategy to control these diseases employs the bacterium Wolbachia whose infection in mosquitoes blocks virus replication. The prerequisite for this measure is to release Wolbachia -infected mosquitoes to replace wild population. Due to the fluctuation of environmental conditions for mosquito growth, we develop and analyze a model of differential equations with parameters randomly changing over multiple environmental regimes. By comparing the dynamics between the stochastic syste

Neural circuitry involved in quitting after repeated failures: role of the cingulate and temporal parietal junction.

The more times people fail the more likely they are to give up, however little is known about the neural mechanisms underlying this impact of repeated failure on decision making. Here we have used a visual shape discrimination task with computer-controlled feedback combined with functional magnetic resonance imaging (fMRI) to investigate the neural circuits involved. The behavioral task confirmed that the more times subjects experienced failure the more likely they were to give up, with three successive failures being the key threshold and the majority of subjects reaching the point where they

Cerebellopontine angle mass mimicking lingual nerve injury after dental implant placement: a case report.

This is a rare case report of a cerebellopontine angle (CPA) mass mimicking lingual nerve injury after a dental implant placement. Lingual nerve injury is a common complication following dental implant placement. CPA masses are likely to cause symptomatic trigeminal neuralgia, and thus can mimic and be easily confused with oral diseases. We experienced a case of CPA mass mimicking lingual nerve injury after dental implant placement. The patient was a 57-year-old Japanese female who complained of glossalgia. She underwent dental implant placement in the mandible before visiting our clinic. Pano

Increased supernormality in patients with multiplet discharges: Evidence for a common pathophysiological mechanism behind multiplets and fasciculations.

OBJECTIVE: To determine whether there is a relation between electrically evoked multiplet discharges (MDs) and motor axonal excitability properties. We hypothesized that electrically evoked MDs share their underlying pathophysiological mechanism with fasciculations. METHODS: High-density surface EMG and motor nerve excitability recordings of the thenar muscles were performed in 22 patients with motor neuron disease (MND) in their differential diagnosis and who were referred for EMG examination. RESULTS: Supernormality (hyperexcitable phase following the refractory period) was significantly i

An integrated system for multichannel neuronal recording with spike/LFP separation, integrated A/D conversion and threshold detection.

A mixed-signal front-end processor for multichannel neuronal recording is described. It receives 12 differential-input channels of implanted recording electrodes. A programmable cutoff High Pass Filter (HPF) blocks dc and low-frequency input drift at about 1 Hz. The signals are band-split at about 200 Hz to low-frequency Local Field Potential (LFP) and high-frequency spike data (SPK), which is band limited by a programmable-cutoff LPF, in a range of 8-13 kHz. Amplifier offsets are compensated by 5-bit calibration digital-to-analog converters (DACs). The SPK and LFP channels provide variable am

[Gitelman's syndrome: an important differential diagnosis of hypokalemia].

HISTORY AND CLINICAL FINDINGS: A 26-year-old woman presented with fatigue, muscle cramps and weakness. Since the age of 8 years she had moderate hypokalemia of unknown origin that was confirmed on multiple occasions. There was no family history of disease. INVESTIGATIONS: Laboratory tests showed moderate to severe hypokalemia with a serum potassium concentration of 2.7 to 3.0 mmol/l, hypomagnesemia, metabolic alkalosis and pronounced stimulation of the renin-angiotensin-aldosterone system. Despite normal serum calcium levels, urinary calcium excretion was below the detection threshold. Increa

A frequency-doubling perimetric study in normal-tension glaucoma with hemifield defect.

PURPOSE: The authors prospectively compare perimetric results obtained by conventional differential light sensitivity (DLS) perimetry and frequency-doubling perimetry (FDP) in normal-tension glaucoma (NTG) with a hemifield visual field defect, assuming that some of these eyes already have early glaucomatous changes in the "intact" hemifield. METHODS: Eleven eyes of 11 patients with NTG who met the enrollment criteria were studied. Seven eyes had an upper hemifield defect and four had a lower defect, determined by DLS with a Humphrey Field Analyzer (Zeiss-Humphrey, Inc., San Leandro, CA, U.S.A

Mitochondria and other calcium buffers of squid axon studied in situ.

Continuous nondestructive monitoring of intracellular ionized calcium in isolated squid axons by differential absorption spectroscopy (using arsenazo III and antipyrylazo III) was used to study uptake of calcium by carbonyl cyanide, p-trifluoromethoxy-phenylhydrazone (FCCP)- and (or) cyanide (CN)-sensitive and insensitive constituents of axoplasm. Known calcium loads imposed on the axon by stimulation produced proportional increments of free axoplasmic calcium. Measurement of increments in ionized calcium as a function of load confirmed earlier reports of buffering in normal and FCCP- and (or)

Headache Emergencies.

Headache is a common reason for seeking medical attention. Most cases are benign primary headache disorders; however, there is significant overlap between symptoms of these disorders and secondary headaches. Differentiating these clinical scenarios requires a careful history with attention to red flag symptoms and a neurologic examination. These details can identify dangerous disorders: subarachnoid hemorrhage, reversible cerebral vasoconstriction syndrome, elevated intracranial pressure, hydrocephalus, cerebral venous sinus thrombosis, arterial dissection, central nervous system infection, an

Auditory brainstem evoked potentials in blast injury.

Blast injury typically consists of a mixed conductive and sensorineural hearing loss. The sensorineural component includes temporary as well as permanent threshold elevations. Auditory brainstem evoked potentials (ABEP) are sensitive to functional changes in various levels along the auditory pathway. ABEP were recorded from 37 survivors of blasts and latency measures were correlated with clinical findings. Prolongation of peak latencies was correlated with the conductive component of blast-induced hearing loss, as well as with the TTS component of the sensorineural impairment. No central effec

Incorporating Scale Uncertainty into Differential Expression Analyses Using ALDEx2.

Differential abundance or expression analyses are routinely performed on metagenomic, metatranscriptomic, and amplicon sequencing data. In such datasets, analysts usually have no information regarding the true scale (i.e., size) of the microbial community or sample under study, with inter-sample differences in sequencing depth instead being driven by technical variation rather than biological factors. Recent work has demonstrated that normalizations used in all analysis tools make incorrect assumptions about the biological scale of the system in question, leading to unacceptably high false-dis
