A new variant of type III jejunoileal atresia.

Jejunoileal atresia (JIA) is a congenital defect that can result in significant loss of bowel length. The traditional classification of JIA was first proposed by Grosfeld and includes 4 subtypes. Among these, type IIIB, or apple-peel atresia, is characterized by a proximal atretic jejunum and a distal segment of spiraled bowel that terminates at the cecum. Owing to this anatomy, patients with type IIIB JIA are at increased risk for short bowel syndrome and intestinal failure. In this report, we described the case of a neonate with a prenatal diagnosis of JIA. At exploration, she was initially

An updated meta-analysis of transanal drainage tube for prevention of anastomotic leak in anterior resection for rectal cancer.

BACKGROUND: Anastomotic leakage (AL) is one of the most serious complications after anterior resection for rectal cancer. Transanal drainage tube (TDT) placement is widely used to reduce AL, but its efficacy remains controversial. We performed a meta-analysis to evaluate the effectiveness of TDT for prevention of AL, using updated evidence. METHODS: Randomized controlled trials (RCTs) and cohort studies evaluating the effectiveness of TDT for prevention of AL after anterior resection for rectal cancer were identified by using a predefined search strategy. Meta-analysis was performed to estima

Intestinal lengthening via multiple in-continuity springs.

BACKGROUND: Short bowel syndrome is a debilitating condition with few effective treatments. Spring-mediated distraction enterogenesis can be used to lengthen intestine. The purpose of this study is to determine whether multiple springs in series can safely increase the total amount of lengthening. METHODS: Juvenile mini-Yucatan pigs each received three nitinol springs placed within their jejunum. Plication was used to narrow the intestine around each spring to secure them. Compressed springs were used in the experimental group, while uncompressed springs were used in the control group. The in

[Falling Short of Minimum Volume Standards, Exemptions and Their Consequences from 2018 Onwards. Complex Procedures on Oesophagus and Pancreas in German Hospitals from 2006 to 2014].

STUDY AIM: The minimum volume standards for hospitals in Germany, in force since 2004, provide four exemptions for non-complying hospitals. This study investigates the extent and importance of these exemptions for complex procedures on the oesophagus and pancreas for all non-complying hospitals and for the revised minimum volume regulations in force since the beginning of 2018. METHOD: Longitudinal, descriptive analyses of data on minimum volume standards and their exemptions for complex procedures on the oesophagus and pancreas, as presented by the hospital quality report cards of the report

The treatment of anal fistula: second ACPGBI Position Statement - 2018.

It is over 10 years since the first ACPGBI Position Statement on the management of anal fistula was published in 2007. This second edition is the result of scrutiny of the literature published during this time; it updates the original Position Statement and reviews the published evidence surrounding treatments for anal fistula that have been developed since the original publication.

Robot-assisted resection of gastric duplication cysts in a child: a detailed case report.

Gastric duplication cysts (GDCs) represent 4-9% of alimentary tract duplications. Early diagnosis and surgical excision are essential to avoid morbidity or neoplastic degeneration. Roboticassisted excision of GDCs has never been described in childhood. We report an asymptomatic male patient with 2 gastric cystic masses at ultrasonography (US)-study (diameter 25mm and 8mm), increasing in size at follow-up. At 20 months of age, magnetic- resonance-imaging-scan confirmed 2 round gastric masses (44×35mm and 16×12mm, respectively). Two months later, an elective robotic-assisted excision of GDCs was

[Congenital haptoglobin deficiency discovered on the occasion of anaphylaxis induced by platelet concentrate transfusion].

A 77-year-old man with myelodysplastic syndrome suffered from duodenal perforation after undergoing endoscopic submucosal dissection (ESD) for treatment of duodenal cancer. He presented with hemorrhagic shock, peritonitis and disseminated intravascular coagulation (DIC), and received transfusions of red blood cells (RBC), fresh frozen plasma (FFP), γ-globulin and albumin (Alb). One month after the last RBC transfusion, prolonged thrombocytopenia was observed, and platelet concentrate (PC) was transfused. However, immediately after starting PC transfusion, he developed dyspnea, hypotension and

Congenital anomaly rectified at birth: one-stage single-incision laparoscopic-assisted anorectoplasty for newborns with anorectal malformations and recto-urethral fistula.

OBJECTIVE: Currently, staged procedures involving stoma formation and closure are the widely accepted treatment for newborns suffering from anorectal malformations (ARM) with recto-urethral fistula. This study aims to evaluate the safety and efficacy of one-stage single-incision laparoscopic-assisted anorectoplasty (SILAARP) as an alternative to the conventional approach. METHODS: Newborns with ARMs and recto-urethral fistula who underwent one-stage SILAARP between June 2013 and December 2014 were reviewed. The procedure involved decompression of the meconium via a small temporary incision of

Transanal and transrectal operations for excisional surgery of the low and mid rectum (with video).

Even with considerable operator experience and expertise, surgical access from the abdomen to the mid and low rectum and mesorectum can be difficult, especially in male and obese patients. This holds true whether the approach from above is performed by laparoscopy or laparotomy. While conventional operations that include extirpation of the anal canal and sphincters of course incorporate a perineal approach for the lowermost aspect of the proctectomy, their efficiency in cephalad extension is limited by difficulties in access and visualization. Recently, the concepts behind transanal endoscopic

Adhesive small bowel obstruction--acute management and treatment in children.

Adhesive small bowel obstruction is a significant cause of short- and long-term morbidity in infants and children. Unfortunately, the majority of scientific literature relative to adhesive obstructions continues to be dominated by adult studies. In this article, the existing literature for infant and pediatric adhesive obstructions is reviewed, with relevant comparisons to the available adult data. In addition, general guidelines for the management of infant and pediatric adhesive obstructions are recommended, based on the best available evidence.

Cardiaplication as a novel antireflux procedure for infants: a proof of concept in an infant porcine model.

INTRODUCTION: Current surgical techniques for gastroesophageal reflux in infants involve gastric fundoplication and crural repair. These operations are complicated by a high recurrence rate in the infant population. The primary cause of failure is generally related to recurrent hiatal hernia. Minimal dissection of the gastroesophageal junction has been linked to reduced hiatal hernia formation. We propose an alternative plication technique for modifying the gastroesophageal junction called cardiaplication. In an effort to evaluate the effectiveness of this alternative plication technique, an a

Laparoscopic versus open reduction of intussusception in children: experience over a decade.

BACKGROUND: Intussusception is a common cause of bowel obstruction in children, which sometimes necessitates operative reduction and or resection. We report our series of patients with intussusception who were treated laparoscopically (LAP group) compared with exploratory laparotomy (OPEN group). SUBJECTS AND METHODS: After institutional review board approval, a retrospective review was performed evaluating outcomes for patients requiring surgical reduction of intussusception over a 10-year period. Analysis was based on intent to treat, and technique of exploration was surgeon's choice. Data

Experience of application of combined preparation Ciprolet A (ciprofloxacine 500 mg, tinidazol 600 mg) in 27 patients after performed reconstructive operations for surgical diseases of abdominal organs as well as in 50 patients in destructive forms of an acute cholecystitis was summarized. Possibility of the preparation application in ambulatory conditions constitutes its advantage.
