A double-blind study of radiologic accuracy in diverticulitis, diverticulosis, and carcinoma of the sigmoid colon.

A review of the barium enema studies in 73 patients with diverticular disease of the colon and with and without associated neoplasm leads to the conclusion that the correct radiologic recognition of an associated neoplasm in a colon with diverticulitis or diverticulosis can be made only about half the time. The limitations of the radiographic method and radiologic findings in distinguishing diverticulitis from carcinoma should be more clearly recognized by the clinician.

Resection of the perforated segment. A significant advance in treatment of diverticulitis with free perforation or abscess.

As a result of improved medical management of chronic diverticular disease, perforation has become the most common indication for surgical intervention. During the past five years sixty-three patients underwent operation for colonic diverticular disease, of which forty-six were for perforation (generalized peritonitis in 8, abscess in 30, and fistula in 8). The eight patients with generalized peritonitis underwent emergency exploration for spreading peritoneal signs and were managed by resection of the perforated segment, end colostomy, and mucous fistula or Hartmann's pouch. Treatment of thir

Recurrent left-sided diverticulitis after surgery: A systematic review and single arm meta-analysis.

INTRODUCTION: Recurrent diverticulitis after resection remains a key concern despite advances in minimally invasive surgery. These recurrences may result from incomplete resection of the diseased segment, unrecognized synchronous diverticulosis or ongoing alterations in colonic motility and microbiome composition. This systematic review and single-arm meta-analysis aimed to estimate long-term recurrence and stoma rates following elective surgery for left-sided diverticulitis. METHODS: Following PRISMA guidelines and PROSPERO registration, PubMed, Cochrane and Scopus databases were searched (2…

Novel polygenic risk score associates with diverticulitis in a multi-institutional, ancestrally diverse cohort.

Diverticulitis is a common and morbid colorectal disease that recurs after an initial attack in up to 30% of patients. Elective surgery to remove the affected portion of the colon is an effective intervention to prevent disease recurrence, but appropriate patient selection is challenging given the limited ability to predict which patients are likely to recur, or the severity with which they may do so. Genetics influence diverticulitis and can be used for risk stratification, but this has only been studied in European ancestry populations. Using state-of-the-art techniques, we created a polygen…

Perforated sigmoid diverticulitis within a Para-umbilical hernia-A case report and literature review.

Diverticulitis is a common complication of colonic diverticulosis and can lead to a range of serious issues, including perforation. Incisional hernias, often resulting from previous abdominal surgeries, usually contain omentum or bowel loops. However, it's uncommon for both conditions to present together, especially in the same anatomical location. We report a rare case of a 64-year-old male who presented with sudden abdominal pain, nausea, and vomiting. Examination revealed a painful, erythematous para-umbilical hernia. CT imaging showed an incisional hernia containing a perforated segment of…

Long-term Follow-up After an Initial Episode of Diverticulitis: A 13-Year Update.

BACKGROUND: For patients with recurrent diverticulitis, the trigger to proceed to elective sigmoid colectomy is unclear. Current clinical practice guidelines suggest that this is an individualized decision between surgeon and patient. OBJECTIVE: To assess the long-term risk of diverticulitis recurrence and determine predictors of recurrent disease. DESIGN: Retrospective case series. SETTING: Tertiary care center. PATIENTS: Consecutive patients with CT-proven diverticulitis who presented as inpatient or outpatient between 2002 and 2008. INTERVENTIONS: Incidence of diverticulitis recurrence…

A novel scoring system for predicting disease severity without CT imaging in acute diverticulitis.

PURPOSE: Clinical scoring could help physicians identify patients with suspected acute diverticulitis who would benefit from further evaluation using computed tomography imaging. The aim of the study was to identify risk factors for complicated acute diverticulitis and create a risk score to predict disease severity in acute diverticulitis. METHODS: Patients diagnosed with CT-verified acute diverticulitis between 2015 and 2017 were included. Data on patients' clinical and laboratory findings and medical histories were collected retrospectively. Risk factors for complicated acute diverticuliti…

Emergency laparoscopic sigmoid colectomy with primary anastomosis for Hinchey stages III and IV diverticulitis.

For patients with perforated diverticulitis, many reports have focused on laparoscopic surgery without primary anastomosis. We performed laparoscopic surgery with primary anastomosis in three patients (two with Hinchey stage III, one with IV), with a median age of 53 years, all female, and no prior medical history. They all were hemodynamically stable. The median operation time was 91 minutes (range: 56-227 minutes) and the median blood loss was 50 mL (range: 0-200 mL). Their post-operative course was uneventful, and patients commenced oral intake at a median of 5 post-operative days and were …

End-to-Side Ileo-Sigmoid Anastomosis and Late Onset Gangrenous Diverticulitis: A Case Report.

End-to-side ileo-sigmoid anastomosis converts the proximal colon into a blind intestinal segment which is excised during the surgery. If we do not resect the proximal colon, it is expected to behave like a colonic diverticulum, but direct evidence of this assumption is lacking. During an exploratory laparotomy, we detected an end-to-side ileo-sigmoid anastomosis and found that the proximal colon was gangrenous. The patient passed away during the postoperative period, yet their remarkably long period of symptom-free survival remained intriguing.

Prospectively Randomized Controlled Trial on Damage Control Surgery for Perforated Diverticulitis with Generalized Peritonitis.

INTRODUCTION: Damage control surgery (DCS) with abdominal negative pressure therapy and delayed anastomosis creation in patients with perforated diverticulitis and generalized peritonitis was established at our Institution in 2006 and has been published. The concept was adopted in other hospitals and published as a case series. This is the first prospectively controlled randomized study comparing DCS and conventional treatment (Group C) in this setting. METHODS: All consecutive patients from 2013 to 2018 with indication for surgery were screened and randomized to Group DCS or Group C. The pri…

Methicillin-resistant Staphylococcus Aureus Colitis Secondary to Complicated Acute Diverticulitis: A Rare Case Report.

A 33-year-old obese female with a recent diagnosis of acute diverticulitis (AD) was admitted to the hospital for severe abdominal pain, intractable nausea and vomiting, and diarrhea two days following oral antibiotic treatment for AD. Stool cultures collected upon her readmission were negative for Clostridium difficile (C. difficile) antigen and toxins A and B, but were notable for methicillin-resistant Staphylococcus aureus (MRSA). She was started on intravenous (IV) piperacillin/tazobactam, IV vancomycin, and an oral liquid vancomycin solution, which resulted in rapid resolution of her sympt…

Extranodal Marginal Zone Lymphoma Presenting within the Meckel Diverticulum as Diverticulitis: A Case Report.

Meckel diverticulum is the most common congenital defect of the gastrointestinal tract. It can be asymptomatic or mimic appendicitis and may be complicated by bleeding, diverticulitis, obstruction, and, rarely, neoplasia. We report the first case of extranodal marginal zone lymphoma occupying a Meckel diverticulum. A 44-year-old man with history of colonic diverticulitis presented to the emergency department for evaluation of acute abdominal pain. Radiography showed enteric obstruction, prompting diagnostic laparoscopy. Above the level of mid-ileum an intact Meckel diverticulum was identified.…

Emergency laparoscopic management of perforated sigmoid diverticulitis: a promising alternative to more radical procedures.

BACKGROUND: Classic emergency surgical management of complicated perforated sigmoid diverticulitis is based on the principle of a two-stage operation, with colon resection and temporary stoma (Hartmann's procedure). But the later second-stage operation can be technically difficult and can be associated with a significant morbidity rate. We argue that laparoscopy may be beneficial in such patients with peritonitis in terms of operative results and could facilitate later surgical management. STUDY DESIGN: We studied all consecutive patients with perforated sigmoid diverticulitis requiring emerg…

Dark-lumen magnetic resonance colonography in patients with suspected sigmoid diverticulitis: a feasibility study.

To assess dark-lumen magnetic resonance colonography (MRC) for the evaluation of patients with suspected sigmoid diverticulitis. Forty patients with suspected sigmoid diverticulitis underwent MRC within 72 h prior to conventional colonoscopy (CC). A three-dimensional T1-weighted volumetric interpolated breath-hold examination sequence was acquired after an aqueous enema and intravenous administration of gadolinium-based contrast agents. All MRC data were evaluated by two radiologists. Based on wall thickness and focal uptake of contrast material and pericolic reaction including mesenteric infi…

Right-sided colonic diverticulitis mimicking acute cholecystitis in pregnancy: case report and laparoscopic treatment.

Right-sided colonic diverticulitis is an uncommon disorder that most frequently mimics appendicitis. During pregnancy, displacement of the diseased cecum and ascending colon into the right upper quadrant may result in symptomatology that mimics cholecystitis. A 37-year-old white woman with a history of previous benign incidental appendectomy presented at 20 weeks' gestation with right upper abdominal pain and nausea for 2 days. Significant findings included local rebound tenderness and palpable fullness over the gallbladder, leukocytosis, and low-grade fever, but otherwise unremarkable routine…
