Neurogenic inflammation is assumed to be an important local defence mechanism against noxious stimuli in various parts of the body. Recent experimental evidence suggests substance P(SP) to be the mediator of this mechanism in the respiratory tract. Keloids are commonly seen as a complication of earlobe piercing. As biopsies are usually not performed routinely in many instances, some of the lesions might, in fact, represent other entities, such as cutaneous pseudolymphoma. Contrary to keloids, for which surgery alone is not recommended, a simple excision or superficial x-ray is sufficient in such cases.

Ear reduction has been performed occasionally for aesthetic considerations. This series is comprised of 8 patients representing 7 bilateral reductions and 1 unilateral reduction for a total of 15 ears. Historically, surgery for macrotia consisted of resections of full-thickness wedges from the periphery of the ear. The surgical defect was repaired by directly approximating the wound edges. Records of 300 consecutive patients who had only one ear operated on by stapedectomy and who received long-term followup were studied. These cases came from 3036 stapedectomy operations performed between January 1961 and April 1969. In general, the two ears behaved the same: if a "flat" sensorineural loss occurred in one ear, it was likely to develop in the other. Similarly, if one ear developed a high-tone loss, the other would do likewise. To accomplish two objectives, morphological and functional improvements, in plastic operation of major ear anomalies, the authors presented a surgical method utilizing exclusively auto materials and several pedicles of skin, fascia and periostium. A partially implantable middle ear implantable hearing aid (Partial-MEI) is also an indication to those anomalies. Incidentally, we implanted the first Partial-MEI to a patient with unilateral atresia-microtia. The case was presented and demonstrated Partial-MEI implantation as one of the approaches for major ear anomalies to improve hearing.

The etiology of painful papule of the external ear, chondrodermatitis nodularis chronica helicis, has been an enigma for over half a century. I reviewed 84 skin biopsy specimens from 82 patients with this histopathologic diagnosis and found evidence that the infundibular portion of the hair follicle is probably the primary cutaneous structure involved in this condition. This lesion appears to evolve from an acute, suppurative, granulomatous dermatitis into a later-fibrosing dermatitis having the clinical and histologic features of perforating folliculitis and prurigo nodularis.

At a time when microsurgical reattachment of amputated limbs is commonplace, successful reattachment of total ear amputations is rare. Avulsed vessels, small diameters (0.3 to 0.7 mm), and technical difficulties provide primary obstacles. This study compared nasopharyngeal sepsis with under-aeration ear disease. Nasopharyngeal sepsis was assessed by culture taken directly from the surface of removed adenoid tissue. 156 cases were studied; 100 showed varying degrees of middle-ear under-aeration and 56 had no active ear disease or history of it. CAT scan by thin slices offers the possibility of evaluation of the bone structures of the ear and their contents. The progressively increasing availability of the scanner should make access much more routine for study of the ear, with a concomitant reduction in tomographies, inadequate in a large number of circumstances. The 4 main indications are chronic otitis with cholesteatoma, trauma, acoustic neuromas and malformations. The respective performances of tomography and CAT scan in each of these situations are discussed and an investigation strategy is suggested.

Chondritis may occur in up to 25 percent of burned ears and commonly follows a superficial partial-thickness injury. The onset is usually insidious and often delayed. Prevention is the key, as the treatment of an established infection frequently leads to disastrous consequences. Herein, we have reviewed 136 consecutive inpatients with burns of the ear. We examined the relationship between electrocochleographic test results and symptoms associated with Meniere's disease-endolymphatic hydrops (MD-ELH). Extratympanic electrocochleography (ECochG) was performed on 55 consecutive patients (110 ears) suspected of having MD-ELH, and the results compared with the clinical symptoms of the patients on the day of recording.
