Neuroleptic malignant syndrome: occurrence in a child after reconstructive surgery.

The occurrence of the rare but potentially fatal neuroleptic malignant syndrome must be considered by the surgeon treating a patient who develops hyperthermia, mental abnormalities, autonomic instability, and muscle rigidity after exposure to phenothiazines or other neuroleptic drugs. The dopamine agonist bromocriptine appears to be the treatment of choice in adults and seemed to be effective and well tolerated in our patient. Although the syndrome cannot be prevented, recognition is crucial, since effective general and specific therapy is available. Differentiating neuroleptic malignant syndr

Total Avulsed Ear Management With Radial Forearm Free Flap.

Avulsive injuries to the ear are extremely challenging to treat. The literature documents numerous ways to manage such injuries. This report describes a case of total auricular cartilaginous avulsion that occurred during a motor vehicle accident. The avulsed segment had an ischemia time longer than 8 hours by the time of presentation at the authors' tertiary care facility. The avulsed segment was de-epithelialized and embedded into the volar forearm. After a healing period of 6 months, a radial forearm free flap was harvested and the ear was reconstructed. After ensuring good flap uptake and a

Pediatric-Onset Relapsing Polychondritis With Otolaryngeal Manifestations.

Relapsing polychondritis (RP) is a rare autoimmune disease that can present with various clinical manifestations. Among the affected sites, the ear, nose, and throat cartilages are frequently involved, often leading to subtle and episodic symptoms that can be challenging to diagnose. A high index of suspicion is necessary for the early identification of these subtle signs, which can aid in early diagnosis and prompt management. In this report, we present a rare case of pediatric-onset relapsing polychondritis that was initially misdiagnosed as laryngotracheobronchitis.

Microsurgical Reconstruction of the Ear and Temporal Region: Structural and Functional Considerations Including Hearing Rehabilitation-A Narrative Review.

Reconstruction of the ear and temporal region presents unique challenges due to the complex anatomy of the lateral skull base and the need to restore both structural integrity and auditory function. Historically managed as separate entities, auricular reconstruction and hearing rehabilitation are increasingly approached in an integrated manner, supported by advances in microsurgical techniques and implantable hearing technologies. This narrative review synthesizes contemporary evidence on microsurgical reconstruction of the ear and temporal region in conjunction with hearing rehabilitation, an

Transcanal Endoscopic Tympanoplasty for the Treatment of Localized Eosinophilic Otitis Media in the Tympanic Cavity: A Report of Two Cases.

Eosinophilic otitis media (EOM) is a form of refractory otitis media characterized by eosinophil-dominant, highly viscous middle ear effusion, for which tympanoplasty is generally contraindicated. In patients with well-controlled otorrhea who do not require transtympanic therapy, tympanic membrane closure may significantly improve clinical severity scores and reduce the risk of infection. We report two cases of EOM localized to the mesotympanic and hypotympanic cavities that were successfully treated with transcanal endoscopic tympanoplasty without the use of biologics. In both cases, the proc

Bioengineered human tissue regeneration and repair using endogenous stem cells.

We describe a general approach to produce bone and cartilaginous structures utilizing the self-regenerative capacity of the intercostal rib space to treat a deformed metacarpophalangeal joint and microtia. Anatomically precise 3D molds were positioned on the perichondro-periosteal or perichondral flap of the intercostal rib without any other exogenous elements. We find anatomically precise metacarpal head and auricle constructs within the implanted molds after 6 months. The regenerated metacarpal head was used successfully to surgically repair the deformed metacarpophalangeal joint. Auricle re

Semilunar conchal cartilage graft in saddle nose reconstruction.

INTRODUCTION: The saddle nose deformity is easily recognized by the loss of septal support and nasal dorsal height with adverse functional and aesthetic consequences. TECHNIQUE: We treated a 50-year-old woman and a 54-year old man that presented with a moderate saddle nose deformity following a previous septorhinoplasty (female patient) and a posttraumatic severe saddle nose deformity (male patient). The patients were treated by open approach rhinoplasty under general anesthesia, and the saddle nose deformity was reconstructed with a semilunar conchal cartilage graft. A semilunar part of the

The malleus cap: Anatomic description of cartilage of the lateral process of the malleus.

PURPOSE: To anatomically describe a cartilaginous cap attached to the lateral process of the malleus. STUDY DESIGN: Histologic and gross anatomic review. METHODS: Twenty temporal bones were histologically reviewed. The anatomical relationship between the tympanic membrane and malleus was then defined at the level of the lateral process of the malleus and the long process of the malleus. Separately, gross evaluation of these levels at the macroscopic level was undertaken through endoscopic imaging in five subjects. RESULTS: All temporal bones reviewed revealed the presence of a cartilaginous

Lectins selectively label cartilage condensations and the otic neuroepithelium within the embryonic chicken head.

Cartilage morphogenesis during endochondral ossification follows a progression of conserved developmental events. Cells are specified towards a prechondrogenic fate and subsequently undergo condensation followed by overt differentiation. Currently available molecular markers of prechondrogenic and condensing mesenchyme rely on common regulators of the chondrogenic program that are not specific to the tissue type or location. Therefore tissue-specific condensations cannot be distinguished based on known molecular markers. Here, using the chick embryo model, we utilized lectin labeling on serial

Microtia reconstruction with adjuvant 3-dimensional template model.

For auricular reconstruction of external auricle, the goals of the surgery are (1) in the fabrication of the morphologically complete 3-dimensional costal cartilage framework (3-dimensional frame) of the auricle, and (2) in the attainment of ample skin surface area to cover the grafted 3-dimensional frame, so that more than satisfactory results can be achieved. In review of the published literature, the cartilage framework used for auricular reconstruction is either insufficient or incomplete in the fabrication of the anatomic structures or inappropriate in proportion. We herein introduce the

Total auricular reconstruction with a three-dimensional costal cartilage framework.

Microtia (hypoplasia of the auricle) is known to result from incomplete development of the auricle during embryonic development so that the size and shape of the deformity vary from case to case. Total auricular reconstruction for microtia is dependent on the degree of hypoplasia in comparison to that of the normal auricle on which the specific reconstruction method is based. I have classified microtia into 3 major types: 1) The lobule type. 2) The concha type, and 3) The small concha type which has been previously described [32]. Regardless of the size and shape of the deformity, the ultimate

Reconstruction of lateral nasal defects.

In planning reconstruction of a lateral nasal soft tissue defect, the surgeon should be aware of alternative methods and select the one most appropriate for the patient. Variables to be considered include the extent, depth, and location of the defect, the age, sex, and tissue laxity of the patient, and the availability of donor tissue. Surface defects are often best corrected with full-thickness skin grafts: upper eyelid, preauricular, postauricular, or supraclavicular, depending on which portion of the nose is being treated. Anterior superior helix composite grafts serve quite nicely to resto

Modification of the second stage of auricular elevation in Nagata's technique for treating microtia.

The ultimate goal in treating congenital microtia is the reconstruction of an auricle with a natural appearance as close as possible to the healthy one in terms of shape, contour, size, texture, position, and projection. Here we present another option to the second stage of raising the ear in Nagata's technique for treating microtia. The first step includes implantation of a cartilaginous framework and transposition of the ear lobe. Raising the cartilage leaves a skin defect behind the auricle. Instead of using a superficial temporoparietal fascia flap as described by Nagata, we raise a simple

[Complications and failures of otoplasty].

Hematoma and infection are particularly serious true complications of otoplasty because of the fragile nature of the cartilage. Perichondritis and chondritis are happily extremely rare sequelae as they may provoke veritable disasters. In contrast, imperfect results due to poor evaluation of protrusion factors or an unadapted technique are encountered less rarely. Schematically they can be divided into anomalies of the external ear in relation to the skull and morphological alterations. The former include overcorrections, or more usually insufficient corrections which may be uni or bilateral, g
