Ruminations of a geriatric Emeritus Regius Professor of Medicine.

The potential for the holder of a senior academic post to influence the medical world is usually enshrined in their job description. Even though Oxford University failed to provide one, this account of 25 years of undirected activity suggests that such posts can influence events, albeit to a limited extent and not always in the expected direction.

[Current role of ultrasonography in orthopedics. Results of a nationwide survey].

The purpose of this study was to determine the current position of diagnostic ultrasound imaging in the day to day orthopaedic practice. A questionnaire was sent to all the 387 conservative and operative orthopaedic hospitals and orthopaedic university departments in Germany. 167 of these hospitals completed the forms and provided the basis for this evaluation (response rate 43.2%). Screening of the fetal hip for CDH was by far the most common indication, the shoulder was ranking second, the knee and the hip joint in children and adults rank third and fourth. Other frequent indications were in

Stapes surgery: implications for training.

The worldwide decline in surgery for otosclerosis had been reflected by a similar pattern here in Great Britain. Unless we are prepared to accept poorer results than have formerly been achieved, we will have to adopt measures to ensure that the skills of stapes surgery are transferred to the next generation of surgeons. Using the results of our experience, at the Queen Elizabeth Hospital, over the past 18 years, I have examined the implications for surgical training and for the management of future patients with otosclerosis.

Comparison of performances of transfer students and first-year acceptees.

Graduates of the Medical College of Pennsylvania during 1970-1977 were studied to determine the impact of three types of preclinical training on prelicensure clinical performance. The 586 students who comprised these groups consisted of 471 students admitted to the first year, 53 students admitted with advanced standing from U.S. medical or graduate programs, and 62 students admitted with advanced standing from foreign medical schools. Academic performances among the groups differed as measured by scores on the examinations of the National Board of Medical Examiners. However, perceived clinica

Give and Take of Operative Autonomy: Entrustment Process of Console Control Between Trainers and Trainees in Robotic-Assisted Surgery Training.

OBJECTIVE: In robotic-assisted surgery (RAS) training, console mediated control of the instruments and operative field, spatial configuration, and different systems change the training dynamics between trainers and trainees. Consequently, RAS changes the entrustment process or how operative autonomy is given and taken in surgical training. This study aims to examine entrustment in RAS training within a national training framework across multiple institutions, specialties, and factors within and beyond intraoperative interactions. DESIGN: We conducted semi-structured interviews and analyzed th

Normalized Competitive Index: An Updated Analysis of Trends in Surgical Fellowship Training Over the Past Decade (2015-2024).

OBJECTIVE: The normalized competitive index (NCI) was introduced as a metric describing surgical fellowship competitiveness. Such information has proven useful to prospective candidates, especially as immediate postresidency subspecialty training continues to be common for General Surgery and other surgical residencies. Using the NCI, we provide an updated analysis of the competitiveness of various surgical subspecialities to each other and themselves. DESIGN: Retrospective cohort study. The competitive index (CI) is defined as the percentage of programs filled divided by the match rate for t

An online survey of the structure and delivery of postgraduate psychiatric training in Nigeria.

BACKGROUND: Nigeria faces a severe shortage of mental health specialists, with fewer than 300 psychiatrists serving a population exceeding 234 million. Strengthening postgraduate psychiatric training is therefore critical, yet empirical evidence on the structure and implementation of residency programmes in Nigeria remains limited. This study aimed to systematically assess the structure and delivery of postgraduate psychiatric training across all accredited training centres in Nigeria, evaluate compliance with regulatory college standards, and identify actionable areas for improvement. METHOD

Building a neuroanesthesia educational infrastructure for perioperative neurosurgical patient care.

PURPOSE OF REVIEW: This article aimed to examine the value of education and training within perioperative neuroscience and its development for supporting the delivery of high-quality, safe patient care. RECENT FINDINGS: Perioperative neuroscience is a rapidly evolving field in anesthesiology, with current research demonstrating the potential for neurological injury in patients undergoing nonneurological and neurological surgery. Expertise is essential to mitigate risk. Current evidence in perioperative neuroscience suggests a patient benefit with subspecialty training. Advanced training progr

Why do so many in academic health care tend to assume that subspecialty surgical training begets higher quality health care than general pediatric surgical training?

BACKGROUND: The author spent all his attending level practice in general hospitals, resulting in early exposure to developing technologies, allowing early adaptation. ILLUSTRATIVE EXAMPLES: The adaptation of balloon dilation of arterial stenoses to treat esophageal strictures in children, and the use of MRI in anorectal malformations. RESULTS: Balloon dilatation of esopageal strictures and MRI assessment of anorectal anomalies became routine care. CHANGES IN ETHICAL LANDSCAPE: Both of these innovations were discussed with the Ethics Committee. Neither were subjected to further ethical overs

Factors associated with matching into pediatric ophthalmology and strabismus fellowship: a San Francisco Match analysis.

PURPOSE: To identify factors associated with matching into pediatric ophthalmology and strabismus fellowships. METHODS: We analyzed deidentified San Francisco Match data from 2021-22 to 2023-24. Categorical variables were compared using χ2 or Fisher exact tests. Continuous variables were summarized as medians with interquartile ranges. Factors associated with match outcome were assessed with multivariable multinomial logistic regression. RESULTS: We analyzed 217 applicants, of whom 129 (59%) matched into pediatric ophthalmology, 29 (13%) matched into other ophthalmology subspecialties, and 5

Building Connection and Resident Understanding of Local Resources Through Community Engagement.

INTRODUCTION: Throughout graduate medical education (GME), it is crucial for learners to not only develop the skills necessary to manage a wide variety of medical conditions, but also to foster personal development and to gain a deeper understanding of the complex and multifaceted needs of our patients. We often refer patients to community sites to address needs such as homelessness, hunger, and domestic violence; however, we frequently make these referrals with only a superficial understanding of what each resource entails. METHODS: To address this issue, our department integrated a two-day

Medical Student Performance Evaluations for International Medical Graduates: Variability in Content and Standardization.

OBJECTIVE: The Medical Student Performance Evaluation (MSPE) could be a standard objective measure to evaluate international medical graduates (IMGs) in the residency application. The purpose of this study was to evaluate whether international MSPEs are compliant with best-practice guidelines and whether the content allows for meaningful comparisons with other candidates. DESIGN: This was a retrospective cohort study of applications submitted in 2023 from 117 international medical schools to the obstetrics & gynecology (OB/GYN) residency program at a single institution. One representative MSP

The experience of pedagogical training on postgraduate rehabilitation health professionals: A qualitative study.

INTRODUCTION: Health professionals that act as lecturers in higher education necessitate the acquisition of pedagogical skills along with clinical ones. Consequently, pedagogical training courses have been introduced as part of professional development or within university curricula. While several studies explored the experiences of attending courses on pedagogical methodology, there is a notable gap in the literature within the rehabilitation field. Hence, this qualitative study explored the experience of Italian postgraduate health professionals in rehabilitation about the experience of atte
