Surgical treatment of elbow contractures in postburn children.

One-hundred and forty-six postburn elbow contractures in children were classified into four categories--simple band, complex band (crosses shoulder and/or wrist joint), diffuse scar, and limited scar--to assess the results of surgical treatment. The best results were seen in children less than 5 years of age and in children with less than 50 percent third-degree total body surface area burns. Types of release included skin grafts, local flaps (with or without graft), and deep releases. Generally, good to excellent results were seen regardless of technique of release, and in no case was the pos…

Tennis elbow. The surgical treatment of lateral epicondylitis.

Of the 1,213 clinical cases of lateral tennis elbow seen during the time period from December 19, 1971, to October 31, 1977, eighty-eight elbows in eighty-two patients had operative treatment. The lesion that was consistently identified at surgery was immature fibroblastic and vascular infiltration of the origin of the extensor carpi radialis brevis. A specific surgical technique was employed, including exposure of the extensor carpi radialis brevis, excision of the identified lesion, and repair. The results at follow-up were rated as excellent in sixty-six elbows, good in nine, fair in eleven…

Arthroscopic Treatment for Coronoid Process Nonunion of the Elbow.

The coronoid process of the ulna (CP) plays a crucial role in elbow stability by preventing posterior dislocation and resisting posterior muscular forces. Isolated CP of the ulna nonunions are rare but can lead to chronic instability, pain, and functional impairment. Traditional management relies on open techniques with internal fixation, but these approaches are invasive and risk fragment devascularization. With advances in arthroscopy, we describe a fully arthroscopic technique for treating CP nonunions using transosseous suture fixation with no bone graft. This minimally invasive approach o…

Anatomical explanation of the screw motion of the elbow joint.

HYPOTHESIS AND BACKGROUND: This study aimed to explore the anatomical factors contributing to the screw motion of the elbow joint during a flexion/extension movement of the elbow joint. Historically, the humero-ulnar joint has been simplified to a symmetrical, mono-axial hinge joint. More recent studies suggest an asymmetrical screw motion or helical movement during the flexion movement. This subtle varus-valgus motion during flexion and extension likely has evolutionary explanations. When the arm is in a dependent position (upper arm next to the body), the valgus angle of the elbow allows the…

Surgical Approaches to the Elbow in Fixation of Traumatic Injuries.

Elbow fractures and fracture-instability injuries are increasingly prevalent. Gaining adequate exposure to the joint surface poses considerable challenges due to the unique articular anatomy and intricacies of the neurovascular structures traversing the elbow. Laterally, the radial head and capitellum are accessed through a classic Kocher or variations of the Kaplan technique. Posteriorly, the distal humerus is exposed by retracting the extensor mechanism through triceps split, reflection, or olecranon osteotomy. The medial approach may expose the coronoid through or around the flexor carpi ul…

Management of Severe Elbow Arthritis in a Young Patient.

Elbow osteoarthritis is occasionally primary and most often post-traumatic. Nonoperative treatment is frequently the best course of action. Available surgical options are governed by the location of cartilage deterioration, bone structure, and preoperative joint instability. Debridement of osteophytes can achieve modest, temporary increases in motion and comfort. Surgical management benefits from knowledge of the substantial varus torque that is exerted when the arm is abducted. In the elbow that is resurfaced or replaced, these forces may strain the soft tissue support and lead to premature s…

Elbow Arthroscopy for Treatment of Osteochondritis Dissecans of the Capitellum.

BACKGROUND: Osteochondritis dissecans (OCD) of the elbow is an idiopathic lesion of subchondral bone resulting in delamination and sequestration with or without articular cartilage involvement; for patients with persistent symptoms, surgical treatment is often recommended. While traditionally performed in the supine or prone position, arthroscopic debridement and microfracture of OCD of the capitellum in the lateral decubitus position permits strategic arm positioning and placement of portals to optimize visualization and reduce the risk of damage to neurovascular structures. INDICATIONS: Art…

Elbow Disorders in an Outpatient Clinic: An Epidemiological Study.

BACKGROUND: Elbow disorders are a common cause of pain and functional impairment in the adult population, yet there is a lack of comprehensive epidemiological studies describing their distribution. This study aims to fill this gap by analyzing the epidemiology of elbow complaints in a specialized orthopedic clinic. OBJECTIVE: To assess the prevalence, age distribution, and sex-related differences of the most common elbow disorders in an outpatient orthopedic setting. METHODS: A single-center cross-sectional study of medical records from patients treated between 2015 and 2024. Adults of both …

[Review of common shoulder and elbow pathologies in athletes].

Sport participation in Switzerland is steadily growing, with 8% sustaining injuries while practicing. Most popular sports include hiking, cycling, swimming, alpine skiing, and fitness. Thirty percent of shoulder injuries in urban areas are sport-related, mainly from football, cycling, and alpine skiing. Pathologies vary by arm movement : below horizontal (running, ice hockey), above the head (swimming), or throwing (handball). It is essential to differentiate biomechanical adaptations from actual injuries. This article summarizes common pathologies and highlights surgical management of less fr

Acute elbow dislocation: a critical appraisal of the literature.

BACKGROUND: The purpose of this study was to perform a narrative review of acute elbow dislocation (AED). There are certain aspects of the management of AED that are controversial, including type and length of immobilization, indications for surgery, type of surgery, and new evidence available. MATERIAL AND METHODS: A literature search was performed using MEDLINE and Embase databases for studies regarding AED. Preference was given to studies according to their level of evidence. Studies regarding the outcome of conservative and surgical treatment, including patient-reported outcomes, complica…

Conservative therapy in ulnar neuropathy at the elbow (Review).

Ulnar neuropathy at the elbow (UNE) is the second most frequent entrapment syndrome in the upper limb after carpal tunnel syndrome. Clinical features are validated through electromyographic and sonographic examination. Although the two aforementioned entrapment syndromes share common pathophysiological traits, the conservative treatment approach for mild and moderate cases of UNE differs from that for median nerve entrapment. The present study identified 23 different types of scientific articles aimed to address this issue. The research stressed the importance of patient education and activity…
