Safe introducer technique for pacemaker lead implantation.

Over the last several years, an introducer approach for pacemaker lead insertion has evolved that eliminates most introducer-related complications. The approach consists of defining a safe region for intrathoracic cannulation of the subclavian vein. If specific conditions cannot be met for entering the "safe" region or if the vein cannot be found, the subclavian vein is cannulated extrathoracically. Recently, this technique was used in 263 consecutive patients undergoing pacemaker implantation. The intrathoracic portion of the subclavian vein was used in 239 (90.9%) cases and the extrathoracic

Chronic unlimited recording electrocorticography-guided resective epilepsy surgery: technology-enabled enhanced fidelity in seizure focus localization with improved surgical efficacy.

OBJECT: Epilepsy surgery is at the cusp of a transformation due to the convergence of advancements in multiple technologies. Emerging neuromodulatory therapies offer the promise of functionally correcting neural instability and obviating the need for resective or ablative surgery in select cases. Chronic implanted neurological monitoring technology, delivered as part of a neuromodulatory therapeutic device or as a stand-alone monitoring system, offers the potential to monitor patients chronically in their normal ambulatory setting with outpatient medication regimens. This overcomes significant

Single pulse electrical stimulation in white matter modulates iEEG visual responses in human early visual cortex.

INTRODUCTION: Electrical stimulation is increasingly used to modulate brain networks for clinical purposes. The basic unit of neurostimulation, a single electrical pulse, can travel through white matter to influence connected neuronal populations. However, the mechanisms by which it influences connected populations is not well understood: stimulation may excite, inhibit, or add noise to neuronal population activity. MATERIALS AND METHODS: In this study, we investigated how single pulses modulate the neuronal processing of images in a well-controlled visual paradigm. In two human subjects impl

Is it possible to generate an additional pleasant and pain-relieving muscle stimulation when using a low-frequency spinal cord stimulation (SCS) for the treatment of lower back pain? Pilot study: A new technique: "MuscleSCS".

OBJECTIVE: The combined use of spinal cord stimulation (SCS) and muscle stimulation, in the treatment of chronic pain, using the same probe, could improve the clinical results. However, this technique has not been established as yet. It was our hypothesis that it is possible to generate muscle stimulation by using low frequencies with SCS electrodes and use it to additionally treat chronic back pain. METHODS: We generated muscle stimulation in patients with previously implanted SCS electrodes, for the treatment of lower back pain, by using low frequencies (2, 4, 6, and 8 Hz) and different con

A score to map the lateral nonprimary motor area: Multispectrum intrinsic brain activity versus cortical stimulation.

OBJECTIVE: Multispectrum electrocorticographic components are critical for mapping the nonprimary motor area (NPMA). The objective of this study was to derive and validate a reliable scoring system for electrocorticography-based NPMA mapping (NPMA score) to replace electrical cortical stimulation (ECS) during brain surgery. METHODS: We analyzed 14 consecutive epilepsy patients with subdural electrodes implanted in the frontal lobe at Kyoto University Hospital. The NPMA score was retrospectively derived from multivariate analysis in the derivation group (patients = 7, electrodes = 713, during

Comparison between Intramuscular Multichannel Electrodes and Supramysial Multichannel Electrodes via EMG Measurements for Potential Use as Larynx Stimulation Electrodes: In Vivo Animal Analysis.

One of the most common causes for larynx paralysis is the injury of the recurrent laryngeal nerve which, among others, causes the paralysis of the posterior cricoarytenoideus muscle (PCA). Electrical stimulation of PCA offers an approach to retaining the function of the paralyzed larynx muscle. The study aim was to test the applicability of an intramuscular multichannel array electrode as a measuring electrode for myoelectrical potentials and as a possible electrode for stimulation, e.g., posterior cricoarytenoideus muscle stimulation. For this purpose, two different kinds of electrodes were c

Uneven interhemispheric connections between left and right primary sensori-motor areas.

To clarify the characteristics of interhemispheric connections, we investigated cortico-cortical evoked potentials (CCEP) in human. Fourteen patients with temporal lobe epilepsy who underwent invasive EEG monitoring with bilaterally implanted subdural electrodes were studied. Electric pulse stimuli were given in a bipolar fashion at two adjacent electrodes on and around the motor area (MA) or sensory area (SA), and CCEP responses were recorded by averaging electrocorticograms from the contralateral hemisphere. Seventy-two pairs of electrodes were stimulated, and 468 recordings were analyzed. F

Two-channel gastric electrical stimulation accelerates delayed gastric emptying induced by vasopressin.

The aim of this study was to investigate the effects of two-channel gastric electrical stimulation (GES) on delayed gastric emptying, gastric dysrhythmias, and motion sickness-like symptoms induced by vasopressin. Seven dogs implanted with four pairs of gastric electrodes and a duodenal cannula were studied in four randomized sessions (saline, vasopressin, single-channel GES, and two-channel GES). The experiment in each session was conducted sequentially as follows: 30-min baseline, ingestion of a liquid meal, 30-min iv infusion of vasopressin or saline, and two 30-min postprandial recordings.

Pectoral implantation of single-lead implantable cardioverter-defibrillators in patients without transvenous access.

With the introduction of the single-lead "Active-Can" implantable cardioverter-defibrillators, the implantation of the internal defibrillators has become a technically easy procedure. With these systems lowest defibrillation thresholds are achieved with a very low complication rate. For patients with thrombosis of both subclavian veins, however, a transvenous implantation technique is not possible. These patients are still equipped with epicardial patch electrodes. This article describes an alternative technique for implantation of this system in such patients, eliminating the need for epicard

Cochlear implantation in ossified cochlea-Merf experience.

Labyrinthitis Ossificans is a sequelae secondary to meningitis wherein ossification of the cochlea may be partial or complete. Among four of our patients who underwent cochlear implantation, two patients required double array insertion while the other two had a partial insertion of straight array electrodes. Despite partial insertion of electrodes, no significant differences in auditory benefit was noted&matched the peers who had complete insertion.

[A case of permanent pacemaker implantation in neonate using steroid-eluting myocardial pacing leads].

A 0-day-old infant with symptomatic congenital atrioventricular block diagnosed by fetal echocardiography was successfully treated with the pacemaker implantation. Two steroid-eluting myocardial electrodes were fixed on the wall of the right ventricle. Steroid-eluting myocardial electrode is smaller than the others, stab-in and screw-in leads, so even in neonate we were able to place the leads by the subxyphoid approach. The postoperative thresholds were excellent.

Depth versus surface: A critical review of subdural and depth electrodes in intracranial electroencephalographic studies.

Intracranial electroencephalographic (IEEG) recording, using subdural electrodes (SDEs) and stereoelectroencephalography (SEEG), plays a pivotal role in localizing the epileptogenic zone (EZ). SDEs, employed for superficial cortical seizure foci localization, provide information on two-dimensional seizure onset and propagation. In contrast, SEEG, with its three-dimensional sampling, allows exploration of deep brain structures, sulcal folds, and bihemispheric networks. SEEG offers the advantages of fewer complications, better tolerability, and coverage of sulci. Although both modalities allow e

Three-dimensional multilayer concentric bipolar electrodes restrict spatial activation in optic nerve stimulation.

Objective.Intraneural nerve interfaces often operate in a monopolar configuration with a common and distant ground electrode. This configuration leads to a wide spreading of the electric field. Therefore, this approach is suboptimal for intraneural nerve interfaces when selective stimulation is required.Approach.We designed a multilayer electrode array embedding three-dimensional concentric bipolar (CB) electrodes. First, we validated the higher stimulation selectivity of this new electrode array compared to classical monopolar stimulation using simulations. Next, we compared themin-vivoby int

Vestibular peripheral function remains robust after two weeks of continuous ionic direct current stimulation.

Objective.Conventional neural prostheses use brief charge-balanced pulses to minimize the risk of electrode polarization and irreversible electrochemistry at the electrode-electrolyte interface, constraining the waveforms that can be delivered long-term. Direct current (DC) stimulation has shown advantages, such as direct excitation and inhibition of tissue, but priorin vivoreports have been largely limited to acute durations. This study explores the physiological and histological effects of prolonged DC stimulation at amplitudes effective for neuromodulation while isolating tissue from the el

[The epifacial electric stimulation of the occipital nerve in cases of therapy-resistant neuralgia of the occipital nerve].

BACKGROUND: The term "neuralgia of the occipital nerve" in clinical work includes different kinds of pain in the occipital region. Correctly, this diagnosis should be reserved for such pain syndromes which, corresponding to the definition of neuralgia, are due to damage of the cervical roots C1-C3 or of the major occipital nerve arising from those roots. We introduce an operative method to treat chronic therapy-resistant headaches in the area of the major and minor occipital nerve. The courses of three of the first patients operated in our hospital are described. METHODS: An electrode with fo

Does electrode polarity alter the energy requirements for transthoracic biphasic waveform defibrillation? Experimental studies.

BACKGROUND: Electrode polarity may alter the success of biphasic shocks from implantable systems. Whether the electrode polarity influences the success of transthoracic biphasic defibrillation is unknown. We determined the effect of electrode polarity on biphasic transthoracic defibrillation in a porcine model. METHOD: In ten anesthetized adult pigs, 16-28 kg, electrode pads were placed in two different orientations on the chest wall; apex-right parasternal and sternal-vertebral column. Ventricular fibrillation (VF) was electrically induced and allowed to persist for 30 s. Truncated exponenti

Impedance changes in chronic vagus nerve stimulator implantation in epileptic dogs.

Computational modelling of the electrical fields generated by helical electrodes surrounding the vagus nerve suggested a current of 1.0 mA should depolarise enough axons to give a therapeutic effect in epilepsy. An assumption that fibrous tissue formation would form between the electrodes and the nerve in implantable vagus nerve stimulators (VNS) led to the recommendation that 1.5 mA should be used to account for the increased impedance this would cause. This increase in impedance has not been documented, meaning that the higher current might not be necessary and could produce avoidable advers

Partial deafness cochlear implantation at the University of Kansas: techniques and outcomes.

BACKGROUND: One of the most significant recent advances in cochlear implantation is the implantation of patients with residual hearing. These patients have a downsloping sensorineural hearing loss with poor speech discrimination and perform poorly with standard amplification. Studies using a variety of different electrode designs have demonstrated that it is possible to implant an inner ear and preserve residual hearing. Initial studies have demonstrated that a combination of residual acoustic hearing in the low frequencies with electrical stimulation in the mid- to high frequencies resulted i
