Prospective analysis of nonadherence in autoimmune hepatitis: a common problem.

OBJECTIVES: To prospectively assess nonadherence to medications, the relationship between nonadherence and medical outcome and the relationship between a psychiatric risk factor (posttraumatic stress) and nonadherence in patients with a diagnosis of autoimmune hepatitis. PATIENTS AND METHODS: Data were obtained in children with autoimmune hepatitis, who had consented to prospective monitoring of adherence, during 1 year of follow-up in our pediatric liver program. An electronic monitoring device as well as posttransplant trough blood levels of tacrolimus was used to evaluate adherence. A vali

Factor structure and validity of the Medication Adherence Questionnaire (MAQ) with cigarette smokers trying to quit.

The Medication Adherence Questionnaire (MAQ) is a scale used to evaluate adherence to medications. The present study assessed the factor structure and validity of the MAQ with cigarette smokers. A principal components analysis was conducted on MAQ scores from a sample of smokers presenting for treatment in a clinical trial of naltrexone and nicotine patch for smoking cessation (N = 385). Indices of convergent and predictive validity were tested using electronic medication caps for naltrexone, nicotine patch counts, plasma drug levels of naltrexone, and treatment outcomes. The principal compone

Standardizing Tracheostomy Discharge Education Using an Interdisciplinary Hand-off Form: A Quality Improvement Study.

BACKGROUND: Adults discharged home with a tracheostomy face preventable risks of tube dislodgement, obstruction, infection, and delayed emergency response. Consistent discharge education and interdisciplinary communication can support safe transitions from hospital to home. LOCAL PROBLEM: On an adult surgical telemetry unit at an academic medical center, tracheostomy discharge education and documentation were inconsistent across the electronic health record. Variable handoff practices between registered nurses and respiratory therapists contributed to fragmented care. METHODS: This quality i

Improving alcohol-related care in small-medium primary care practices: An evaluation of an adaptation of the SPARC trial intervention for small-medium sized practices.

INTRODUCTION: The Sustained Patient-centered Alcohol-Related Care (SPARC) trial demonstrated that 6 months of practice facilitation, decision support in electronic health records (EHRs) and performance feedback, increased identification and treatment of unhealthy alcohol use (UAU) in primary care (PC). The Michigan SPARC (MI-SPARC) study tested an adaptation of SPARC for small-medium PC practices. METHODS: PC practices were recruited for and participated in alcohol-related quality improvement (2/2020-2/2023). Outcomes collected for quality improvement were used for this evaluation, with data

Inpatient Age-Friendly 4Ms Care and Outcomes in an Urban Academic Medical Center.

The Age-Friendly Health Systems (AFHS) movement has advanced adoption of the 4Ms (What Matters, Medication, Mentation, and Mobility) framework, but health systems need evidence of whether or not adherence is associated with outcomes. The authors describe an effort that adapted and implemented previously developed electronic health record (EHR)-based 4Ms measures at a large academic medical center. In a retrospective cross-sectional analysis, the authors measured encounter-level adherence to 4Ms care for patients 65 years of age and older hospitalized for more than 24 hours between December 202

Pictographs: feasibility and acceptability of a novel method of newborn identification to reduce wrong-patient errors in the NICU.

Wrong-patient errors cause serious harm in newborns. These errors involve ordering and administering tests, procedures, medications, and breast milk to an unintended patient. Newborns receiving care in neonatal intensive care units (NICUs) are at particularly high risk. Although more distinct newborn naming conventions as recommended by the Joint Commission significantly reduce wrong-patient orders, name similarities among multiple-birth infants and truncation of differentiating information in some electronic health record (EHR) systems contribute to this persistent increased risk. Accordingly

From Report to Record: Prompt-Based Information Extraction from Gynecology Oncology Reports Using LLMs.

The growing capabilities of Large Language Models (LLMs) in understanding and generating clinical text are transforming the processing of unstructured medical data. This study presents a prompt-based framework for extracting structured information from Multidisciplinary Tumor Board (MTB) reports on ovarian cancer, written in Italian. Focusing on the anamnestic section, the main features (about Demographic, Scores, Comorbidities, Treatments and Gynecological History) are extracted through in-context learning prompts, and evaluated with the LLM-as-a-judge majority-voting scheme. Results show hig

Clustering-based federated causal discovery for multicenter clinical data analysis.

OBJECTIVE: Traditional causal structure learning algorithms struggle in distributed and privacy-sensitive environments, particularly when dealing with non-independent and identically distributed (non-IID) data. To address these limitations, this study proposes the Clustering-Based Federated Causal Discovery (CFedCD) framework, designed to enhance causal learning accuracy and applicability in multicenter clinical data analysis. METHODS: The CFedCD framework integrates deterministic representation encoding and federated optimization techniques to address the data heterogeneity and privacy const

Development and Validation of an Automated Acute Kidney Injury E-Alert System Integrated with Clinical Decision Support for Hospitalized Patients.

Automated electronic alerts integrated into inpatient electronic health records (EHRs) may improve recognition of moderate-to-severe acute kidney injury (AKI) and support timely medication review. An AKI e-alert system integrated with medication-focused clinical decision support (CDS) was implemented to identify adults with KDIGO stage 2-3 hospital-acquired AKI in near real time while minimizing alert burden. The alert logic used KDIGO serum creatinine (SCr) criteria, defined the operational real-time inpatient baseline as the lowest SCr value within the preceding seven days, and applied suppr

National trends in oncology specialists' EHR inbox work, 2019-2022.

BACKGROUND: Electronic health record (EHR) burden is an important driver of the ongoing physician burnout crisis. In particular, EHR-based messaging (also known as "inbox")-including messages from patients-is associated with burnout and decreased well-being. Little is known about EHR messaging burden for oncologists. To address this gap, we assessed trends in oncologist EHR messaging volume and EHR time from 2019 to 2022 across oncology subspecialties. METHODS: This study used EHR metadata for all US oncology physicians (including medical oncologist/hematologists, radiation oncologists, pedia

Enhancing Clinical History Taking Through the Implementation of a Streamlined Electronic Questionnaire System at a Pediatric Headache Clinic: Development and Evaluation Study.

BACKGROUND: Accurate history taking is essential for diagnosis, treatment, and patient care, yet miscommunications and time constraints often lead to incomplete information. Consequently, there has been a pressing need to establish a system whereby the questionnaire is duly completed before the medical appointment, entered into the electronic health record (EHR), and stored in a structured format within a database. OBJECTIVE: This study aimed to develop and evaluate a streamlined electronic questionnaire system, BEST-Survey (Bundang Hospital Electronic System for Total Care-Survey), integrate

Evaluation of matched control algorithms in EHR-based phenotyping studies: a case study of inflammatory bowel disease comorbidities.

The success of many population studies is determined by proper matching of cases to controls. Some of the confounding and bias that afflict electronic health record (EHR)-based observational studies may be reduced by creating effective methods for finding adequate controls. We implemented a method to match case and control populations to compensate for sparse and unequal data collection practices common in EHR data. We did this by matching the healthcare utilization of patients after observing that more complete data was collected on high healthcare utilization patients vs. low healthcare util

Diagnosis of neuronal ceroid lipofuscinosis by ultrastructural examination of peripheral blood lymphocytes.

A 7-year-old child with a history of seizures, psychomotor regression, and progressive visual loss was found to have juvenile type neuronal ceroid lipofuscinosis on the basis of characteristic ophthalmoscopic and electroretinographic findings. Although transmission electron micrographs of two separate skin biopsy specimens were normal, peripheral blood lymphocytes were vacuolated and demonstrated, by ultrastructural examination, characteristic "fingerprint profiles" confirming the diagnosis. Peripheral blood lymphocytes from the patient's mother, who had an acquired seizure disorder, demonstra
