[A new technique of aortic occlusion in patient with severe aortic calcification].

A 63-year-old woman with severe aortic valve insufficiency and bilateral subclavian artery occlusion underwent ascending aorta-to-biaxillary artery bypass and aortic valve replacement, simultaneously. Because of severe aortic calcification, a hand-made aortic occluder was utilized to prevent embolic stroke. We believe that the new technique of aortic occllusion might be very useful for patients with severe aortic calcification.

Successful thrombectomy in acute terminal internal carotid occlusion using a basket type microsnare in conjunction with temporary proximal occlusion: a case report.

Recanalization therapy is controversial in patients with acute stroke due to terminal internal carotid artery (ICA) occlusion. A 78-year-old man underwent successful percutaneous transarterial microsnare-assisted thrombectomy (PMT) for the treatment of acute terminal ICA occlusion, which was recanalized completely. This case report suggests that PMT is a potential treatment for patients with acute embolic occlusion of the terminal ICA.

Device closure of a patent foramen ovale in a patient with lipomatous hypertrophy of the atrial septum.

The presence of a patent foramen ovale has been recognized as a risk factor for embolic stroke, and transcatheter device closure of patent foramen ovales is increasingly performed to reduce future neurological events. The present report discusses a patient who continued to have residual shunting following percutanous closure because of improper seating of the device in the presence of a lipomatous atrial septum.

Hemorrhagic cerebral infarction in pediatric patients.

Two patients with ischemic strokes, presumably due to embolism, developed spontaneous hemorrhagic cerebral infarctions which were detected by follow-up cranial computed tomography. Clinical deterioration was associated with hemorrhagic transformation in one patient. In each case, immediate anticoagulant therapy was considered but was deferred. Implications of the use of anticoagulant therapy in pediatric patients with acute embolic stroke are discussed.

Stroke and Noninfective Native Valvular Disease.

PURPOSE OF REVIEW: Embolic stroke of undetermined source is a challenging clinical entity. While less common than atrial fibrillation and endocarditis, many noninfective heart valve lesions have been associated with stroke and may be considered as culprits for cerebral infarcts when other more common causes are excluded. This review discusses the epidemiology, pathophysiology, and management of noninfective valvular diseases that are commonly associated with stroke. RECENT FINDINGS: Calcific debris from degenerating aortic and mitral valves may embolize to the cerebral vasculature causing sma…

Late Embolization of a Patent Foramen Ovale Occluder Successfully Retrieved Percutaneously.

BACKGROUND: Percutaneous patent foramen ovale (PFO) closure is an established therapy for secondary prevention of cryptogenic stroke. Device embolization is a rare but potentially serious complication, usually occurring early after implantation. CASE SUMMARY: A 68-year-old woman underwent PFO closure after ischemic stroke. At 3-month follow-up, the device was not visualized on transthoracic or transesophageal echocardiography. Computed tomography angiography revealed embolization to the distal abdominal aorta. The device was successfully retrieved using a percutaneous snare technique; retriev

Mechanical thrombectomy for pediatric stroke arising from an atrial myxoma: case report.

Children experiencing severe neurological deficit due to acute ischemic stroke may benefit from endovascular intervention. The authors describe the use of mechanical thrombectomy in the treatment of embolic occlusion secondary to an atrial myxoma in a pediatric patient. This case involved an 11-year-old boy with a history notable for Raynaud syndrome and a distal extremity rash who presented to the emergency department with dense hemiparesis secondary to thromboembolic occlusion of the M1 segment of the middle cerebral artery. Following mechanical thrombectomy, the patient's pediatric National

[Cardiogenic ischemic strokes: pathogenetic aspects].

Eighty-three patients with acute ischemic stroke (56 men and 27 women, mean age 90.1 +/- 10.8 years) were examined in order to elucidate the significance of paroxysmal heart rhythm disorders and silent myocardial ischemia and determine the pattern of hemorheological changes. Traditional clinical instrumental examinations were supplemented by Holter monitoring and measurements of a wide spectrum of hemostatic and hemorheological values. Cardiogenic ischemic strokes were pathogenetically heterogeneous. Holter monitoring helped detect the significance of paroxysmal atrial fibrillation in the path

Atrial fibrillation: natural history, complications, and management.

Atrial fibrillation is a clinically important arrhythmia that carries important prognostic and therapeutic implications. Hypertension, ischemic heart disease, and rheumatic valvular disease are the commonest causes of atrial fibrillation. The presence of chronic or paroxysmal atrial fibrillation places the patient at increased risk for embolic stroke and/or death. When atrial fibrillation develops, there is loss of the atrial transport factor ("atrial kick"), with consequent decrease of cardiac output. Stroke output declines by 20-30% in normal individuals with loss of atrial kick; the decline…

Persistent intracranial steno-occlusion from calcified embolism: a treatment challenge.

INTRODUCTION: Calcified arterial cerebral embolism is a rare occurrence among large and medium vessel occlusions causing ischemic stroke and its diagnosis and treatment is a challenge. The sources of calcified embolism might be a calcific atheroma from the aortic arch and carotid artery, but also heart valve disease has been reported in the literature. Calcified embolism is frequently simultaneous on multiple vascular territories. The prognosis of patients is usually poor, including patients treated by using endovascular thrombectomy (EVT) and this diagnosis could be easily missed in the acute

Artificial intelligence for microembolic signal detection by transcranial Doppler in ischemic stroke: a mini-review.

Microembolic signals (MES) detected by transcranial Doppler (TCD) provide real-time information on ongoing embolic activity in patients with ischemic stroke and transient ischemic attack. MES have been associated with stroke recurrence and high-risk conditions including large-artery atherosclerosis, atrial fibrillation, moyamoya disease, cancer-related stroke, and complex aortic arch plaques. Despite its clinical value, conventional TCD is limited by operator dependency, suboptimal acoustic windows, and limited ability to discriminate embolus characteristics. Recent advances in artificial inte

Complex decision-making in stroke: preoperative mechanical thrombectomy of septic embolus for emergency cardiac valve surgery.

Stroke is a common and devastating embolic manifestation of infective endocarditis. We report a case of cardioembolic stroke in a patient with enterococcal endocarditis, with National Institutes of Health Stroke Scale score of 3. A middle-aged patient with bacterial endocarditis exhibited mild intermittent left hemiparesis and dysarthria in the setting of severe aortic insufficiency requiring urgent aortic valve replacement. Cerebrovascular imaging revealed a partially occlusive thrombus in the M1 segment of the right middle cerebral artery, which became symptomatic during relative hypotension…

Anticoagulation for atrial fibrillation.

For over two decades, valuable insights have been accumulated from epidemiologic studies and randomized trials about the risks for and prevention of AF-related stroke. AF substantially raises the risk of stroke, most likely through an atrio-embolic mechanism. Warfarin and other members of its class of oral anticoagulants targeted at an INR of 2.5 can abrogate the risk of stroke attributable to AF effectively and fairly safely. High-quality management of anticoagulation can be achieved in usual clinical care. These insights have important implications for the care of individual patients and mor

Wall Shear Stress Associated with Stroke Occurrence and Mechanisms in Middle Cerebral Artery Atherosclerosis.

BACKGROUND AND PURPOSE: Various mechanisms are involved in the etiology of stroke caused by atherosclerosis of the middle cerebral artery (MCA). Here, we compared differences in plaque nature and hemodynamic parameters according to stroke mechanism in patients with MCA atherosclerosis. METHODS: Consecutive patients with asymptomatic and symptomatic MCA atherosclerosis (≥50% stenosis) were enrolled. MCA plaque characteristics (location and plaque enhancement) and wall shear stress (WSS) were measured using high-resolution vessel wall and four-dimensional flow magnetic resonance imaging, respec

Outcome after percutaneous closure of a patent foramen ovale using the Intrasept device: a multi-centre study.

OBJECTIVES: This multi-centre study reports safety and efficacy of percutaneous patent foramen ovale (PFO) closure, using the fourth generation Intrasept device. BACKGROUND: PFO has been associated with paradoxical embolism and cryptogenic stroke. Percutaneous PFO closure seems to reduce the risk for recurrent paradoxical thrombo-embolism. Currently, different devices are used for PFO closure. METHODS: Patients, who underwent a PFO closure with the Intrasept device (Cardia, Eagan, MN) between July 2002 and September 2006, were included in the study. The primary endpoint was defined as reoccu

Extracranial carotid artery. Current concepts of diagnosis and management.

Extracranial carotid artery disease is a frequent cause of transient ischemic attack (about 50%), but a much less common cause of cerebral infarction (about 15%). Transient ischemic attack almost invariably precedes strokes caused by extracranial carotid stenosis, but rarely heralds strokes that result from cardiogenic embolism or intracranial vascular disease. When extracranial carotid stenosis produces a transient ischemic attack or stroke, artery-to-artery embolism is the predominant mechanism. Asymptomatic significant (>50%) carotid stenosis poses special clinical questions in patients sch

Safety of anticoagulation after hemorrhagic infarction.

Cerebral hemorrhagic infarction visualized on CT, secondary to embolic stroke in an anticoagulated individual, is usually associated with clinically stable or improving neurologic signs; fear of transforming the hemorrhagic infarction into a hematoma, however, usually prompts cessation of anticoagulation until the blood has cleared on CT, despite the recognized risk of recurrent embolism during this non-anticoagulated period. We now report our experience with 12 patients with hemorrhagic infarction who remained anticoagulated. Eleven men and one woman, ages 33 to 77, developed hemorrhagic infa…
