Pediatric hernias and hydroceles. The urologist's perspective.

Pediatric inguinal hernias and hydroceles are due to incomplete or abnormal obliteration of the processus vaginalis. Surgical correction of these conditions is the most common surgical procedure performed on young children. The embryology, anatomy, evaluation, and management of pediatric inguinal hernias and hydroceles are reviewed. A thorough understanding of these topics will aid with the sometimes difficult decisions encountered in the care of these patients.

First cleft branchial fistula in a child--a modified surgical technique.

Congenital first branchial cleft fistulae, their embryology, anomalies, varied relationships to the facial nerve and surgical techniques for their excision have been well described in the literature. We report a case of a type II first cleft fistula in a three-year-old child which required a modification of the standard surgical approach to achieve safe and complete excision with identification and preservation of the facial nerve.

Paravaginal tumor: ureterolithiasis in an ectopic ureter.

A case of long-standing, undetected urinary tract malformation is presented. Over a period of 15 years the patient had gone for regular examinations by a gynecologist because of paravaginal tumor and urinary incontinence. Operation revealed urinary calculi in an ectopic ureter. Despite of life-long continuous urinary incontinence, an i.v. pyelography was never offered. Embryology, symptoms and an investigation program concerning ectopic ureter are presented.

[Infected cystic malformations of a seminal vesicle with ipsilateral uretero-renal agenesis. Description and discussion of the surgical approach].

The author reports a case of rare malformation affecting all of the elements derived from the Wolffian duct, the embryology of which is briefly reviewed. The clinical picture, as in the majority of previously published cases, was initially suggestive of acute prostatitis, but clinical, radiological and ultrasonographic examination revealed the malformation. The treatment is surgical and most authors prefer an intra-peritoneal transvesical approach.

[Anomalous Origin of the Ophthalmic Artery from the Anterior Cerebral Artery Associated with the Paraclinoid Internal Carotid Artery Aneurysm].

The ophthalmic artery usually arises from the supraclinoid portion of the internal carotid artery. Here, we present an extremely rare case of abnormal origin of the ophthalmic artery from the anterior cerebral artery associated with the paraclinoid internal carotid artery aneurysm. As the embryology of the ophthalmic artery is complex, this case provides additional insight into the variation of the ophthalmic artery.

A rare marginal tentorial artery to ophthalmic artery anastomosis.

We report a patient with a marginal tentorial artery to ophthalmic artery anastomosis. A middle-aged man presented with subarachnoid hemorrhage and underwent angiography, where selective microcatheter injection of the meningohypophyseal trunk and marginal tentorial artery revealed a collateral to the distal ophthalmic artery. The radiological findings, embryology behind ophthalmic artery anastomoses and the neurosurgical and neurovascular relevance are discussed.

Intussusception: an anatomical perspective with review of the literature.

Originally described over 300 years ago, the clinical scenario of intussusception remains incompletely understood. Intussusception is now one of the conditions that can be, most of the time, preoperatively diagnosed and treated with success. This article reviews the literature regarding this pathological state of the abdomen and discusses what is known regarding the presentation, diagnosis, embryology, and anatomy of intussusception.

A histologic review of vulvar inflammatory dermatoses and intraepithelial neoplasm.

Patients with urogenital complaints are frequently encountered by family practitioners, gynecologists, and dermatologists. This review article provides practical information regarding normal anatomy and embryology of the vulva, followed by a summary of procedural techniques. The new classifications of vulvar inflammatory and intraepithelial neoplasm proposed by the ISSVD are used as guidelines to review the wide spectrum of diseases involving this organ.

Congenital neck masses.

Congenital neck lesions reflect abnormal embryogenesis in head and neck development. A thorough knowledge of embryology and anatomy is critical in the diagnosis and treatment of these lesions. The appropriate diagnosis of these lesions is necessary to provide appropriate treatment and long-term follow up, because some of these lesions may undergo malignant transformation or be harbingers of malignant disease.

Congenital anterior penile isolated urethrocutaneous fistula: a case report.

Urethrocutaneous fistula is a common complication after hypospadias repair. If congenital, it is usually associated with other genitourinary and gastrointestinal anomalies. Isolated congenital urethral fistula is a very rare anomaly. We present a 4-year old circumcised boy with this unusual anomaly. Etiology, embryology, and management are discussed. We emphasize meticulous clinical examination for the diagnosis and to rule out other associated anomalies.

A rare craniofacial cleft: numbers 7, 2, and 3 clefts accompanied by a single median lip pit.

A unique case of a unilateral partial Tessier no. 7 cleft accompanied by no. 2 and 3 clefts along with a single median lip pit is presented. The history, genetics, epidemiology, classifications, embryology, pathogenesis, and correction techniques are briefly reviewed. After an extensive review of the literature, we conclude that this particular constellation of anomalies has not previously been described.

Second branchial cleft-pouch set fistulae, sinuses and cysts in children.

A perfect understanding of the embryology of the branchial apparatus and the cervical anatomy is absolutely essential for the classification and treatment of branchial fistulae, sinuses and cysts, particularly with respect to those of the second cleft-pouch set. Some of our recent cases provide evidence of the need for precise dissection, a condition sine qua non to avoid surgical failures.

Nonrotation of the midgut presenting in the adolescent and adult.

The most common abnormality of midgut rotation and fixation is nonrotation of the midgut. This most commonly presents in the neonatal period with duodenal obstruction or midgut volvulus. However, this condition can present outside the neonatal period as chronic abdominal pain, or as an abdominal emergency. Two such cases are reported. Normal and abnormal embryology is described, and diagnosis and management are discussed.

Introital adenosis associated with Stevens-Johnson syndrome.

A 23-year-old woman presented with vulvar pain and dyspareunia 1.5 years after a Stevens-Johnson syndrome. A red friable lesion was identified at the vestibule and lower third of the vagina. The histologic diagnosis was adenosis with a tubal glandular epithelium. The possible histogenesis of this lesion is discussed with a review of the embryology of the lower genital tract.

Congenital anomalies of the larynx.

Congenital anomalies of the larynx are infrequent, though potentially life-threatening, defects with which both pediatricians and otolaryngologists should be familiar. We reviewed the basic stages in normal laryngeal embryology and outlined a scheme for the examination of a neonate with a possible laryngeal abnormality. In particular, symptoms, laryngoscopic appearance, and treatment of the more commonly encountered anomalies should be considered.

Cysts of the female external genitalia in the newborn infant.

Nineteen cases of cysts of the external female genitalia were diagnosed over a two-year period, giving an incidence of six per 1,000 female infants. The embryology, morphology, and differential diagnosis are discussed. No treatment was given and follow-up at two months of age revealed complete resolution of the cysts, thereby establishing their benign course in the newborn period.

Branchial cysts within the parotid salivary gland.

Cystic lesions within the parotid gland are uncommon and clinically they are frequently misdiagnosed as tumours. Many theories have been proposed as to their embryological origin. A 20-year retrospective review was undertaken of all pathological codes (SNOMED) of all of patients presenting with any parotid lesions requiring surgery. After analysis seven subjects were found to have histopathologically proven parotid branchial cysts in the absence of HIV infection and those patients are the aim of this review. Four of the most common embryological theories are also discussed with regard to these

Septate uterus with cervical duplication: rethinking the development of müllerian anomalies.

We present a case of a septate uterus with duplication of the cervix and a vaginal septum, which challenges the embryologic concept of unidirectional caudad to cephalad müllerian ducts fusion. This unique case is, to the best of our knowledge, the seventh reported case of this müllerian anomaly. It challenges the classic theory of unidirectional caudad to cephalad fusion of the müllerian ducts, and might be additional evidence to support the hypothesis suggested by Musset et al. Accurate diagnosis and reports of such cases are important not only for the benefit of treatment, but to reflect the

Examination of the external ear.

Normal ears come in a wide variety of sizes and shapes, likely because of the impact of racial and familial characteristics (phenotypes), the complexity of the embryologic development (malformations), and the potential for intrauterine factors such as oligohydramnios or intrauterine compression to produce distinct physical characteristics in normally formed tissues (deformations). This article will provide an overview of the embryologic origins of the external ear. Relevant terminology used to describe the ear will be reviewed, along with objective guidelines for the assessment of ear size, sh
