Transtympanic electrocochleography in the assessment of perilymphatic fistulas.

An objective method for the pre-operative diagnosis and the post-operative assessment of a presumed perilymphatic fistula (PLF) using transtympanic electrocochleography is presented. Three cases are reported in which the history of the disease and the symptoms strongly suggested the presence of a PLF. Pre-operative transtympanic electrocochleography (TT ECoG) recordings at rest showed changes similar to those of endolymphatic hydrops and signs of instability of the inner ear hydrodynamic system during raised intrathoracic pressure. Surgery revealed a visible leak in two of the three cases. Bot…

Antibodies of type II collagen and immune complexes in Menière's disease.

In a previous study, we showed that endolymphatic hydrops was induced in 36% of guinea pigs immunized with type II collagen and that dominant negative summating potential (SP) was recorded for these hydropic animals. In the present study, we evaluated the levels of serum antibody to type II collagen and three other immune indexes in patients with Menière's disease, especially those who showed negative SP dominance in ECoG. Twenty-nine Menière's cases, 22 normal volunteers serving as controls, 28 rheumatoid arthritis (RA) cases, and 9 patients complaining of vertigo were examined to detect seru…

A Rare Case of Co-occurring Ménière's Disease and Red Ear Syndrome.

Ménière's disease (endolymphatic hydrops) is a multifactorial syndrome affecting the inner ear, typically distinguished by attacks of variable frequency which include vertigo, aural fullness, tinnitus, and fluctuating hearing loss. Red ear syndrome is characterized by paroxysmal erythema of the external ear, either unilaterally or bilaterally, with pain, burning sensation, and/or swelling. Both conditions have complex and heterogeneous etiologies. Ménière's disease and red ear syndrome have not previously been associated in the literature, yet they can have overlapping characteristics. This ca…

Effect of locally applied drugs on the endolymphatic sac potential.

In Ménière's disease, an inner ear disorder related to an endolymphatic hydrops, an alteration of the functioning of the endolymphatic sac has been proposed. The endolymphatic sac is assumed to be involved in the secretion/resorption of endolymph. The epithelial transport systems have been indirectly studied by the recording of the endolymphatic sac transepithelial potential (ESP) in control conditions and after the local injection of drugs such as diuretics that have been proposed in the treatment of Ménière's disease. The ESP was recorded, in vivo, in guinea pigs up to 150 minutes after the …

Ocular vestibular-evoked myogenic potentials in unilateral Ménière's disease: a narrative literature review.

The ocular vestibular-evoked myogenic potential (oVEMP) is a promising non-invasive tool for assessing utricular function and superior vestibular nerve integrity. In the context of definite unilateral Ménière's disease, studies have consistently reported altered oVEMP responses in affected ears, including altered amplitudes, higher stimuli needed before receiving a verifiable response or prolonged latencies. These findings are thought to reflect the presence and accumulation of endolymphatic hydrops. However, variability across studies is substantial. This inconsistency may reflect fluctuating…

Significance of an augmented response on cervical vestibular evoked myogenic potential testing in Meniere's disease.

The potential mechanism of augmented response on cervical vestibular evoked myogenic potential (cVEMP) testing and its decrease following treatment in a patient with Meniere's disease (MD) are discussed. Changes of static posturography and cVEMP testing before and after a glycerol drip in a 69-year-old man with unilateral MD, in which significant endolymphatic hydrops (EH) was confirmed on magnetic resonance imaging (MRI) on the diseased side, were evaluated. Values of total locus lengths, areas of postural sway, and their Romberg ratios were decreased after the glycerol drip. On cVEMP testing…

Short term effects of induced middle ear pressure changes on the electrocochleogram in Menière's disease.

Electrocochleographic recordings were made in 17 patients with Menièr's disease before and immediately after exposure to pressure applications in their middle ear. The variables of the response to clicks such as the SP/AP ratio and the width of the complex as well as the amplitudes of the SP in response to long tone bursts were evaluated. Improvement of electrocochleographic recordings, characterized by normalization of the waveforms and reduction of the SP, was found to be significant. The results indicate a mechanoelectric change in the hydroptic cochlea registered directly after exposure to

Local overpressure treatment reduces vestibular symptoms in patients with Meniere's disease: a clinical, randomized, multicenter, double-blind, placebo-controlled study.

OBJECTIVES: To evaluate the efficacy of a new device, the Meniett, in the treatment of Meniere's disease. The device delivers pressure pulses to the middle ear through a ventilating tube in the tympanic membrane at a frequency of 6 Hz for 0.6 second. After rising to a pressure level of 1.2 kPa, the pressure oscillates between 0.4 and 1.2 kPa. It is believed that the pressure changes are conveyed to the inner ear, inducing a transport of fluids via the pressure outlets and thus reducing the endolymphatic hydrops. STUDY DESIGN: A clinical, randomized, multicenter, double-blind, placebo-controll…

Meniere's disease: the selection and assessment of patients for surgery using electrocochleography.

Transtympanic electrocochleography has been used to measure the action potential (AP), summating potential (SP), and cochlear microphonic (CM) in patients with Menière's disease. In this condition the widening of the AP/SP complex is thought to be due to an enhanced negative SP which shows no adaptation and this may be the result of an increased asymmetry produced by endolymphatic hydrops. Measurements have also been made using two groups of agents -- those designed to increase the cochlear blood flow (the metabolic activator naftidrofuryl and inhalations of carbon dioxide) and those designed …

Use of narrow band imaging and 4K technology in otology and neuro-otology: preliminary experience and feasibility study.

The advent of the endoscope has facilitated otological surgery in recent years. Advances in optical technology have led to the development of high definition visualisation, such as 4K magnification. Narrow band imaging (NBI) has been shown to improve visualisation of tissue based on varying degrees of vascularity. This is a feasibility study, the first of its kind, aiming to investigate the appearances of middle ear anatomy and pathology (tympanic membrane, tympanosclerosis, cholesteatoma, granulation, endolymphatic sac hydrops, otosclerosis) under the NBI filter, using a high definition 4K en
