Unique and shared areas of cognitive function in children with intractable frontal or temporal lobe epilepsy.

OBJECTIVE: Previous findings have been mixed in terms of identifying a distinct pattern of neuropsychological deficits in children with frontal lobe epilepsy (FLE) and in those with temporal lobe epilepsy (TLE). The current study investigated the neuropsychological similarities and differences across these two pediatric medically intractable localization-related epilepsies. METHOD: Thirty-eight children with FLE, 20 children with TLE, and 40 healthy children (HC) participated in this study. A comprehensive battery of standardized tests assessed five neuropsychological domains including intell

Executive dysfunction is associated with an altered executive control network in pediatric temporal lobe epilepsy.

OBJECTIVES: Children with temporal lobe epilepsy (TLE) exhibit executive dysfunction on traditional neuropsychological tests. However, there is limited evidence of neural network alterations associated with this clinical executive dysfunction. The objective of this study was to characterize working memory deficits in children with TLE via activation of the executive control network on functional magnetic resonance imaging (fMRI) and determine the relationships to fMRI behavioral findings and traditional neuropsychological tests. EXPERIMENTAL DESIGN: Functional magnetic resonance imaging was c

Beyond depression: The impact of executive functioning on quality of life in patients with temporal lobe epilepsy.

OBJECTIVE: Individuals with temporal lobe epilepsy (TLE) often experience diminished quality of life (QoL). Although comorbid depression is one of the most recognized predictors of poor QoL in TLE, impairments in verbal memory (VM) and executive functioning (EF), have also been identified as risk factors, independent of other biological and psychosocial factors. In this study, we examine the contribution of depression, VM, and EF to QoL in 52 well-characterized medically-refractory TLE patients. METHODS: Quality of life was assessed with the Quality of Life in Epilepsy (QOLIE-31) questionnair

Comparative effectiveness of antiepileptic drugs in patients with mesial temporal lobe epilepsy with hippocampal sclerosis.

OBJECTIVE: Mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS) is a common epilepsy syndrome that is often poorly controlled by antiepileptic drug (AED) treatment. Comparative AED effectiveness studies in this condition are lacking. We report retention, efficacy, and tolerability in a cohort of patients with MTLE-HS. METHODS: Clinical data were collected from a European database of patients with epilepsy. We estimated retention, 12-month seizure freedom, and adverse drug reaction (ADR) rates for the 10 most commonly used AEDs in patients with MTLE-HS. RESULTS: Seven hundred si

The relationship between hippocampal volumes and nonverbal memory in patients with medial temporal lobe epilepsy.

OBJECTIVE: To explore the involvement of medial temporal lobe structures such as the hippocampus, amygdala, and entorhinal cortex (EC) in memory consolidation by volumetric magnetic resonance imaging (MRI). METHODS: Sixty-two consecutive patients with medial temporal lobe epilepsy (MMTLE) were assessed using the Clinical Memory Scale (CMS) and MRI to measure the volumes of the hippocampus, amygdala, and EC. Participants were grouped according to MRI findings into 3 groups: left MRI-positive (abnormal hippocampal formation on the left side; n=17), right MRI-positive (abnormal hippocampal forma

Receptor for Advanced Glycation Endproducts is upregulated in temporal lobe epilepsy and contributes to experimental seizures.

Toll-like receptor 4 (TLR4) activation in neuron and astrocytes by High Mobility Group Box 1 (HMGB1) protein is a key mechanism of seizure generation. HMGB1 also activates the Receptor for Advanced Glycation Endproducts (RAGE), but it was unknown whether RAGE activation contributes to seizures or to HMGB1 proictogenic effects. We found that acute EEG seizures induced by 7ng intrahippocampal kainic acid (KA) were significantly reduced in Rage-/- mice relative to wild type (Wt) mice. The proictogenic effect of HMGB1 was decreased in Rage-/- mice, but less so, than in Tlr4-/- mice. In a mouse mes

P-glycoprotein expression and function in patients with temporal lobe epilepsy: a case-control study.

BACKGROUND: Studies in rodent models of epilepsy suggest that multidrug efflux transporters at the blood-brain barrier, such as P-glycoprotein, might contribute to pharmacoresistance by reducing target-site concentrations of antiepileptic drugs. We assessed P-glycoprotein activity in vivo in patients with temporal lobe epilepsy. METHODS: We selected 16 patients with pharmacoresistant temporal lobe epilepsy who had seizures despite treatment with at least two antiepileptic drugs, eight patients who had been seizure-free on antiepileptic drugs for at least a year after 3 or more years of active

Detection of hippocampal atrophy in patients with temporal lobe epilepsy: a 3-Tesla MRI shape.

In patients with mesial temporal lobe epilepsy (MTLE), brain MRI often detects hippocampal sclerosis (HS). Almost half of patients with MTLE do not show any hippocampal damage on visual or volumetric assessment. Here, we wished to prospectively assess 65 patients with MTLE (41 women, mean age: 39±10years, range: 21-69; right (12/65 patients) (MRI-negative) nMTLE; right (14/65 patients) (MRI-positive with HS) pMTLE; left (24/65 patients) nMTLE; and left (15/65 patients) pMTLE) using shape analysis (SA). There were significant differences among pMTLE versus nMTLE for age at seizure onset (20.2±1

Incidental aneurysms in temporal lobe epilepsy surgery: report of three cases and a review of the literature.

OBJECTIVE AND IMPORTANCE: In rare instances, patients who are undergoing evaluation for temporal lobe epilepsy surgery will also be found to have an incidental aneurysm adjacent to the temporal lobe. Although busy epilepsy centers will eventually come across cases like this, to our knowledge there are currently no publications regarding such cases or the complex management decisions that follow in caring for these patients. Factors affecting decision making in these cases are discussed in detail. CLINICAL PRESENTATION: Three cases where an incidental aneurysm was discovered during the evaluat

Rapamycin suppresses axon sprouting by somatostatin interneurons in a mouse model of temporal lobe epilepsy.

PURPOSE: In temporal lobe epilepsy many somatostatin interneurons in the dentate gyrus die. However, some survive and sprout axon collaterals that form new synapses with granule cells. The functional consequences of γ-aminobutyric acid (GABA)ergic synaptic reorganization are unclear. Development of new methods to suppress epilepsy-related interneuron axon sprouting might be useful experimentally. METHODS: Status epilepticus was induced by systemic pilocarpine treatment in green fluorescent protein (GFP)-expressing inhibitory nerurons (GIN) mice in which a subset of somatostatin interneurons e

Changes in extratemporal integrity and cognition in temporal lobe epilepsy: a diffusion tensor imaging study.

BACKGROUND: Diffusion tensor imaging (DTI) is a sensitive technique for studying cerebral white matter. Only a few studies have examined the association between changes in extratemporal integrity and cognition in temporal lobe epilepsy (TLE), especially in Chinese patients. AIM: We used DTI to characterize microstructural changes and investigate their associations with cognition in patients with temporal lobe epilepsy. PATIENTS AND METHODS: We studied 27 adult patients with TLE and 21 healthy controls. A standardized neuropsychological evaluation and diffusion tensor imaging examination were

The mammalian target of rapamycin signaling pathway mediates epileptogenesis in a model of temporal lobe epilepsy.

Understanding molecular mechanisms mediating epileptogenesis is critical for developing more effective therapies for epilepsy. We recently found that the mammalian target of rapamycin (mTOR) signaling pathway is involved in epileptogenesis, and mTOR inhibitors prevent epilepsy in a mouse model of tuberous sclerosis complex. Here, we investigated the potential role of mTOR in a rat model of temporal lobe epilepsy initiated by status epilepticus. Acute kainate-induced seizures resulted in biphasic activation of the mTOR pathway, as evident by an increase in phospho-S6 (P-S6) expression. An initi

Behavioral alterations in a mouse model of temporal lobe epilepsy induced by intrahippocampal injection of kainate.

Patients with temporal lobe epilepsy are frequently afflicted with psychiatric comorbidity and deficits in spatial and other forms of declarative memory. The relationship between epilepsy and psychopathology is poorly understood, so that systematic research in this area is important. In the present study, we characterized various behaviors and learning and memory in a mouse model in which major aspects of mesial temporal lobe epilepsy can be reproduced. In this model, a single unilateral injection of kainate into the dorsal hippocampus induces a nonconvulsive status epilepticus, followed by de

Electrophysiological, neurochemical and regional effects of levetiracetam in the rat pilocarpine model of temporal lobe epilepsy.

This study compared levetiracetam (Keppra) with reference antiepileptic drugs (AEDs) in the rat pilocarpine model of temporal lobe epilepsy. Electroencephalogram (EEG) recordings showed that i.p. administration of valproate (300 mg/kg), phenobarbital (5 mg/kg) and clonazepam (0.5 mg/kg) all significantly delayed the appearance of the first epileptic spike discharge in hippocampus as well as synchronous epileptiform activity in hippocampus and cortex. In contrast, i.p. administration of levetiracetam (17 mg/kg) only significantly delayed the appearance of the latter. This was corroborated by fi

The clinical-pathogenic mechanisms of hippocampal neuron loss and surgical outcomes in temporal lobe epilepsy.

A retrospective study was carried out to determine whether a prior cerebral injury or medical illness was associated with hippocampal sclerosis in intractable, surgically treated temporal lobe epilepsy (TLE), or whether there was evidence for progressive hippocampal neuron damage from repeated seizures. Temporal lobe epilepsy patients (n = 162) from one epilepsy centre were retrospectively and blindly catalogued into groups based on the presence or absence of an initial precipitating injury (IPI) and whether, when an IPI was present, it had involved seizures (independent variables). Patients w

Localizing value of seizure manifestations of temporal lobe epilepsies and the consequence of analyzing their sequential appearance.

We investigated the localizing and lateralizing value of principal seizure manifestations in temporal lobe epilepsies (signal symptoms, oroalimentary automatisms, somatomotor manifestations, unilateral dystonic posturing, ictal speech, motionless stare) of 223 complex partial seizures in 50 patients. All the patients had invasive long-term monitoring with the combined implantation of intracerebral electrodes in and subdural electrodes on the bilateral temporal lobes. Postoperative freedom from seizures was ascertained for longer than one year. We found that 35 patients had amygdalohippocampal

Contralateral piriform cortex alterations after surgery for temporal lobe epilepsy: A pilot MRI volumetry study.

PURPOSE: Temporal lobe epilepsy (TLE) is increasingly conceptualized as a network disorder, with the piriform cortex as an implicated region. We characterized postoperative contralateral piriform cortex-cortical amygdala (PCA) structural and perfusion-related changes in a small seizure-free TLE cohort. METHODS: Nine patients with drug-resistant TLE who underwent epilepsy surgery and achieved postoperative seizure freedom were included in this single-center pre-post pilot study. Bilateral PCA was manually delineated on 3D T1-weighted MRI using ITK-SNAP, and volumes were normalized to total int

Disrupted modular and hub topology in right temporal lobe epilepsy: a multimodal MRI network analysis.

Right temporal lobe epilepsy (rTLE) is associated with disruptions in functional brain networks and structural connectivity, yet underlying mechanisms remain unclear. This study investigated the alterations in modular interactions, connector hub (CH) topology, and related structural changes in rTLE patients. It included 30 rTLE patients and 30 matched healthy controls (HCs), all of whom underwent resting-state functional MRI (rs-fMRI), diffusion-weighted imaging (DWI), and volumetric MRI (vMRI). Functional networks were analyzed by assessing modular interactions, functional connectivity (FC),

EEG microstate-derived dynamic network biomarkers for lateralization and structural etiology in temporal lobe epilepsy.

OBJECTIVE: Temporal lobe epilepsy (TLE) is the most common focal epilepsy but remains highly heterogeneous across hemispheric and structural etiology. This study aimed to characterize microstate-based network dynamics in TLE and evaluate their diagnostic value for seizure lateralization and structural etiology using machine learning. METHODS: Resting-state electroencephalography (EEG) recordings from 150 patients with unilateral TLE (71 right, 79 left) and 65 healthy controls (HCs) were analyzed. EEG signals were segmented into canonical microstates (A, B, C, D), and microstate-specific spati
