Free fibular transfer in a growing individual (long-term results).

We present a case report of a 4-year-old in whom the distal fibula and epiphyseal plate had been traumatically destroyed. A free vascularized epiphyseal transfer using the ipsilateral proximal fibula was performed which provided good bony stability at the ankle, as well as excellent long-term growth characteristics. We demonstrate immediate postoperative results, as well as long-term (5 years) follow-up.

[Chondrodysplasia punctata].

Authors deal with the classification of chondrodysplasia punctata, in detail they analyze the clinical and radiomorphological symptoms. From the diagnostic viewpoint it is important to evaluate typical X-ray epiphyseal changes in the first year of age of the affected child. Clinical symptoms vary and they are not typical only for this disease. They are often combined with various congenital developmental anomalies. The case study presents the correlation between X-ray epiphyseal changes in the knee and the arthroscopic findings.

A prediction model of pulmonary metastasis risk in pediatric patients with stage IIB osteosarcoma in the long bone of extremities.

INTRODUCTION: While epiphyseal plates may resist osteosarcoma invasion, the correlation between epiphyseal involvement (EI) and pulmonary metastasis (PM) or prognosis remains unclear, and no PM prediction models specifically target pediatric patients. MATERIALS AND METHODS: This study enrolled 221 patients (≤14 years) with stage IIB osteosarcoma in the long bone of extremities. Using LASSO and multivariate Cox regression analyses, we identified significant risk factors for PM and prognosis, integrating them into a nomogram and nine machine learning (ML) models. After comprehensive performance

Femoral morphology differs between deficient and excessive acetabular coverage.

UNLABELLED: Structural deformities of the femoral head occurring during skeletal development (eg, Legg-Calvé-Perthes disease) are associated with individual shapes of the acetabulum but it is unclear whether differences in acetabular shape are associated with differences in proximal femoral shape. We questioned whether the amount of acetabular coverage influences femoral morphology. We retrospectively compared the proximal femoral anatomy of 50 selected patients (50 hips) with developmental dysplasia of the hip (lateral center-edge angle [LCE] < or = 25 degrees ; acetabular index > or = 14 deg

Development of sensory innervation in rat tibia: co-localization of CGRP and substance P with growth-associated protein 43 (GAP-43).

The development of sensory innervation in long bones was investigated in rat tibia in fetuses on gestational days (GD) 16-21 and in neonates … and intercondylar eminence (GD21), the metaphyses (PD1), the cartilage canals penetrating into the epiphyses (PD7), and finally in the secondary ossification centres (PD10) and epiphyseal bone marrow (PD14). Maturation of the fibres, manifested by their immunoreactivity for CGRP and SP, was visible on GD21 in the epiphyseal perichondrium, the periosteum of …

The pathophysiology of osteochondrosis.

Osteochondrosis is a disorder of epiphyseal cartilage about which there is considerable confusion in the literature. We believe that this is due to the fact that osteochondrosis has been studied in the chronic stage when the lesions are morphologically complicated and the initial causative insult is impossible to determine. The etiology of osteochondrosis appears to be multifactorial, with trauma, hereditary factors and rapid growth, nutritional factors, and ischemia all having a role in its pathogenesis. Although predilection sites are variable among species, the morphology of the early lesio

[Symptomatic calcification in the newborn. Phenocopies of chondrodysplasia punctata].

Stippled epiphyses occur in the new-born and young infant in the different hereditary forms of chondrodysplasia punctata. Symptomatic stippling has been described also in association with chromosomal anomalies, gangliosidosis and drug induced embryopathies. We present patients with Cumarin-embryopathy (2), fetal alcohol syndrome (1), Zellweger-syndrome (2) and chromosomal anomaly 16 (1) and discuss the typical roentgenographic features, distribution and differential diagnosis of epiphyseal stippling.

Developmental disorders of the proximal epiphysis of the hallux.

The development and developmental variation of the proximal epiphysis of the hallux have been studied in 1,500 children who presented to the Emergency Department with minor injuries. An abnormality was present in 3.5% of the children representing epiphyseal fragmentation, sclerosis, cone-shaped epiphysis, and trauma. Those subjects with developmental anomalies were reviewed clinically and 25% of those with epiphyseal fragmentation were found to be suffering from hallux rigidus. The radiological features and clinical considerations are presented.

[Diaphyseal and articular fractures of the growing skeleton of the hand].

The closed as well as the operative treatment of fractures of the skeleton of the hand during growth must take into consideration the growth potential of the bones. It is particularly important in its potential to correct shortening as well as axial deviation and angulation (except rotational deformity). There are, however, limits to this potential as shown in epiphyseal fractures. Modern internal fixation has, therefore, an important -- though very restricted -- place in the treatment of these epiphyseal fractures. Other rare indications are mentioned.

Aging reduces strain magnitude and heterogeneity while increasing apparent stiffness of the rat proximal tibia.

The epiphyses of long bones play a critical role in joint integrity by absorbing and redistributing joint forces. While aging induces microstructural and material changes in this anatomical site, their impact on the internal mechanical environment of the epiphysis remains poorly characterized. In this study, we quantified age-related alterations in epiphyseal strain distribution in the knee joints of adult and aged male Wistar rats using in situ mechanical testing combined with micro-computed tomography, digital volume correlation, and finite element analysis. Aging was associated with a prono…

Tall Stature and Scoliosis Associated With a Novel Homozygous Loss-of-Function Missense Variant in NPR3.

NPR3-related tall stature is characterized by tall stature, elongated big toes, and additional epiphyses in hand and foot bones. The condition is caused by biallelic loss-of-function variants affecting natriuretic peptide receptor 3 (NPR3). Five individuals from four different families have been reported. Here we describe three siblings with NPR3-related tall stature who were tall (height z-scores between +2.9 and + 4.9) and had markedly elongated proximal and middle phalanges. Two siblings had additional epiphyses in phalangeal and metacarpal bones. All three siblings developed scoliosis, req…

Slipped capital femoral epiphysis in skeletally immature patients.

Fixation by a single screw is considered the current treatment of choice for a slipped capital femoral epiphysis. This approach promotes premature physeal closure. The use of a modified, standard, single, cannulated screw designed to maintain epiphyseal fixation without causing premature closure of the physis was reviewed in ten patients. The nine boys and one girl aged between 10.6 and 12.6 years with unilateral slipped capital femoral epiphysis (SCFE), were markedly skeletally immature (Tanner stage I, bone age 10 to 12.6 years). Clinical and radiological review at a mean follow-up of 44.3 m

99mTc-DPD uptake in juvenile hemarthrosis. Scintimetry and autoradiography of the knee in dogs.

The pathogenesis of subchondral bone lesions and growth plate affection in hemophilic arthropathy was studied in puppies by means of repeated regional 99mTc-diphosphonate scintimetry and contact autoradiography. Unilateral hemarthrosis of the knee was induced by biweekly intraarticular injections of autologous blood for 12 weeks. Hemarthrosis caused an early (2 to 4 weeks) decrease in uptake of 99mTc-diphosphonate in the juxtaarticular growth plates (ratio 0.7) and a delayed (8 to 10 weeks) increase in epiphyseal uptake (ratio 1.5). In a recovery phase after hemarthrosis, growth plate uptake r

Toe-to-thumb transfer: a new technique.

Our technique combines the advantages of two proven techniques of the wraparound flap and vascularized joint transfer while offering a more normal thumb, both functionally and cosmetically. Its advantages are as follows: 1. A more normal-looking thumb with good length 2. Preservation of motion through joint transfer 3. Maintenance of growth potential through transfer of vascularized epiphyses 4. Minimal donor-site morbidity

Spondylometepiphyseal dysplasia congenita, Strudwick type.

A case of spondylometepiphyseal dysplasia congenita, Strudwick type is presented. At birth, this condition cannot be distinguished from spondyloepiphyseal dysplasia congenita. Features in common include delayed ossification of the public bones and proximal femoral epiphyses, coxa vara, odontoid hypoplasia and lumbar lordosis. The distinguishing radiologic feature of this condition is the striking irregularity of long bone metaphyses which develops during infancy.

[A computer macro-programme for osteoscintimetry and activity profile in Perthe's disease (author's transl)].

A computer macro-programme has been developed which permits quantitative scintigraphy by means of an activity profile. The slope and width of the profile are variable; a view at right angles to the epiphyseal plate of the femoral head is able to determine activity in the plate and in the epiphysis. Measurements of the abnormal and normal hip produce uptake rates. These indicate to what extent the abnormal femoral head and epiphyseal plate differ from the normal side, with respect to their uptake. The method and results are discussed.

The foot and ankle in multiple hereditary exostoses.

Twenty-two cases of multiple hereditary exostoses revealed pathological changes in two areas: the first group caused by epiphyseal disturbances, and the second group due to mechanical problems created by the exostoses. The epiphyseal disturbances resulted in ankle valgus, shortened metatarsals, and angular deformities of the necks of the metatarsals. The exostoses produced local tenderness, synostosis of the medical subtalar facet resulting is loss of subtalar motion, and asymptomatic synostosis of the lower tibial fibular syndesmosis.

Hip Vascularity: A Review of the Anatomy and Clinical Implications.

Throughout development, the vascular supply to the proximal femur and acetabulum undergoes a series of changes during which it is susceptible to injury. Before age 3 months, the ligamentum teres and lateral epiphyseal arteries are the dominant supply to the developing head. The dominant supply shifts to the lateral epiphyseal vessels by age 18 months. The distinct metaphyseal and epiphyseal circulations of the adult proximal femur form in adolescence when an increasingly rich metaphyseal circulation supplies the subphyseal region, terminating at the physeal plate. The acetabular blood supply d

Orthopaedic manifestations of Bardet-Biedl syndrome.

Bardet-Biedl syndrome is an autosomal recessive disorder poorly characterized in the orthopaedic literature. Classic manifestations of the syndrome include pigmentary retinopathy, obesity, polydactyly, hypogonadism, and mild mental retardation. Previous reports have implied that orthopaedic findings are due to an epiphyseal dysgenesis inherent to the syndrome. The purpose of this study was to evaluate the orthopaedic manifestations of 27 patients with this syndrome. Detailed medical histories and physical examinations as well as pedigree analyses and radiographic bone surveys were performed. O
