Different types of osteochondrodysplasia in a consecutive series of newborns.

Among 14816 consecutive live births there were 7 cases of osteochondrodysplasia (incidence 1:2117). In addition there was 1 case of stippled epiphyses, possibly induced by anticonvulsive drugs taken by the mother during the pregnancy, and one case of cerebro-hepato-renal syndrome of Zellweger with roentgenological features similar to those of hypochondrodysplasia. None of 102 stillborns seen at the same time had osteochondrodysplasia. For genetic counselling an exact diagnosis is mandatory, radiological examination of the skeleton is often of decisive importance.

Intertrochanteric Imhäuser osteotomy combined with osteochondroplasty in treatment of moderate-severe stable slipped capital femoral epiphysis: a case series study.

The aim of the study is to assess the safety and efficacy of Imhäuser osteotomy combined with osteochondroplasty in the treatment of moderate-severe stable slipped capital femoral epiphysis (SCFE) on short-term basis. Nineteen patients (20 hips) with moderate-severe stable SCFE were surgically treated by Imhäuser osteotomy combined with osteochondroplasty and followed up for 3-4 years. The cases aged between 12 and 18 years at the time of surgery and complained of a variety of symptoms and signs that included pain, limping, limited range of motion (ROM), and/or abductor weakness. The outcomes

Outpatient Management of Slipped Capital Femoral Epiphysis Is Common, Safe, and Saves Costs: A Single-Institution Analysis and Results From a National Database Survey.

INTRODUCTION: Slipped capital femoral epiphysis (SCFE) is a condition requiring surgical fixation on a semiurgent basis. Although recent studies have evaluated the safety of outpatient SCFE management, none have examined cost in this population. Furthermore, it is unclear what percent of in situ SCFE fixation is done on an outpatient basis nationally. METHODS: A retrospective chart review of patients treated between the years 2015 to 2024 was conducted at a large tertiary-care children's health system. Patients undergoing in situ fixation for stable SCFE and a minimum 90 days follow-up were i

Medium-long-term follow-up of slipped capital femoral epiphysis treated with in situ fixation and patient-acceptable symptom state evaluation.

This study evaluates the Patient Acceptable Symptom State (PASS) in patients with slipped capital femoral epiphysis (SCFE) treated with in situ fixation, focusing on medium to long-term outcomes and quality of life. Its primary goal is to establish a subjective well-being cutoff, using subjective methods and the iHOT33 scale, for assessing patients in future studies. Additionally, it explores functionality differences between mild and moderate-severe SCFE, case series epidemiology and potential complications. A retrospective analysis of 63 patients (73 hips), treated for SCFE between 2000 and

Rates of Contralateral SCFE in the United States: Analysis of the Pediatric Health Information System.

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is relatively common in pediatric patients. Subsequent contralateral SCFE is also common and rates of bilateral SCFE have been reported to range from 12% to 80% in the literature. The purpose of this study was to determine the rates of subsequent contralateral SCFE in patients who present with unilateral SCFE in the United States and determine risk factors for the development of subsequent contralateral SCFE. METHODS: The authors performed a retrospective study using data from the Pediatric Health Information System (PHIS). Patients who und

Musculoskeletal complications following total body irradiation in hematopoietic stem cell transplant patients.

Total body irradiation (TBI) is commonly used in conditioning regimens for allogeneic hematopoietic stem cell transplantation (HSCT) to treat benign and malignant disease. Though life-saving, these therapies place patients at risk for important side effects, including musculoskeletal complications such as short stature, osteonecrosis, slipped capital femoral epiphysis, and the development of benign and malignant bone tumors. With an increasing number of HSCT survivors, there is a growing need for awareness of the musculoskeletal complications of HSCT and TBI.

Chronic slipped capital femoral epiphysis: a radiographic evaluation of the Southwick osteotomy.

The aim of this study was to evaluate the radiographic correction and the complications in Southwick osteotomy for slipped capital femoral epiphysis (SCFE) on the basis of radiographic evaluation. We retrospectively analyzed the charts of 37 patients with chronic, unilateral, moderate, or severe SCFE, who underwent the procedure, verifying Southwick angles, articulotrochanteric distance, and the articular narrowing on preoperative, postoperative periods, and in the last follow-up evaluation, using the contralateral, healthy hip, for comparison. Southwick's angle changed from 117.8 to 147.3° on

The asymptomatic contralateral hip in unilateral slipped capital femoral epiphysis: morbidity of prophylactic fixation.

Prophylactic contralateral fixation in unilateral slipped capital femoral epiphysis (SCFE) remains a controversial issue. In this study, 66 patients treated for unilateral SCFE (July 1997-April 2009) were screened for complications with need for surgical reintervention in the asymptomatic contralateral hip. Except for one patient, prophylactic fixation was performed in all cases. Major complications such as avascular necrosis were not seen; minor complications such as wound revision occurred in 4.6% (three of 65). In 16.9% (11/65) secondary loss of fixation with need for second fixation occurr

Slipped capital femoral epiphysis: reduction as a risk factor for avascular necrosis.

Complication rates after the treatment of slipped capital femoral epiphysis (SCFE) were retrospectively evaluated to identify possible risk factors. Twenty-six patients with SCFE underwent reduction and fixation with a single screw or multiple wires. The prevalence of avascular necrosis (AVN) of the hip or chondrolysis was 14.8 and 3.6%, respectively. The degree of SCFE (P=0.050) and reduction before the seventh day after the slip (P=0.028) were associated with AVN. The risk factors for AVN were found to be a higher grade of SCFE correction and reduction between the second and the seventh day

Current concepts in the diagnosis and management of femoroacetabular impingement.

Femoroacetabular impingement is becoming increasingly recognised as a cause of hip pain in the young adult. It is thought that the condition may lead to acetabular labral tears, chondro-labral separation, chondral delamination and eventually predispose to osteoarthritis of the hip. Efforts have thus been directed to diagnosing and treating the underlying pathology and subsequent sequelae. This article presents the relevant literature with regards to the aetiology of femoroacetabular impingement, its clinical features, investigations and options of management. Finally outcomes relating to both

Does skeletal maturity predict sequential contralateral involvement after fixation of slipped capital femoral epiphysis?

BACKGROUND: The management of the uninvolved hip after fixation of unilateral slipped capital femoral epiphysis (SCFE) remains controversial. The purpose of this study was to determine the relationship of sequential contralateral slip to the initial patient presentation, specifically skeletal maturity. METHODS: The medical records and radiographs for all patients who underwent fixation of unilateral SCFE between June 1996 and October 2003 were reviewed retrospectively. Data were gathered on age, sex, stability, chronicity, body mass index, modified Oxford bone age (mOBA), physeal slope angle,

Subcapital femoral neck osteotomy without surgical hip dislocation in epiphyseolysis capitis femoris lenta with severe displacement.

THE PROBLEM: Severe displacement in the lenta form of slipped capital femoral epiphysis leads to distinct joint incongruence with subsequent severe deformity, limitations of function and walking ability as well as early joint degeneration. THE SOLUTION: Subcapital femoral neck osteotomy restores the congruence of the hip joint as far as possible. This procedure is a so-called secondary prophylactic intervention. SURGICAL TECHNIQUE: Disimpaction of the slipped epiphysis in several planes by wedge-shaped osteotomy in the subcapital region of the femoral neck (slippage is in a posterior and cau

The management of avascular necrosis after slipped capital femoral epiphysis.

Avascular necrosis is a serious complication of slipped capital femoral epiphysis and is difficult to treat. The reported incidence varies from 3% to 47% of patients. The aims of treatment are to maintain the range of movement of the hip and to prevent collapse of the femoral head. At present there are no clear guidelines for the management of this condition and treatment can be difficult and unrewarding. We have used examination under anaesthesia and dynamic arthrography to investigate avascular necrosis and to determine the appropriate method of treatment. We present 20 consecutive cases of

Outcome of slipped capital femoral epiphysis in renal osteodystrophy.

The cases of renal osteodystrophy-associated slipped capital femoral epiphysis in 11 consecutive patients were reviewed. Nine patients had bilateral involvement, totaling 20 hips. The mean age at presentation was 10.6 years. Slip location was physeal in 13 and metaphyseal in seven hips. All patients had prompt medical treatment of their bone disease, and nine patients underwent surgical stabilization. Fixation consisted of multiple custom-machined Steinmann pins that were smoothed distally but threaded proximally, allowing continued proximal femoral growth. The mean radiographic and clinical f

[Early damage to the acetabular cartilage in slipped capital femoral epiphysis. Therapeutic consequences].

Epiphyseolysis capitis femoris represents the most common disorder of the adolescent hip, which is followed by a significant rate of early osteoarthrosis. Based on intraoperative findings during the surgical management of 23 hips with epiphyseolysis, early acetabular cartilage abrasion by a cam effect and acetabular rim impingement elicited by the prominent femoral metaphysis have been identified. Both phenomena cause direct damage to the hip joint, especially during flexion and flexion/internal rotation of the hip. As evidenced during surgery, the prominent and sometimes sharp-edged anterior
