Conservative Approach to Treating American Football Players With Medial Collateral Ligament Grade 2 Sprain During the Season.

CONTEXT: The medial collateral ligament is the most commonly injured ligament in the knee. The high-speed pivoting and agility movements that are common in the sport of American Football put participants at an increased risk for a valgus force stress from contact or noncontact injuries. Positional release therapy (PRT) also considered strain/counterstrain focuses on releasing the tension in a tissue through unloading the involved body part. CASE PRESENTATION: Two male student-athletes participating in football with a mean age of 20.5 years were diagnosed by a physician with medial collateral

Matrix stiffening by self-mineralizable guided bone regeneration.

Collagen membranes produced in vitro with different degrees of intrafibrillar mineralization are potentially useful for guided bone regeneration (GBR). However, highly-mineralized collagen membranes are brittle and difficult for clinical manipulation. The present study aimed at developing an intrafibrillar self-mineralization strategy for GBR membrane by covalently conjugating high-molecular weight polyacrylic acid (HPAA) on Bio-Gide® membranes (BG). The properties of the self-mineralizable membranes (HBG) and their potential to induce bone regeneration were investigated. The HBG underwent the

Efficacy and safety of alvimopan use in benign urinary tract reconstruction.

PURPOSE: To analyze the use of alvimopan, a peripheral mu-opioid receptor antagonist, in expediting gastrointestinal recovery after benign abdominal urinary tract reconstruction. Alvimopan use has been well defined in the management of radical cystectomy and urinary diversion for oncologic indications. It has not been studied in benign abdominal genitourinary reconstruction. METHODS: Patients who underwent urinary reconstruction utilizing harvested bowel segments for benign conditions from 12/2014-7/2019 were retrospectively reviewed. From 5/2018-7/2019 our institution approved the use of per

Generation and function of progenitor T cells from StemRegenin-1-expanded CD34+ human hematopoietic progenitor cells.

Broader clinical application of umbilical cord blood (UCB), as a source of hematopoietic stem/progenitor cells (HSPCs), is limited by low CD34+ and T-cell numbers, contributing to slow lymphohematopoietic recovery, infection, and relapse. Studies have evaluated the safety, feasibility, and expedited neutrophil recovery associated with the transplantation of CD34+ HSPCs from ex vivo expansion cultures using the aryl hydrocarbon receptor antagonist StemRegenin-1 (SR1). In a phase 1/2 study of 17 patients who received combined SR1-expanded and unexpanded UCB units, a considerable advantage for en

Lung Transplant Patients on Kilimanjaro.

BACKGROUND: After its initial difficulties were overcome, lung transplantation became an accepted and effective treatment for end-stage lung disease. Patients can take part in almost all kinds of sports after lung transplantation, including high-altitude mountaineering, which is an extreme sport even for healthy individuals. Several articles have been published about high-altitude tolerance of transplanted patients. However, this was the first high-altitude expedition that included only lung transplant patients. METHODS: The Vienna lung transplantation team organized an expedition in 2017 to

Electronic Consultations in Allergy/Immunology.

BACKGROUND: Allergic condition management more often requires allergist guidance than allergy testing; necessary testing may be unavailable at initial drug allergy consultations. Electronic consultations (e-consults) provide expedited, problem-focused, potentially cost-saving care in other medical specialties, but have not yet been studied in Allergy/Immunology. OBJECTIVE: To describe e-consult use at an academic allergy/immunology practice. METHODS: E-consult data (August 10, 2016 through July 31, 2018) and in-person consult data (August 1, 2014 through July 31, 2018) were reviewed to deter

Thirty-Month Complete Response as a Surrogate End Point in First-Line Follicular Lymphoma Therapy: An Individual Patient-Level Analysis of Multiple Randomized Trials.

Purpose Follicular lymphoma (FL) is an indolent cancer, with effective but rarely curative treatment options. As a standard study end point for first-line FL therapy, progression-free survival (PFS) requires extended follow-up (median PFS, > 7 years). To provide patients with earlier access to newer therapies, an earlier end point to expedite clinical trials is needed. Our objective was to formally assess the complete response rate at 30 months (CR30) after initiation of induction therapy as a potential surrogate end point for PFS in first-line FL therapy. Patients and Methods We analyzed indi

Unusual electrocardiographic signs in the emergency department: A narrative review.

The electrocardiogram (ECG) remains an indispensable tool across clinical settings, routinely interpreted by physicians to evaluate acute symptoms, manage chronic conditions, and perform cardiovascular screening. Although common ECG patterns are readily recognized, less common or atypical findings can be challenging to identify despite their clinical significance, even for experienced clinicians. Familiarity with these distinctive patterns can improve and expedite clinical decision-making, particularly in time-sensitive diagnostic dilemmas, by facilitating the evaluation of both cardiac and no

Haemophagocytic lymphohistiocytosis complicating typhoid fever: A case report from Senegal.

We report a case of secondary haemophagocytic lymphohistiocytosis (HLH) triggered by confirmed typhoid fever (Salmonella Typhi) in a 23-year-old Senegalese man. The initial radiological presentation suggested ileo-caecal tuberculosis. Thus, persistent fever, marked hyperferritinaemia and systemic inflammation raised suspicion of HLH, confirmed by bone marrow examination demonstrating hemophagocytosis. The H-Score was 206, corresponding to 88-93% probability. Blood cultures isolated ciprofloxacin-susceptible Salmonella Typhi. Fluoroquinolone monotherapy led to complete clinical and biological r

Branched endovascular aneurysm repair complicated by disseminated intravascular coagulation while using tocilizumab.

Disseminated intravascular coagulation (DIC) has been reported as a complication following endovascular procedures. We present the case of a 69-year-old female with giant cell arteritis on tocilizumab therapy, who underwent a first-stage branched endovascular aneurysm repair for a Crawford type III thoracoabdominal aneurysm. Postoperatively, she developed DIC. After excluding other causes, we assumed endoleakage through the celiac branch combined with tocilizumab-induced hypofibrinogenemia as the primary cause. The second stage was expedited and tocilizumab discontinued. The DIC resolved withi

Enhanced recovery after surgery for inflammatory bowel disease surgery: A systematic review.

Enhanced Recovery after Surgery (ERAS) protocols, though well-established across surgical disciplines, remains underutilized in inflammatory bowel disease (IBD) surgeries due to complexities like malnutrition and immunosuppression. This systematic review evaluated 13 studies involving 3,539 patients to assess the impact of ERAS implementation in IBD surgery. Data consistently showed that ERAS protocols reduced hospital stay, expedited bowel function return and decreased complications and readmissions compared to conventional care. Key components included nutritional optimization, laparoscopic

Standardized ERAS protocol in liver and pancreatic surgery.

ERAS (enhanced recovery after surgery) represents a comprehensive strategy aimed at expediting patient recovery, reducing complications, and optimizing postoperative care. The ERAS protocol encompasses recommendations for the preoperative, perioperative, and postoperative phases of patient care. Implementation of the ERAS protocol yields a multitude of benefits for both patients and the healthcare system. It shortens hospital stays, diminishes the number and severity of postoperative complications, and enhances patient's quality of life. These factors contribute to healthcare cost sav-ings and

Isolated left ventricular cardiac tamponade diagnosed on point-of-care ultrasound in the emergency department: A case report and brief literature review.

In this case report, we describe isolated left ventricular cardiac tamponade, a rare complication of cardiac surgery, diagnosed on point-of-care ultrasound (POCUS) in the emergency department (ED). To our knowledge, this is the first report of such a diagnosis made on ED bedside ultrasound. Our patient was a young adult female with a history of recent mitral valve replacement who presented to the ED with dyspnea and was found to have a large loculated pericardial effusion causing left ventricular diastolic collapse. Rapid diagnosis via POCUS in the ED allowed for expedited definitive treatment

Minimally Invasive Foot and Ankle Surgery: A Primer for Orthopaedic Surgeons.

There is rapidly growing interest in minimally invasive surgery (MIS) of the foot and ankle. Technological advances, specifically with the advent of low-speed high-torque burrs, have enabled the expansion of MIS techniques. Accordingly, there is growing literature reporting excellent outcomes of MIS surgery to address many different pathologies of the foot and ankle. MIS techniques are particularly useful for conducting percutaneous osteotomies and bony débridement. These can be used to address bunion deformity, hammertoes, metatarsalgia, bunionette deformity, bone spurs, and hindfoot deformit

Radiological abnormalities in progressive multifocal leukoencephalopathy: Identifying typical and atypical imaging patterns for early diagnosis and differential considerations.

Progressive multifocal leukoencephalopathy (PML) is a rare viral central nervous system (CNS) demyelinating disease primarily associated with a compromised immune system. PML is seen mainly in individuals with human immunodeficiency virus, lymphoproliferative disease, and multiple sclerosis. Patients on immunomodulators, chemotherapy, and solid organ or bone marrow transplants are predisposed to PML. Recognition of various PML-associated typical and atypical imaging abnormalities is critical for early diagnosis and differentiating it from other conditions, especially in high-risk populations.

Tissue Engineering for Tracheal Replacement: Strategies and Challenges.

The critical feature in trachea replacement is to provide a hollow cylindrical framework that is laterally stable and longitudinally flexible, facilitating cartilage and epithelial tissue formation. Despite advanced techniques and sources of materials used, most inherent challenges are related to the complexity of its anatomy. Limited blood supply leads to insufficient regenerative capacity for cartilage and epithelium. Natural and synthetic scaffolds, different types of cells, and growth factors are part of tissue engineering approaches with varying outcomes. Pre-vascularization remains one o

Diagnosis of Posterior Urethral Valves in an Infant Using Point-of-Care Ultrasound.

This case describes a 7-week-old male infant presenting with vomiting and decreased urine output. Initial point-of-care ultrasound (POCUS) demonstrated a normal pylorus; however, assessment of bladder volume revealed the problem. The bladder was distended with a thickened, trabeculated wall and there was bilateral hydroureter, consistent with bladder outlet obstruction. Renal POCUS revealed bilateral hydronephrosis and perinephric fluid collections consistent with calyceal rupture. A voiding cystourethrogram confirmed the diagnosis of posterior urethral valves which were eventually ablated by

General practitioners' attitudes towards acne management: Psychological morbidity and the need for collaboration.

METHOD: A qualitative descriptive study collected data via telephone interviews with 20 purposively sampled GPs working in New South Wales. A thematic analysis guided by the study objectives was undertaken. RESULTS: The participating GPs had divergent management approaches to acne treatment, infrequently provided acne patients with written resources, and would value additional dermatological support. Furthermore, the GPs recognised psychosocial ramifications and patient distress as drivers for treatment and expedited referral to dermatologists. DISCUSSION: Avenues for improved patient outcom
