We hypothesized that patients with hemophilia or von Willebrand disease might be protected from atherosclerosis because of their coagulation defect. We studied 40 subjects affected by these two coagulation diseases using echocolor Doppler of the abdominal aorta and leg arteries, and compared the results with those obtained in 40 control patients who were homogenous with study patients in terms of sex, age, and risk factors for atherosclerosis. The probands presented a lower number of plaques than the 40 control subjects in the aorta and in the leg arteries. The most serious hemophilic patients

Hemophilia A and B are X-linked hemorrhagic disorders, characterized by low/dysfunctional factor (F)VIII or FIX, respectively. The history of hemophilia treatment has evolved remarkably over time from the introduction of cryoprecipitate in the 1960s to plasma-derived factors in the 1970s. The 1980s brought a tragic chapter, as many plasma-derived products were contaminated with blood-borne viruses. In response to ongoing safety concerns, recombinant factors were developed in the 1990s. However, coagulation factors had a relatively short half-life upon infusion. As technology progressed, extend

BACKGROUND & AIMS: Patients with non-cirrhotic non-tumoral portal venous system thrombosis (NCNT-PVT) require long-term anticoagulation to prevent rethrombosis (either splanchnic or at any site, referred to as overall rethrombosis - oRT) when there is an underlying high-risk thrombophilic disorder or history of previous thrombotic episodes. However, up to 25% of patients without such indications still develop oRT. Elevated factor VIII (≥150%), D-dimer (≥500 ng/ml) or the presence of high molecular risk (HMR) variants have been proposed as risk predictors for oRT in these patients. This study a…

INTRODUCTION: This case report describes a newborn male presenting with mucosal bleeding and low Factor VIII activity, ultimately diagnosed with a rare compound heterozygous form of von Willebrand Disease (VWD) involving both type 2A and type 2N variants through previously unknown pathogenic mutations.
METHODS: A single patient case report.
RESULTS: Genetic testing revealed two heterozygous variants in the VWF gene c.2422_2424del [p.Cys808del] and c.2999A > G [p.Asp1000Gly]), previously described as variants of unknown significance, inherited in trans from each parent, resulting in a severe …

BACKGROUND: Hemophilia A patients are treated with factor (F) VIII prophylactically to prevent bleeding. In general, dosage and frequency are based on pharmacokinetic measurements. Ideally, an alternative dose adjustment can be based on the hemostatic potential, measured with a thrombin generation assay (TGA), like the Nijmegen hemostasis assay.
OBJECTIVE: The objective of this study was to investigate the predicted performance of a previously developed pharmacokinetic-pharmacodynamic model for FVIII replacement therapy, relating FVIII dose and FVIII activity levels with thrombin and plasmin

INTRODUCTION: Current treatment of severe haemophilia A includes prophylaxis with factor VIII (FVIII) replacement. The supply of plasma-derived FVIII is short in China.
PURPOSE: To evaluate the efficacy and safety of a new B-domain deleted (BDD) recombinant FVIII (TQG202) produced by human-derived cells for prophylaxis in severe haemophilia A patients and compare the bioequivalence with Xyntha.
METHODS: This multicentre, clinical trial consisted of an open-label, randomized, two-period cross-over trial assessing single-dose pharmacokinetics (PK), and a single-arm clinical trial evaluating th…

Acquired factor VIII inhibitor (AFI) is a rare disorder and is even more uncommon in multiple myeloma patients, with only five cases reported in literature. Solid and hematologic malignancies, autoimmune conditions, drugs, and infections are the conditions commonly associated with the development of this condition, with mucocutaneous bleeding being the most common presenting sign. Diagnosis is usually made with the laboratory finding of an elevated partial thromboplastin time aPTT that cannot be corrected by plasma mixing, and further confirmed by low factor VIII activity/antigen levels along …

The objectives of the present study were to evaluate the analytical sensitivity of the recently developed Clot Formation and Lysis (CloFAL) global assay for factor VIII (FVIII) deficiency, both in vitro and ex vivo, to determine whether this global assay is influenced by FVIII inhibitors, and to investigate the coagulative response to FVIII replacement in haemophilia A patients using the CloFAL assay in comparison with FVIII activity. Among adults and children alike, the CloFAL assay coagulation index (CI) was significantly decreased in FVIII-deficient vs. healthy subjects (adults median CI: 2…

Patients with severe hemophilia A frequently develop neutralizing anti-factor VIII antibodies after replacement therapy with factor VIII (FVIII). In a search for new strategies to induce immune tolerance against FVIII in these patients, we used a murine model of hemophilia A to investigate the importance of CD40/CD40 ligand (CD40L) interactions for the initiation of the anti-FVIII immune response. We focused our attention in particular on the induction of neutralizing anti-FVIII antibodies and the Th1/Th2 polarization of FVIII-specific T cells. The development of anti-FVIII antibodies was anal…

Inhibitor formation in patients with haemophilia receiving factor VIII (FVIII) concentrate is a common problem requiring tolerance induction therapy. Immune tolerance is dependent on defective T cell/antigen-presenting cell (APC) interactions and inhibitor antibody formation is associated with effective T-cell/B-cell interaction. We studied the expression of the cell-surface molecules involved with these interactions using multiparameter flow cytometry and a whole blood stimulation assay-phytohaemaglutinin (PHA) to activate T cells and Escherichia coli lipopolysaccharide (LPS) to activate mono…

The intracellular localization of a factor VIII-related antigen in human umbilical veins derived both from a patient with 'severe Type 1' von Willebrand disease and from a normal full-term baby was investigated by immunoelectron microscopy. In normal vascular endothelial cells, the factor VIII-related antigen was detected in Weibel-Palade bodies. The endothelial cells of von Willebrand disease did not include these structures, but were filled with distended vesicles, in which the factor VIII-related antigen was found immunohistochemically. These results suggest that Type 1 von Willebrand disea…

Ten per cent of patients with haemophilia A develop intracranial haemorrhage (ICH) with a mortality rate of 30% and an incidence of psycho-neurological sequelae in 50% of survivors. ICH may be spontaneous or in association with trauma and other pathology. The generally recommended management is conservative replacement therapy using bolus injections of factor VIII and no neurosurgical intervention. Adjusted dose continuous infusion therapy provides an alternative method of factor VIII administration that is simple, more cost effective and safer through the maintenance of stable plasma VIII:C l…

This study aimed at evaluation of the immunological status and the clinical course of both HIV seronegative and seropositive hemophiliacs treated with either an ultra-pure factor VIII product (UP-F VIII), or a high-purity F VIII (HP-F VIII) concentrate. Eighteen HIV seronegative patients were divided into two groups of therapy and their immune status was followed for 2 years. During the second year of the study 8 patients of the HP-F VIII and 6 from the UP-F VIII therapy groups were switched to the alternative F VIII concentrates. Eighteen asymptomatic HIV seropositive patients were also divid…

Of 14 asymptomatic hemophilia A patients receiving factor VIII concentrates, 11 severe hemophilia patients had an inverted T-helper/suppressor ratio but 3 moderate patients had a normal ratio. Most hemophilia patients showed poor lymphocyte proliferative responses to mitogens and diminished in vitro immunoglobulin-producing ability of lymphocytes. One important finding was that most patients were found to have increased numbers of Ia-like antigen-positive suppressor T cells, suggesting that circulation activated suppressor T cells. In addition, OKT8+ suppressor T cells from severe hemophilia p…
