INTRODUCTION: Antegrade continence enema (ACE) is recommended for patients with fecal incontinence/constipation refractory to conservative management. The Malone ACE utilizes the appendix as a catheterizable channel whereas the Macedo-Malone (MM) precludes the use of the appendix. We aimed to illustrate the MM procedure.
METHODS: The left colon is brought out through a small transverse incision on the upper left abdomen and a 3-cm transverse flap in a tenia is created. A 12-Fr silicone Foley catheter is placed on the mucosal surface of the flap to allow creating a tubular conduit. After closu…

Claire Bohr describes how she introduced rectal irrigation as a treatment for children with faecal incontinence who had failed to respond to conservative treatment. She won a Nursing Times Award in the continence care category for this service in 2008.

An implantable device for establishing urinary continence has been developed and has received FDA approval (AS 800, American Medical Systems, Inc.). The authors have applied this device to the control of anal continence in dogs. Fifteen mongrel dogs underwent either anal sphincter disruption (7) or abdominal-perineal resection (8). Such animals have bowel movements almost hourly. In each case, the device was implanted at the time of surgery. In dogs with working cuffs and disabled or absent sphincters, continence was maintained in seven out of 15 animals for periods of 4-8 hours. After cuff ac

Incontinence-associated dermatitis (IAD) is often treated a hygienic challenge, rather than a serious condition with potentially life-threatening consequences. More appropriate education on the management strategies specific to IAD is required, in order for personalised and effective care that reflects the critical nature of this condition to be provided. Francesca Ramadan provides an overview of the key elements of best practice in IAD management and treatment.

Pelvic floor dysfunction (PFD) is a common clinical problem that can lead to bladder and bowel dysfunction such as urinary incontinence, urinary retention, fecal incontinence, pelvic organ prolapse, and sexual dysfunction. Pelvic floor rehabilitation aids are essential tools in the treatment of PFD. However, there is limited understanding of the efficacy and mechanisms of these aids, and there is a lack of standardized guidelines for selecting appropriate aids for different types of PFD. To assist patients in choosing suitable pelvic floor rehabilitation aids to their needs, it is necessary to…

INTRODUCTION: Incontinence-associated dermatitis (IAD) is a type of irritant contact dermatitis due to prolonged exposure of the skin to moisture induced by urine or/and faeces. The main principles when treating IAD involves protecting the skin from further exposure to irritants, establishing a healing environment, and eradicating skin infections. This study aimed to evaluate the effectiveness of the hydrocolloid crusting method (HCM) versus the standard care method (SCM) in treating IAD.
METHOD: A randomised controlled trial was conducted in an acute tertiary hospital in Singapore between Au

The skin's ability to function optimally is compromised when skin integrity is altered. The goals for skin management during rehabilitation include maintaining skin integrity, avoiding skin injury, providing early intervention, and comprehensive education for long-term skin management. Assessment of the whole patient as well as the skin is essential. Common issues seen in rehabilitation such as aging skin, incontinence dermatitis, intertrigo/yeast, surgical wounds, and pressure injuries are addressed.

INTRODUCTION: Spina bifida (SB) is the most common permanently disabling congenital defect. In 2016, the Spina Bifida Association (SBA) began updating the Guidelines for the Care of People with Spina Bifida (Guidelines). These Guidelines identified over 250 research gaps for the care of individuals with SB. The community of people living with SB prioritized these research gaps to create a formal research agenda that the SBA could support.
OBJECTIVE: To present the process and the final research agenda created by individuals with SB, their care partners, clinicians, and researchers.
DESIGN: A

CONTEXT: Managing patient and parent expectations regarding urinary and fecal continence is important with congenital conditions that produce neurogenic bladder and bowel dysfunction. Physicians need to be aware of common treatment algorithms and expected outcomes to best counsel these families.
OBJECTIVE: To systematically evaluate evidence regarding the utilization and success of various modalities in achieving continence, as well as related outcomes, in children with neurogenic bladder and bowel dysfunction.
EVIDENCE ACQUISITION: We performed a systematic review of the literature in PubMe
