Osteogenic sarcoma is a heterogeneous family of tumors that has a variable biologic behavior. Low grade central osteogenic sarcoma is an uncommon form that is characterized by a long premorbid history and is compatible with prolonged survival after treatment. Twenty cases of low grade central osteosarcoma with long-term followup (16 [2.5-48] years) were studied retrospectively. The age distribution was broad (range, 15-83 years). All tumors arose in the lower limb. The primary symptom was pain; mean duration was 44 months (range, 1-180 months). A diagnosis of low grade central osteosarcoma was

Guarded optimism could be clearly stated for the treatment of osteogenic sarcoma. Improved methods of diagnosis and staging have been made possible by the computed tomography (CT) scan and other new modalities. The advent of pre- and postoperative chemotherapy has significantly enhanced survival rates that are now approaching 85%. Improved concepts of en bloc resection and better reconstructive techniques suggest that limb salvage procedures are not only possible but can provide excellent functional results in the context of muscle loss. Continued work must be directed toward improving the rec

The surgical treatment of malignant fractures of the extremities includes a wide range of methods today. The intramedullary Küntscher nailing of the femur and tibia by the AO (Society for Osteosynthesis) method, Hackethal's multiple nailing in the upper arm and fore arm, and the bone glue-plate compound osteosynthesis methods are described. The alloplastic joint prostheses for hip and knee are extended to proximal and distal femur prostheses. Total femur prosthesis with simultaneous hip and knee joint alloplasty seems to be full of prospect.

BACKGROUND: Primary malignant tumors of the proximal femur in children are rare, and there is no consensus about the optimal reconstruction method for this condition. While methods for biological reconstruction-such as vascularized fibular allograft and rotationplasty-and nonbiological options like expandable endoprosthesis exist as reconstructive choices, each method has its distinct advantages and disadvantages. In this study, we compared the outcomes of osteoarticular allograft (OAA) with allograft-prosthetic component (APC) in reconstructing the proximal femur in a cohort of adolescent and

Predicting pathologic fractures in femora with metastatic lesions remains a clinical challenge. Currently used guidelines are inaccurate, especially to predict non-impeding fractures. This study evaluated the ability of a nonlinear quantitative computed tomography (QCT)-based homogenized voxel finite element (hvFE) model to predict patient-specific pathologic fractures. The hvFE model was generated highly automated from QCT images of human femora. The femora were previously loaded in a one-legged stance setup in order to assess stiffness, failure load, and fracture location. One femur of each

BACKGROUND: To address large tumor-related defects and lower limb-length discrepancies during limb-salvage surgery in children with malignant tumors in the distal femur, a new custom-made dual-mobility semi-knee prosthesis (DMK) was made. This study aimed to provide a theory and references for further clinical applications of this prosthesis.
METHODS: Based on computed tomography data from adult knee joint samples, we used Mimics/Geomagic/Pro-E software and computer numerical control milling technology to design and manufacture the DMK. An in vitro study was carried out to examine the related

AIMS: After intercalary resection of a bone tumour from the femur, reconstruction with a vascularized fibular graft (VFG) and massive allograft is considered a reliable method of treatment. However, little is known about the long-term outcome of this procedure. The aims of this study were to determine whether the morbidity of this procedure was comparable to that of other reconstructive techniques, if it was possible to achieve a satisfactory functional result, and whether biological reconstruction with a VFG and massive allograft could achieve a durable, long-lasting reconstruction.

A few cases of the pituitary apoplexy have been reported after spinal anesthesia in patients with pituitary tumor. A patient with giant pituitary adenoma underwent open reduction of the ankle fracture. This 69-year-old man had no symptoms related to pituitary adenoma. Femoral and sciatic nerve blocks were chosen to avoid pituitary apoplexy, cerebral herniation and other complications related to spinal anesthesia. The surgery was successfully done without any complications and the patient was discharged uneventfully 25 days after surgery. When patient has a giant pituitary tumor, peripheral ner

INTRODUCTION: classical osteosarcoma is defined as a very malignant spindle cell sarcoma, characterized by the production of osteoid matrix, it is the most common primary malignant bone tumor. It is most common among males in their twenties. Factors involved include those related with the skeletal growth and development, pathological fractures, and very young patients who are still growing. Conservative surgery is inappropriate; however, preoperative chemotherapy and a good choice of conservative treatment are an alternative.

External compression of the common femoral artery secondary to a lymphangioma has not been reported before. A female patient presented with intermittent claudication of the right lower extremity. She was examined with Doppler vascular ultrasound and computed tomography, which demonstrated a cystic mass that did not infiltrate the arterial wall. At operation we found a mass adjacent to and causing compression of the common femoral artery, which was completely resected. The histological report described a cystic cavernomatous lymphangioma. Surgical treatment of lymphangiomas includes complete re

The large and potentially sensate skin territory, the reliability, and the versatility of the anterolateral thigh flap have led to its use for the reconstruction of soft-tissue defects of the head and neck region, extremities, and trunk. Most authors describe the functional disturbances in the donor site as minimal and, to date, no cases of circulatory disturbance of the lower limb as a result of anterolateral thigh flap transplantation have been described. The authors report a patient with partial necrosis of the foot and calf caused by the interruption of the descending branch of the lateral

The purpose of this study was to evaluate the results and complications of hip arthroplasty done for salvage of failed treatment of pathologic proximal femoral fractures secondary to malignancy. Between 1980 and 2000, 42 patients with a mean age of 63 years were treated with hip arthroplasty to salvage failed treatment of a pathologic proximal femoral fracture. Total hip arthroplasty was done in 16 patients (3 uncemented, 2 hybrid, 11 cemented), and bipolar hemiarthroplasty in 26 (2 uncemented, 24 cemented). A modular, proximal femoral replacement construct was used in 15 patients. Patients we

We describe a case of a 75-year-old man with abdominal aortic and right femoral tuberculous pseudoaneurysms 32 months after intravesical bacillus Calmette-Guerin therapy for bladder cancer. These aneurysms were probably brought on by systemic infection by Mycobacterium bovis. The infrarenal aorta and right common femoral artery were successfully replaced with an in situ expanded polytetrafluoroethylene graft. Tuberculous pseudoaneurysm after bacillus Calmette-Guerin therapy for malignancy is very rare, and we review the related literature.

From 1996 to 2002, 39 consecutive intramedullary reconstructions (three bilateral) in 36 patients were performed in a group of subtrochanteric femoral metastatic bone disease using the long gamma nail (LGN). Reconstruction was performed prophylactically in 28 femurs and, for actual fractures in 11. All patients achieved good functional results with pain relief and improved mobility. No major intraoperative complication or long-term mechanical failures were observed. Minor technical, medical and implant related complications were seen in 14 patients. The LGN is a valuable intramedullary reconst

Treatment of actual or impending pathologic fractures of the femur provides the senior author with some of the most rewarding surgical interventions of his practice. The patients' survival outlook is not changed, but their quality of life is enhanced significantly. Most health care providers usually provide the metastatic cancer patient only temporary symptomatic relief, at best, and often at the expense of continued pain, suffering, or sickness, such as is seen with chemotherapy-associated morbidity. Patients with metastatic bone disease are usually incredibly grateful for the restoration of

We report a case of large medial meniscal cyst responsible for symptomatic saphenous nerve compression in a 49-year-old male with a history of mild trauma to the affected knee. We are not aware of any similar cases in the literature. The lesion was delineated by ultrasonography and even more clearly by magnetic resonance imaging. At surgery, the saphenous nerve was seen to be displaced by the cyst. Cystectomy and partial meniscectomy were performed. The outcome was favorable.

Over the years, patients with primary hyperparathyroidism are often asymptomatic and diagnosis is suspected on the incidental finding of hypercalcaemia. We report a patient, a middle-aged man from northern Sumatra, with a florid but uncommon presentation of severe bone pains and bilateral femoral neck fractures. He was found to have a solitary parathyroid adenoma which was successfully removed surgically. Postoperatively, the patient had severe symptomatic hypocalcaemia that required aggressive replacement therapy with calcium, magnesium and active vitamin D. He was well clinically and had sig

33 patients treated operatively for plasma cell disease were analysed. There were 21 men and 12 women with an average age of 54 years. There was an undefined bone tumour in 23 cases, and a pathologic fracture in 10 cases. In only 6 cases was the diagnosis known before the operation. The primary tumour localisations were: vertebral column in 13, pelvis in 7, femur in 6, humerus in 2, rib in 1, tibia in 1, fibula in 1, scapula in 1 and olecranon in 1 case. 16 diagnostic biopsies were taken. Vertebral tumours were mainly evacuated or decompressed, combined with a stabilising procedure in 8 cases.
