A case of colovesical fistula in a 51-year-old male patient is described. The connection, between the base of the bladder and the rectosigmoid junction, was caused by an appendicitis in the tip of a pelvic appendix which had perforated into the bladder and rectosigmoid. There was no appendicovesical fistula connecting with the caecum.

Over a five-year period (1982-86), 77 patients (59 males, 18 females; mean age 41.32 y) with fistula in ano treated at the University of Ife Teaching Hospitals Complex were reviewed. As many as 66.24 per cent of these patients presented rather late at the hospital after 2 yr from the onset of illness. Significant clinical presentation included rectal pain, perianal itching, lax-anal-sphincteric tone and purulent fistula discharge. A young man with a fistula between the right femoral artery and vein due to bullet wound presented 6 years later with exertional dyspnea and palpitations and venous insufficency of the right leg. Angiography showed a wide (greater than 20 mm) arteriovenous fistula. Moderate cardiac hypertrophy and cardiac output 14.8 l/min were found. During division and closure of the fistula, 8 l/min flow was measured in the right external iliac artery. The cardiac symptoms disappeared immediately and the venous insufficiency gradually.

From 1970-1985, 303 women with genitourinary fistulas were seen at the Mayo Clinic. The fistula formed after treatment for benign conditions in 74% of the patients and malignant conditions in 14%; in 12%, we were unable to establish the nature of the condition. Reverse vascularization of the cephalic vein using the medial cubital vein was performed in 16 patients. No complications were found in a follow-up period that ranged from 2 to 47 months. In selected anatomic cases, this reverse fistula is preferable to the direct brachiocephalic fistula.

Colouterine fistula complicating diverticulitis is rare. Our experience with two patients, one with chronic vaginal discharge and the other with acute overwhelming sepsis, emphasizes the wide spectrum of clinical presentations that may accompany this entity. In patients with chronic symptoms, surgery is indicated to forestall further local infectious complications, and a single-stage sigmoid resection without hysterectomy may be adequate. Of 80 cases of vesicovaginal fistula in children less than thirteen years of age in Northern Nigeria, 48 were due to labour, 12 to the traditional practice of gishiri cutting, and 20 to other causes including congenital abnormalities, coitus, infections, and trauma. Preventive measures include improved socioeconomic status of the community, education, and reevaluation of the role of females in society.

We report on a patient with a fistula between the right common iliac artery and the distal right ureter who had undergone pelvic exenteration for carcinoma of the uterine cervix. The patient also had received prior radiation therapy and was being treated with an indwelling ureteral stent at the time the fistula developed. Diagnosis was made by an occlusive ureterogram and the lesion was treated successfully with embolization of the common iliac artery.
