Bone grafting in femoral neck fractures: results in 28 cases operated on with multiple pinning and cancellous bone grafting.

Twenty-eight patients with displaced femoral neck fractures were operated on with multiple pinning combined with cancellous bone grafting from the ipsilateral greater trochanter. The bone graft was introduced through a drilled channel in the femoral neck without exposing the fracture. At follow-up after 2 years, seven patients had died. Two patients had been reoperated with prosthetic replacement of the hip. Nineteen fractures were united and one had developed segmental collapse. In femoral neck fractures, complications from non-union and segmental collapse are still a major problem. Even if t

Interactions Between HR-pQCT Bone Density and D3 Cr Muscle Mass (or HR-pQCT Bone Structure and HR-pQCT Muscle Density) in Predicting Fractures: The Osteoporotic Fractures in Men Study.

We examined if an interaction exists between bone and muscle in predicting fractures in older men. Prospective data from the Osteoporotic Fractures in Men study was used to build Cox proportional hazards models. Predictors included HR-pQCT total volumetric BMD (Tt.BMD), trabecular BMD (Tb.BMD), cortical BMD (Ct.BMD) and cortical area (Ct.Ar) at distal radius/tibia, HR-pQCT muscle volume and density (diaphyseal tibia), D3 -creatine dilution (D3 Cr) muscle mass, and grip strength and leg force, analyzed as continuous variables and as quartiles. Incident fractures were self-reported every 4 month

Masquelet technique for reconstructing bone defects in open lower limb fracture: Analysis of the relationship between bone defect and bone graft.

OBJECTIVE: The aim of this study was to retrospectively analyze the experience of treating long segmental bone defects. Bone union was used as the criteria for the assessment of the quantitative relationship between tibial bone defects and bone grafts, which could help reduce the complication incidence in patients. METHODS: A total of 45 patients with tibial bone defects treated with the Masquelet technique at the Ninth People's Hospital of Wuxi Affiliated to Soochow University between February 2013 and December 2017 were recruited. The age, sex, body mass index (BMI), complications, site of

Collagen-Related Viscoelasticity as a Metric for Fracture Assessment in Cortical Bone.

PURPOSE: The mechanical competence of bone relies on both elastic stiffness and viscous damping; however, existing assessments have primarily involved stiffness quantification using bone mineral density but lack a collagen matrix-focused metric to predict fracture risk. This study introduces an experimentally anchored, time-resolved analysis framework for quantifying cortical bone matrix viscoelasticity and evaluating fracture susceptibility arising from collagen organization defects. It further proposes a collagen-related viscoelasticity metric for bone assessment. METHODS: Using murine diab

Impact of quantization on various CNN architectures for bone fracture detection.

The skeletal system, comprising bones, plays a crucial role in physical support and locomotion. Any deformation of bones, due to trauma or medical conditions like osteoporosis, can lead to a bone fracture. Bone fractures are painful conditions requiring immediate medical attention. Automating fracture detection can play a significant role in early diagnosis and intervention for bone fractures. Convolutional neural networks (CNN) have shown promising results in multiple medical imaging applications. There is a wide variety of convolutional neural network architectures. Due to their high computa

Abnormal Bone Remodeling and Regeneration After Condylar Fracture Surgery.

After condylar fracture surgery, the temporomandibular joint undergoes varying degrees of self-repair. Usually, slow bone remodeling occurs. However, 2 abnormal patterns of condylar remodeling after condylar fracture surgery are found: one is in situ condylar bone regeneration, and the other is ectopic bone formation within the soft tissue of the joint capsule. Both of these abnormal patterns of bone formation require increased caution. Timely mouth-opening training can prevent the occurrence of joint ankylosis. Regular follow-up is necessary.

Osteocalcin as a predictor of bone fracture in children with chronic kidney diseases.

BACKGROUND: The associations between biochemical indicators of osteocalcin and bone health are fairly well established in adults through observational studies. In this context, our objective was to assess serum level of uncarboxylated osteocalcin (uOC) and its relation to the incidence of bone fractures other bone health indices in children with CKD. METHODS: We enrolled 102 patients classified into two groups: group A: CKD without KRT; group B: CKD on regular HD. The patients were followed throughout the study period. The study's endpoint was either the occurrence of a bone fracture or the c

Symptomatic Fracture of the Subhallucal Interphalangeal Sesamoid Bone: A Case Report.

The hallucal interphalangeal sesamoid bone can often be misdiagnosed when fractured and show presenting symptoms of forefoot pain. Primary presentation is asymptomatic but can become symptomatic in cases of trauma, overuse, or pressure to the area. However, even when symptomatic, misdiagnosis can often occur and lead to improper management that can exacerbate or prolong symptoms. This case is unique in revealing a rare anatomical variation and shows the proper approach to treatment and management from a prior misdiagnosis of right big toe pain without indication of a fracture.

[Fractures of the finger and metacarpal bones II-V : Diagnostics and treatment].

The function of the hand can be substantially impaired by fractures of the finger and metacarpal bones. Metacarpal fractures account for up to 50% of hand fractures, followed by finger fractures (25%), with the fifth metacarpal most commonly affected. The diagnostics include the clinical examination, X‑ray and often computed tomography (CT) imaging. The treatment depends on the type of fracture and clinical function as well as the patient's needs. In many cases conservative treatment is possible. Surgically, K‑wires, plate or screw osteosyntheses and rarely external fixators are used. Postoper

Arthroscopic bone grafting in scaphoid fracture nonunion: Is it a universal solution?

Since 1997, arthroscopic bone grafting has offered hand surgeons a solution to the riddle of scaphoid fracture nonunion. The intended merit of an arthroscopic approach for scaphoid fracture nonunion management lies in its minimal invasiveness, which preserves the tenuous vascularity of the scaphoid, poses minimal disturbance to carpal ligaments, and allows comprehensive evaluation in diagnostic uncertainty. What does the evidence tell us about its outcomes and limitations compared to open techniques? Level III evidence suggests that arthroscopic bone graft yields similar union rates and potent

Lee's TRIAD-osteoporosis, fragility fracture, and bone health optimization.

UNLABELLED: Osteoporosis, fragility fractures, and bone health optimization share the same pathophysiology, diagnostic tools, risk assessment, and treatments. Grouping them into "Lee's TRIAD" allows surgeons and physicians to collaborate more efficiently, using unified principles and strategies for managing these conditions. PURPOSE: The primary goal of osteoporosis management is to prevent fragility fractures, which occur from falls from standing height or less in individuals over fifty. However, the management of bone health optimization is often neglected in patients undergoing elective su

Bone Mineral Density Measurements and Fracture Prediction in the Very Elderly.

Bone density decreases with age, increasing the risk of fragility fractures. Dual-energy X-ray absorptiometry (DEXA) scanning may be used to measure bone density and estimate this risk. Degenerative changes in the spine may decrease the accuracy of DEXA results. We present the clinical course of a 92-year-old woman with a diagnosis of osteopenia of the left femoral neck and a risk for fragility fracture that was previously below the threshold for treatment, who underwent routine follow-up bone density screening. The DEXA scan showed an increase in bone density at the lumbar spine. Within a wee

Primary lymphoma of bone in the femur complicated by a fracture and listeria osteomyelitis.

We present a case of primary diffuse large B-cell lymphoma of bone accompanied by osteomyelitis, which was challenging to diagnose. CT revealed no distant metastases; thus, primary bone tumour and osteomyelitis were included in the differential diagnosis. A needle biopsy of an extraosseous lesion found no malignancy, and granulation tissue with abscess formation was pathologically diagnosed. An incisional biopsy was performed to assess osteomyelitis. A culture grew Listeria monocytogenes, but malignant lymphoma was diagnosed based on a pathological analysis of an open biopsy specimen. Imaging

Purtscher's Retinopathy Following Long Bone Fracture: A Case Report.

Purtscher's retinopathy represents an occlusive retinal microvasculopathy that poses a potential threat to vision and is linked to traumatic events. This condition typically manifests in individuals following trauma, commonly associated with long bone fractures, head injuries, or thoracic compression. We report a rare case of unilateral Purtscher's retinopathy after sustaining a long bone fracture. A 27-year-old healthy man sustained an open, comminuted midshaft fracture of the right femur after an alleged motor vehicle accident. On day 3 post trauma, he developed sudden right eye painless red

Approach to the elderly patient with vertebral fracture due to bone fragility.

Osteoporosis is a metabolic and systemic disease characterized by alterations at the level of bone tissue with loss of bone mineral density, changes in microarchitecture, mineralization and remodeling that determine greater bone fragility and risk of fracture. Falls in the elderly are a risk factor closely related to fragility fractures and numerous studies demonstrate this relationship. Vertebral fractures are a major cause of morbidity and mortality. The epidemiology differs from osteoporotic fractures at other skeletal sites, as only one-third are clinically recognized. In the elderly, the

Rehabilitation After Lower Limb Fracture Fixation in Osteoporotic Bone.

Malaria is a disease that generates a broad spectrum of clinical features. The purpose of this study was to evaluate the clinical spectrum of malaria in semi-immune populations. Patients were recruited in Mâncio Lima, a city situated in the Brazilian Amazon region. The study included 171 malaria cases, which were diagnosed via the use of a thick blood smear and confirmed by molecular methods. A questionnaire addressing 19 common symptoms was administered to all patients. Multiple correspondence analysis and hierarchical cluster analysis were performed to identify clusters of symptoms, and logi

Scaphoid and Carpal Bone Fracture: The Difficult Cases and Approach to Management.

Scaphoid and carpal bone fractures are challenging injuries to manage and have significant functional implications. Prompt diagnosis is important and relies on systematic evaluation via history, examination, and imaging. Cross-sectional imaging is often helpful for diagnosis and treatment planning. Treatment for nondisplaced fractures is often closed but these injuries typically require prolonged immobilization and may still result in nonunion or avascular necrosis. Displaced carpal bone fractures, and those associated with carpal instability, typically require open reduction internal fixation
