Oblique fractures of the metacarpal and phalanx are inherently unstable especially when there is comminution at the fracture site. Nonoperative management of these fractures requires prolonged immobilization and results in poor outcome. Internal fixation is the preferred method of treatment for these fractures. The various methods used for internal fixation are the Kirschner wires, intraosseous wiring, tension band wiring, minifragment screws and plates. Kirschner wires are the most commonly used fixation device because of their versatility and easy availability. The main disadvantage is lack

BACKGROUND: Intramedullary nailing is a mainstay in the treatment of subtrochanteric femoral fractures. But nailing is often unsuitable for difficult fracture patterns with comminution or when the medullary canal is narrow. The purpose of this study was to clarify the efficacy of minimally invasive plate osteosynthesis (MIPO) with locking compression plate in the treatment of subtrochanteric fractures.
METHODS: Twenty patients with subtrochanteric fractures (mean age 49.6 years) were prospectively treated using the MIPO technique using a locking plate. Indications of procedure were fractures

Cross-sectional spinal canal area was measured before and after surgery in 12 patients with thoracolumbar burst fractures and canal narrowing caused by retropulsed fragments. Patients were classified into Denis type A or type B. Denis type A fractures have comminution of both end-plates of the vertebral body creating multiple smaller fractures; Denis type B fractures have comminution of the superior end-plate only with a single vertical fracture line into the inferior end-plate creating larger fragments. The degree of neurologic impairment was assessed before and after surgery using the Franke

This study will evaluate the therapeutic effect of pin distractor assisted reduction of fibular fracture on ankle fracture. A retrospective analysis was performed on 98 patients diagnosed as ankle fracture or pilon fracture. These patients were all combined with fibular fracture. Retrospective analysis included 48 patients in the experimental group (distractor assisted reduction) and 48 patients in the control group (without pin distractor). The statistical indexes include: the time spent in fracture reduction and fixation, the length of surgical incision, the operative blood loss of fracture

BACKGROUND: Humeral diaphyseal fractures have been traditionally stabilized with plates and screws. However, surgical morbidity can be quite extensive, particularly in more complex segmental and comminuted fracture patterns. An intramedullary nail (IMN) has the biomechanical advantage of being a load-sharing device and can be placed with a more minimally invasive technique. The purpose of this study was to evaluate the clinical and radiographic outcomes of complex humeral shaft fractures treated with an IMN utilizing a percutaneous surgical technique.
METHODS: A retrospective review was perfo

Computerized tomography (CT) of the hindfoot is introduced as an effective technique in the evaluation of acute, intra-articular calcaneal fractures. Thirty-two fractures in 30 patients have been reviewed, and common fracture characteristics identified. These included the presence of a sustentacular fragment, displacement and comminution of the lateral weightbearing surface, and lateral spread with loss of calcaneal height. Based on the improved understanding of fracture anatomy provided by the CT scan, treatment often can be determined more accurately.

Conventional appliances used for treating fractures of the hip frequently are unsuccessful in the management of subtrochanteric fractures of the femur. Such appliances must only be employed when fracture stability can be obtained by meticulous open reduction. The appliance used must be strong, of sufficient length, and well fixed to the underlying bone. In fracture types where rigid internal fixation cannot be accomplished because of comminution or poor bone stock, intramedullary fixation utilizing the Ender principle is advocated.

Radial head fractures are common injuries comprising 1/3 of the fractures about the elbow. As a secondary valgus stabilizer of the elbow, radial head fractures can compromise elbow stability leading to long-term pain, dysfunction, and degenerative change. Furthermore, radial head fractures can be accompanied by associated injuries including ligamentous lesions that can further exacerbate elbow instability, leading to long-term sequelae. As fixation principles and technology have evolved, so has the ability to render surgical treatment for these fractures. In general, minimally displaced and so

Treatment of tibial plateau fractures is evolving, and two treatment methods have emerged. One is the use of small incisions and percutaneous stabilization of depression fractures; another is the use of thin wire external fixation for bicondylar fractures. Preoperative planning is aided by classifying fractures as depressed, split, or bicondylar. Depressed fractures can often be treated with insertion of subchondral screws and supporting bone graft under fluoroscopic or arthroscopic guidance. Split fractures are treated with an open approach and application of a buttress plate. Bicondylar frac

OBJECTIVE: Post-traumatic arthritis is a frequent cause of disability and occurs most commonly and predictably after articular fracture. The objective of this investigation was to examine the effect of fracture severity on acute joint pathology in a novel murine model of intra-articular fracture.
DESIGN: Low and high energy articular fractures (n=25 per group) of the tibial plateau were created in adult male C57BL/6 mice. The acute effect of articular fracture severity on synovial inflammation, bone morphology, liberated fracture area, cartilage pathology, chondrocyte viability, and systemic

BACKGROUND: The functional results associated with nonoperative treatment of severely impacted valgus fractures of the proximal part of the humerus are poor, and these injuries are difficult to treat with minimally invasive percutaneous fixation techniques. The aim of this study was to review the functional and radiographic results and complications of a new operative technique in a series of twenty-five patients.
METHODS: Over a two-year period, we treated twenty-nine patients with a severely impacted valgus fracture of the proximal part of the humerus. Three patients were lost to follow-up

Carpometacarpal (CMC) joint dislocations without fractures are rare and often missed due to subtle clinical and radiographic signs. We report a case of a 28-year-old male patient involved in a motorbike rollover who sustained open comminuted radius and ulna fractures, an ulnar styloid fracture, and dorsal dislocation of multiple CMC joints without metacarpal fractures. He presented with absent distal pulses and finger numbness, which resolved after emergency reduction. CT angiography ruled out arterial injury. He underwent internal fixation of forearm fractures and Kirschner wire stabilization

The objective of our literature review was to inform orthopaedic nurses about the complications of clavicular fracture, which are easily misdiagnosed. For this purpose, we searched MEDLINE (1965-2005) using the key words clavicle, fracture, and complications. Fractures of the clavicle are usually thought to be easily managed by symptomatic treatment in a broad arm sling. However, it is well recognized that not all clavicular fractures have a good outcome. Displaced or comminuted clavicle fractures are associated with complications such as subclavian vessels injury, hemopneumothorax, brachial p

Guidelines for the choice of conservative or operative treatment of fractures of the proximal radius are derived from a study comprising 21 children. Postoperative complications and long-term failures are specifically stressed. True intraarticular fractures of the radial head are rarely observed in children. Major problems arise in the treatment of comminuted fractures of the radial head associated with dislocation of the elbow, since these show the worst prognosis. Displaced marginal fractures below 3 mm are suited for early functional treatment using conservative procedures. 18 patients suff

Intraarticular fractures require a surgical therapy. The functional results are depending on the anatomical reconstruction of the joint surfaces and on early kinesitherapy. Remaining disabilities may be controlled in 2/3 of the cases to such an extent that no alterations are necessary in private or professional life. The overall result was very good in 23.8%, good in 36.0%, moderate in 22.3% and bad in 17.9%. These bad results are due to the rate of polytraumatized patients (18%) and the high rate of open comminuted fractures (27%).
