Central Sensitization in Knee Osteoarthritis: Relating Presurgical Brainstem Neuroimaging and PainDETECT-Based Patient Stratification to Arthroplasty Outcome.

OBJECTIVE: The neural mechanisms of pain in knee osteoarthritis (OA) are not fully understood, and some patients have neuropathic-like pain associated with central sensitization. To address this, we undertook the present study in order to identify central sensitization using neuroimaging and PainDETECT and to relate it to postarthroplasty outcome. METHODS: Patients awaiting arthroplasty underwent quantitative sensory testing, psychological assessment, and functional magnetic resonance imaging (fMRI). Neuroimaging (fMRI) was conducted during punctate stimulation (n = 24) and cold stimulation (

Centrality of prefrontal and motor preparation cortices to Tourette Syndrome revealed by meta-analysis of task-based neuroimaging studies.

Tourette Syndrome (TS) is a neurodevelopmental condition characterized by chronic multiple tics, which are experienced as compulsive and 'unwilled'. Patients with TS can differ markedly in the frequency, severity, and bodily distribution of tics. Moreover, there are high comorbidity rates with attention deficit hyperactivity disorder (ADHD), obsessive compulsive disorder (OCD), anxiety disorders, and depression. This complex clinical profile may account for apparent variability of findings across neuroimaging studies that connect neural function to cognitive and motor behavior in TS. Here we c

Imaging structure and function in refractory focal epilepsy.

Over the past decade there have been many advances in data acquisition and analysis for structural and functional neuroimaging of people with epilepsy. New imaging sequences and analysis techniques have increased the resolution of images such that underlying structural pathology can be seen in many patients with "cryptogenic" epilepsy. When an epileptogenic lesion is present, antiepileptic drugs alone rarely prevent seizures. However, the success of surgical treatment is improved when a structural lesion has been identified. Lesions might not overlap with the area of the cortex generating seiz…

Abnormalities of intrinsic brain activity in late-life depression: a meta-analysis of resting-state functional imaging studies.

Neuroimaging studies using a variety of techniques have demonstrated abnormal patterns of spontaneous brain activity in patients with late-life depression (LLD). However, the findings are variable and often inconsistent, hindering understanding of underlying neuropathology. We conducted a meta-analysis of whole-brain resting-state functional neuroimaging studies in LLD compared to healthy controls (HC), using anisotropic effect-size seed-based d mapping, to identify the most consistent brain activity alterations and their relation to clinical features. After systematic literature search, we in…

Biomarkers of functional movement disorders - a systematic review.

INTRODUCTION: Functional movement disorders (FMD) are defined by diverse phenotypes of altered movements that lack corresponding pathology in an anatomical region, and are typically characterized by inconsistent findings on neurological examination. STATE OF THE ART: While there are several suggestive clinical features indicating FMD, objective biomarkers are still lacking. We conducted a systematic review of the literature with an emphasis on literature published after February 2019 aiming to summarise current knowledge on biomarkers of FMD. We divided our findings into four main categories:

Mild cognitive impairment in multiple system atrophy: a brain network disorder.

Cognitive impairment (CI), previously considered as a non-supporting feature of multiple system atrophy (MSA), according to the second consensus criteria, is not uncommon in this neurodegenerative disorder that is clinically characterized by a variable combination of autonomic failure, levodopa-unresponsive parkinsonism, motor and cerebellar signs. Mild cognitive impairment (MCI), a risk factor for dementia, has been reported in up to 44% of MSA patients, with predominant impairment of executive functions/attention, visuospatial and verbal deficits, and a variety of non-cognitive and neuropsyc

Functional Near-Infrared Spectroscopy in the Study of Speech and Language Impairment Across the Life Span: A Systematic Review.

Purpose Functional brain imaging is playing an increasingly important role in the diagnosis and treatment of communication disorders, yet many populations and settings are incompatible with functional magnetic resonance imaging and other commonly used techniques. We conducted a systematic review of neuroimaging studies using functional near-infrared spectroscopy (fNIRS) with individuals with speech or language impairment across the life span. We aimed to answer the following question: To what extent has fNIRS been used to investigate the neural correlates of speech-language impairment? Method

Neural correlates and role of medication in reactive motor impulsivity in Tourette disorder.

Abnormality of inhibitory control is considered to be a potential cognitive marker of tics in Tourette disorder (TD), attention deficit hyperactivity disorder (ADHD), and impulse control disorders. The results of the studies on inhibitory control in TD showed discrepant results. The aim of the present study was to assess reactive inhibitory control in adult TD patients with and without antipsychotic medication, and under emotional stimulation (visual images with positive, neutral and negative content). We assessed 31 unmedicated and 19 medicated TD patients and 26 matched healthy controls usin

Parametric fMRI of paced motor responses uncovers novel whole-brain imaging biomarkers in spinocerebellar ataxia type 3.

Machado-Joseph Disease, inherited type 3 spinocerebellar ataxia (SCA3), is the most common form worldwide. Neuroimaging and neuropathology have consistently demonstrated cerebellar alterations. Here we aimed to discover whole-brain functional biomarkers, based on parametric performance-level-dependent signals. We assessed 13 patients with early SCA3 and 14 healthy participants. We used a combined parametric behavioral/functional neuroimaging design to investigate disease fingerprints, as a function of performance levels, coupled with structural MRI and voxel-based morphometry. Functional magne…

The Influence of Thyroid Hormones on Brain Structure and Function in Humans.

The pleiotropic function of thyroid hormones (TH) is mediated by an organ specific expression of thyroid hormone transporters, deiodinases and TH receptors. In a series of studies we used the model of an experimentally induced hyper- or hypothyroidism in human volunteers to delineate TH action on the brain. A battery of neuropsychological testing paradigms was employed and complemented by structural and functional multimodal neuroimaging. Experimentally induced mild thyrotoxicosis for 6 weeks was associated with changes in brain structure (determined with voxel-based morphometry), resting stat

Multimodal attention fusion deep self-reconstruction presentation model for Alzheimer's disease diagnosis and biomarker identification.

The unknown pathogenic mechanisms of Alzheimer's disease (AD) make treatment challenging. Neuroimaging genetics offers a method for identifying disease biomarkers for early diagnosis, but traditional approaches struggle with complex non-linear, multimodal and multi-expression data. However, traditional association analysis methods face challenges in handling nonlinear, multimodal and multi-expression data. Therefore, a multimodal attention fusion deep self-restructuring presentation (MAFDSRP) model is proposed to solve the above problem. First, multimodal brain imaging data are processed throu

Neuroimaging in neuro-ophthalmology.

The modern imaging era has introduced a variety of techniques that aid in the evaluation of complex neurologic problems. To optimize the yield of neuroimaging the clinician must, first and foremost, determine the nature of the neuro-ophthalmic disorder; and then localize the lesion. Once the localization of the neuro-ophthalmic problem is understood, the optimal imaging modality can be directed toward the anatomic region of interest. In this article the approach to neuroimaging is discussed, with emphasis on the anatomic localization of lesions affecting afferent and efferent visual function.
