Clinical and laboratorial impact of antiretroviral therapy in a cohort of Portuguese patients chronically infected with HIV-2.

INTRODUCTION: HIV-2 infection is endemic in West Africa and some European countries, namely Portugal. HIV-2 antiretroviral (ARV) treatment presents some restrains related to intrinsic resistance to non-nucleoside reverse transcriptase inhibitors (NNRTI) and fusion inhibitors, and poorer response to protease inhibitors (PI). MATERIAL AND METHODS: Retrospective observational study of a cohort of 135 infected HIV-2 patients, diagnosed between 1989 and 2008. OBJECTIVES: Evaluation of epidemiologic, clinical, immunologic and virologic progression, comparing to groups of patients (naïve vs ARV exp

Initiation of dolutegravir vs. efavirenz on 12- and 24-month retention and viral suppression: a target trial emulation.

BACKGROUND: South Africa's antiretroviral therapy (ART) treatment guidelines in 2019 were revised to use dolutegravir as part of first-line ART instead of efavirenz due to recommendations from the World Health Organization and findings from clinical trials indicating noninferior efficacy and reduced side effects. Utilizing the target trial framework, we estimated the effect of initiating a dolutegravir-based regimen compared to an efavirenz-based regimen among treatment-naïve people living with HIV initiating treatment in Johannesburg, South Africa from 2019 to 2022 on retention and viral supp

Influence of concurrent oral calcium carbonate supplementation on steady-state pharmacokinetics of once daily oral raltegravir in persons with HIV: a protocol for a prospective open-label non-randomised study in Canada.

INTRODUCTION: Raltegravir is a potent HIV-integrase strand transfer inhibitor (INSTI). Despite its strong activity against HIV-1 strains resistant to other antiretroviral drug classes, it is usually used in combination with other antiretroviral drugs due to the empirical requirement for anti-HIV drug combinations to ensure effective anti-retroviral therapy (ART). As an early-arriving INSTI, raltegravir is clinically familiar for its safety, tolerability and treatment effectiveness. High-dose calcium carbonate formulated as an antacid (as opposed to a supplement formulation) taken orally togeth

Clinical presentation and effectiveness of antiretroviral therapy in newly diagnosed people living with HIV before, during and after the COVID-19 pandemic.

BACKGROUND: In Italy, the COVID-19 pandemic substantially disrupted access to health care services, including HIV screening, contributing to a decline in new HIV diagnoses and increasing late presentations in the postpandemic years. This study assessed the profile and trends of newly diagnosed people living with HIV (ndPLWH) across the prepandemic, peripandemic and postpandemic periods in two major Italian centres. METHODS: We conducted a retrospective observational study including adults diagnosed with HIV-1 infection between 2015 and 2023 at two HIV outpatient clinics in Central (Centre A)

Patient-Reported Outcomes Among Virally Suppressed People Living with HIV Switching to Dolutegravir/Lamivudine vs. Continuing Bictegravir/Emtricitabine/Tenofovir Alafenamide from the Randomized DYAD Study.

In the DYAD study, switching to dolutegravir/lamivudine (DTG/3TC) was noninferior to continuing bictegravir/emtricitabine/tenofovir alafenamide (B/F/TAF) in virologically suppressed adults. Here, we present changes in patient-reported outcomes (PROs) among participants who completed Week (W) 48. DYAD used PRO measures to evaluate HIV- and antiretroviral-related symptoms (HIV-SI) and treatment satisfaction (HIVTSQ/c) at baseline, W24, and W48. Symptoms described as bothersome versus not bothersome were compared across time by treatment group using chi-squared testing. Changes from baseline in H

HIV-1 subtype-specific drug resistance on dolutegravir-based antiretroviral therapy: protocol for a multicentre study (DTG RESIST).

INTRODUCTION: HIV drug resistance poses a challenge to the United Nation's goal of ending the HIV/AIDS epidemic. The integrase strand transfer inhibitor (InSTI) dolutegravir, which has a higher resistance barrier, was endorsed by the WHO in 2019 for first-line, second-line and third-line antiretroviral therapy (ART). This multiplicity of roles of dolutegravir in ART may facilitate the emergence of dolutegravir resistance. METHODS AND ANALYSIS: Nested within the International epidemiology Databases to Evaluate AIDS (IeDEA), DTG RESIST is a multicentre study of adults and adolescents living wit

Engineering vaginal film platform for mucoadhesion and sustained drug release for HIV-1 prevention.

User adherence contributes to the effectiveness of topical pre-exposure prophylactic products designed to protect against human immunodeficiency virus type 1 (HIV-1) infection. Long-acting approaches that do not require daily or coitally-dependent use could potentially improve user adherence. This study aims to develop a long-acting vaginal film to deliver an integrase inhibitor, MK-2048, for prevention of HIV-1 infection. To this end, a film platform fabricated with thiolated chitosan and poly(lactide-co-Ɛ-caprolactone) (PLACL) was designed to achieve long residence time and sustained drug re

Curcumin-enhanced elvitegravir therapy mitigates neuroinflammation and cognitive deficits in EcoHIV mice.

HIV-associated neurocognitive disorders (HAND) persist in up to 50% of people living with HIV (PLWH) despite effective antiretroviral therapy (ART), driven by chronic neuroinflammation, oxidative stress, and neuronal damage. This study investigates the therapeutic potential of combining elvitegravir (EVG), an integrase strand transfer inhibitor, with curcumin (CUR), a natural polyphenol with anti-inflammatory and antioxidant properties, in a murine EcoHIV model of HAND. EcoHIV-infected mice were treated with EVG, CUR, or their combination (EVG + CUR), and cognitive, motor, and molecular outcom

HIV-1 subtype B-infected MSM may have driven the spread of transmitted resistant strains in France in 2007-12: impact on susceptibility to first-line strategies.

BACKGROUND: Our study describes the prevalence of transmitted drug resistance (TDR) among 1318 French patients diagnosed at the time of primary HIV-1 infection (PHI) in 2007-12. METHODS: HIV-1 resistance-associated mutations (RAMs) were characterized using both the 2009 WHO list of mutations and the French ANRS algorithm. A genotypic susceptibility score was estimated for each first-line recommended ART combination. RESULTS: Patients were mainly MSM (72.6%). Non-B variants were identified in 33.7% of patients. The proportion of TDR was estimated as 11.7% (95% CI 10.0-13.5). The prevalences o

Cardiopulmonary Performance and Subclinical Myocardial Remodeling in Virologically Suppressed HIV Patients: The Role of the Metabolic Age Gap.

Despite the success of modern antiretroviral therapy in achieving virological suppression, people living with HIV face an elevated risk of cardiovascular diseases, particularly heart failure with preserved ejection fraction. This study evaluates the cardiometabolic phenotype and functional capacity in a Romanian HIV cohort to delineate the metabolic footprint of chronic infection. In this cross-sectional study based on prospectively collected, protocol-driven phenotyping, we evaluated 50 consecutive outpatients from a university-affiliated infectious diseases clinic in Timisoara. Real-World Safety and Effectiveness of Cabotegravir in People Living with HIV: Interim Report of a Post-marketing Surveillance in Japan.

INTRODUCTION: Cabotegravir (CAB) is an integrase strand transfer inhibitor and the first long-acting injectable antiretroviral agent for human immunodeficiency virus (HIV), administered with rilpivirine. Because approval in Japan relied largely on overseas and international collaborative trials, domestic real-world evidence remains limited. This study evaluated the real-world safety and effectiveness of CAB in people living with HIV (PLHIV) in Japan. METHODS: This interim analysis (June 27, 2022 and June 17, 2025) was part of an ongoing post-marketing surveillance of Japanese PLHIV treated wi
