Assessing attitudes towards elements of the overdose response hotlines/applications (ORHAs).

INTRODUCTION: In response to the overdose epidemic, novel strategies including Overdose Response Hotlines and Applications (ORHAs) have been introduced to help mitigate the crisis. These technologies enable individuals with a phone to access harm reduction support via smartphones and applications. Such supports include overdose monitoring, access to social services, mental health referrals, and more. This study analyzed data from the Canadian National Questionnaire on Overdose Monitoring (CNQOM), a large bilingual national survey, to evaluate the perspectives of people who use unregulated subs…

Exploratory Analysis of the Characteristics Associated With the Use of Fentanyl Test Strips Among People Who Use Drugs in Rhode Island, 2021-2023.

OBJECTIVES: Fentanyl overdose deaths are a critical public health issue in the United States. Rhode Island, an early epicenter of the fentanyl crisis, has seen fentanyl overdose deaths increase 30-fold since 2009. In response, harm reduction tools such as fentanyl test strips (FTSs) have been introduced to help people who use drugs (PWUD) detect fentanyl in their substances. We analyzed FTS use among PWUD in Rhode Island from 2021 through 2023 and identified characteristics associated with their use. METHODS: We conducted a pooled cross-sectional analysis using aggregate data from the Rhode I…

Development of lower-risk guidelines for online sports and race betting in Australia using objective behavioural data.

BACKGROUND: Lower risk gambling guidelines provide clear, actionable advice for reducing the risk associated with gambling by specifying behaviour associated with non-harmful use. Here we aimed to develop lower-risk guidelines specific to online sports and race betting in Australia that Currie (Addiction 112:2011-20, 2017) captured behaviours unique to this activity associated with high risk of experiencing gambling harms (e.g., depositing frequency, number of sports & races bet on) and Currie (Health Promot 34:1207-17, 2019) were based on objective behavioural data from a gambling operator no

Modelling the impact of cuts in US PEPFAR funding for opioid agonist therapy and needle and syringe programmes on drug-related deaths and HIV and hepatitis C transmission among people who inject drugs.

BACKGROUND: The US government paused funding for the US President's Emergency Plan for AIDS Relief (PEPFAR) in 2025, including disruptions i … number of PWID that may stop accessing OAT through PEPFAR (2024 data; 37,024) and used a survey of harm reduction providers to estimate decreases in NSP provision (46.4% decrease, range 21.1-71.7% across 7/9 countries). We estimated the additional primary HIV/HCV infections over 1-year of disruptions, potential secondary infections over 5-years, and relative in …

Treatment of Attention-Deficit/Hyperactivity Disorder With Stimulants: Reducing Harms.

An increase in attention-deficit/hyperactivity disorder (ADHD) diagnoses since 2020, along with shortages in prescription stimulants and an increase in overdose deaths from stimulants, has triggered alarm within the public sector, regulatory agencies, and health care systems. Stimulants are classified as Schedule II drugs by the Drug Enforcement Administration due to the potential for misuse and physiological and psychological dependence. Despite concerns, stimulants are effective first-line treatments for children and adults with ADHD. The majority of deaths related to stimulants are linked t

US drug policy does not align with experts' rankings of drug harms: a multi-criteria decision analysis.

Background United States drug policy is primarily based on the Controlled Substances Act of 1970 and largely contradicts scientific evidence about how to mitigate drugs' harms. Expert consensus on drug harms could inform policy that improves the health of people who use drugs while reducing negative societal impacts of drug use. Therefore, this study adapted and extended the relevant 2010 United Kingdom multi-criteria decision analysis, rating drug harms by criteria comprising health and social impacts to people who use drugs and their families, communities, and society. Methods Seventeen exper

From Digital Inclusion to Digital Transformation in the Prevention of Drug-Related Deaths in Scotland: Qualitative Study.

BACKGROUND: Globally, drug-related deaths (DRDs) are increasing, posing a significant challenge. Scotland has the highest DRD rate in Europe and one of the highest globally. The Scottish Government launched the Digital Lifelines Scotland (DLS) program to increase the provision of digital technology in harm reduction services and other support services. Digital technology responses to DRDs can include education through digital platforms, improved access to treatment and support via telehealth and mobile apps, analysis of data to identify risk factors, and the use of digital tools for naloxone d…

How post overdose response efforts can address social determinants of health among people who use drugs: perspectives from the New York State Department of Health (NYSDOH) Post Overdose Response Team (PORT).

Individuals who have survived an overdose often have myriad needs that extend far beyond their drug use. The social determinants of health (SDOH) framework has been underutilized throughout the opioid overdose crisis, despite widespread acknowledgment that SDOH are contributors to the majority of health outcomes. Post Overdose Response Teams (PORTs) engage with individuals who have experienced 1 or more nonfatal overdoses and bear witness to the many ways in which overdose survivors experience instability with healthcare, housing, employment, and family structure. Employing a harm reduction mo…

PrEP in Prisons: HIV prevention in incarcerated populations.

PURPOSE: The purpose of this paper is to discuss the possibility of using pre-exposure prophylaxis (PrEP) as an HIV harm reduction intervention in prisons. PrEP is primarily discussed in relation to men who have sex with men (MSM), meaning other high-risk populations, such as prisoners, are often side-lined. The authors wanted to consider how it could prove beneficial beyond the MSM community. DESIGN/METHODOLOGY/APPROACH: First, the authors discuss whether the common objections to existing HIV harm reduction interventions in prisons, such as needle exchanges, are applicable to PrEP. The autho…

Overdose prevention for prisoners in New York: a novel program and collaboration.

This is a brief report on the establishment of a new program in New York State prisons to prepare prisoners to avoid the increased risks of drug overdose death associated with the transition to the community by training them in overdose prevention and making available naloxone, a medication that quickly reverses the effects of an opioid overdose, to all prisoners as they re-enter the community. It is a milestone collaboration in the USA between public health, the correctional system, and a community-based harm reduction program in response to the growth of heroin and opioid analgesic use and r…

Sharp force injuries in "clinical" forensic medicine.

In general, the forensic evaluation of sharp force injuries in living and dead individuals follows the same morphologic principles. Still, there are some special features of sharp force injuries in the clinical context, which have to be considered as examination findings on the living are interpreted to differentiate between accidental origin, self-infliction or homicidal assault. These include the frequency and localization of defence injuries, injuries of the perpetrator, and artificial injuries, especially those inflicted for the purpose of insurance fraud. Characteristics and differential

Thinking ethically about needle and syringe programs.

Accepting-for the sake of argument-our current legal policies concerning heroin use and its users, what ethical questions are raised for needle and syringe program (NSPs)? Do they weaken drug laws, send the wrong message or obscure the right message, do little to eliminate the harm of drugs, detract from alternatives, and/or constitute a counsel of despair? I suggest that in the absence of established better alternatives, NSPs constitute a morally acceptable and in some cases even desirable option despite the continued criminalization of injecting drug use. Yet they must be conceived and admin
