Reorganizing Hospital Cancer Care In Nepal.

While mortality rates for major common cancers- of the lung, breast, stomach, ovary, and lymphomas-- have declined significantly in high-income countries over the last decade, comparable rates for these malignancies have not fallen in Nepal. The explanations for the high-income country mortality trends are multiple and certainly include incidence reductions, but better treatments are also contributory. How to bring to Nepal the global lessons in achieving better cancer care is uncertain. We need to create an environment of ideas and a powerful vision to meet the needs for better major cancer m

Regional Implementation of a Pediatric Cardiology Syncope Algorithm Using Standardized Clinical Assessment and Management Plans (SCAMPS) Methodology.

BACKGROUND: Pediatric syncope is common. Cardiac causes are rarely found. We describe and assess a pragmatic approach to these patients first seen by a pediatric cardiologist in the New England region, using Standardized Clinical Assessment and Management Plans (SCAMPs). METHODS AND RESULTS: Ambulatory patients aged 7 to 21 years initially seen for syncope at participating New England Congenital Cardiology Association practices over a 2.5-year period were evaluated using a SCAMP. Findings were iteratively analyzed and the care pathway was revised. The vast majority (85%) of the 1254 patients

Electronic Health Record Use in Internal Medicine Clerkships and Sub-internships for Medical Students Graduating from 2012 to 2016.

BACKGROUND: As electronic health records (EHRs) became broadly available in medical practice, effective use of EHRs by medical students emerged as an essential aspect of medical education. While new federal clinical documentation guidelines have the potential to encourage greater medical student EHR use and enhance student learning experiences with respect to EHRs, little is known nationally about how students have engaged with EHRs in the past. OBJECTIVE: This study examines medical student accounts of EHR use during their internal medicine (IM) clerkships and sub-internships during a 5-year

Psychosocial care to affected citizens and communities in case of CBRN incidents: a systematic review.

Disasters are associated with a substantial psychosocial burden for affected individuals (including first responders) and communities. Knowledge about how to address these risks and problems is valuable for societies worldwide. Decades of research into post-disaster psychosocial care has resulted in various recommendations and general guidelines. However, as CBRN (chemical, biological, radiological, nuclear) events form a distinctive theme in emergency planning and disaster preparedness, it is important to systematically explore their implications for psychosocial care. The aim of this study i

The use of deliberative dialogue in health services research: a scoping review.

INTRODUCTION: Deliberative Dialogue (DD) is a structured participatory approach that brings together research evidence, professional expertise, and lived/living experience to support informed discussion and decision-making in health research. DD is increasingly used in the co-design, co-implementation, and co-evaluation of health-promoting interventions, where decisions must be both evidence-informed and responsive to local contexts, priorities, and needs. However, published studies vary considerably in how DD is described and reported, particularly in relation to facilitation, evidence use, p

A quality management system aiming to ensure regulatory-grade data quality in a glaucoma registry.

BACKGROUND: Disease/patient registries are underutilized despite their multiple advantages over clinical trials in the clinical evaluation of drugs, such as the capacity for long-term curation, provision of patient outcome data in routine clinical practice, and provision of benchmark data for comparison. Ensuring the fit-for-purpose quality of data generated from such registries is important to informing regulatory decision making. Here, we report the construction of a quality management system aiming to ensure regulatory-grade data quality for a registry of Japanese patients with glaucoma to

Medical Advice for Travelers.

BackgroundIn 2019, 1.5 billion international tourist trips were counted worldwide. Germany, with 70.8 million vacations lasting ≥ 5 days, was one of the populations most willing to travel. These days, even elderly and multimorbid persons regularly travel long-distance, which can be associated with significant health risks. By advising travelers and implementing preventive measures, the risk of illness can be reduced significantly.MethodsA selective survey of PubMed was performed to identify publications on medical advice for travelers between 2000 and 2020. We included guidelines, studies, and

Identifying core competencies for community oral health practice among dental hygienists: a scoping review.

BACKGROUND: Community oral health practice increasingly requires dental hygienists to perform prevention-oriented, population-based, and interprofessional roles beyond traditional chairside clinical care. However, existing competency frameworks primarily emphasize clinical practice or general professional competencies, and an integrated competency framework specifically designed for community oral health practice remains unavailable. This study aimed to identify core competencies for community oral health practice among dental hygienists through a scoping review. METHODS: A scoping review was

Curricular integration of a 4-year simulation-based education program on clinical reasoning for physiotherapy students: a descriptive design and implementation report.

BACKGROUND: Simulation-based education remains underutilized in physiotherapy and is often limited to isolated experiences rather than longitudinal and systematically integrated approaches with a specific curriculum. This restricts its potential to foster clinical reasoning (CR)-a core competence for diagnosis and treatment planning. This study presents the design and implementation report of a four-year, simulation-based education program (SBEP) focused on CR. METHODS: This study reports the design, implementation and iterative refinement of a four-year SBEP using instructional and quality-i

UK consensus guidelines for multidisciplinary care of children and young people with achondroplasia: a modified Delphi process.

BACKGROUND: Achondroplasia (ACH), the most common skeletal dysplasia, arises from gain-of-function variants in the fibroblast growth factor receptor 3 gene. Children with ACH experience lifelong medical, functional and psychosocial challenges requiring coordinated and anticipatory care. Although international guidance exists, the UK lacks national clinical care recommendations specific to its healthcare systems. OBJECTIVE: To develop UK-specific, multidisciplinary clinical recommendations for the care of children and young people (CYP) with ACH. METHODS: The UK Achondroplasia Network develop
