A neuroma of a calcaneal nerve has never been reported. A series of 15 patients with heel pain due to a neuroma of a calcaneal nerve are reviewed. These patients previously had either a plantar fasciotomy (n = 4), calcaneal spur removal (n = 2), ankle fusion (n = 2), or tarsal tunnel decompression (n = 7). Cytokine levels were compared in aqueous extracts of stratum corneum from psoriatic lesions and normal heel. Samples from heel contained high levels of interleukin-1 alpha (IL-1 alpha) and beta measured in immunoassays, although only the IL-1 alpha was biologically active. The Syme and Boyd amputations at the ankle were compared in thirty-four children followed for three to sixteen years. Good results were found with both techniques. The best results occurred when the heel pad was aligned in the plantigrade position, which produced almost normal function of the heel pad. The Boyd technique was a surer method of obtaining such alignment but was technically more difficult. Both the Boyd and Syme techniques are recommended for ankle amputation in children, but a distinct advantage of one technique over the other could not be defined.

For clubfeet resistant to conservative treatment, an early soft-tissue release and reduction of talonavicular dislocation are recommended. To prevent the deforming influences towards varus of the often small inverted heel and make it reach the ground properly, a Dwyer osteotomy of the calcaneum is indicated. In a series of 22 patients this operation was performed on 29 clubfeet. The results at follow-up were satisfactory in all of the patients operated on before the age of 1 year, the youngest being 3 months. Young patients with a varus and small inverted heel respond very well.

An 81-year-old Japanese man with no history of diabetes mellitus was admitted to our hospital for a fever with a new ulcerative lesion on the left heel. A 2-year-old boy presented at our hospital with severe familial popliteal pterygium syndrome with extensive skin webbing from thigh to heel and severely reduced range of motion of the knee and ankle joints. For accomplishment of knee extension, the patient underwent surgery with resection of the fibrous bands, freeing of the sciatic nerve, Z-lengthening of the Achilles tendon and multiple Z-plasties. One year after surgery, the patient can put his heel on the ground and has almost complete range of motion in the knee and ankle joints.

Fifteen patients complaining of chronic heel pain underwent three-phase Tc-99m MDP bone scintigraphy. Ten patients demonstrated abnormal scan findings consistent with plantar fasciitis (PF) and had responded to conventional therapy. Two patients were found to have calcaneal stress fractures, and one patient demonstrated a calcaneal spur that required no treatment. The remaining two patients had normal scans and did not appear clinically to have PF. The three-phase bone scan is therefore very useful in diagnosing PF and in distinguishing it from other etiologies of the painful heel syndrome.
