AIMS: To identify patients with a past history of peptic ulcer and to treat Helicobacter pylori infection if confirmed.
METHODS: Patient records from 6.5 full-time equivalent general practitioners were reviewed to record prescribing of H2-receptor antagonists in the last year and to review previous upper gastrointestinal investigations.

The aim of this study was to determine prospectively whether Helicobacter pylori-associated gastritis is associated with specific symptoms by evaluating whether these symptoms are relieved by treatment of the infection. Symptoms resolved after the eradication of H. pylori in only three of eight children with H. pylori-associated gastritis alone, in comparison with all six children with duodenal ulcer disease.

Enzyme-linked immunosorbent assays (ELISA) have been developed to diagnose H. pylori infection. However, the methods are not standardized. My group and others have asked whether H. pylori itself alters gastric physiology. This infection has been found to decrease local expression of the inhibitory peptide somatostatin, and to increase release of the acid-stimulating hormone gastrin. BACKGROUND: All duodenal ulcers and the vast majority of gastric ulcers are the consequence of Helicobacter pylori-associated inflammation. In duodenal ulcer disease, the inflammation is maximally intense in the antrum and is associated with gastric metaplasia in the bulb. H. pylori is presumably more virulent in duodenal ulcer disease. Gastrin homeostasis is disturbed and there is robust acid secretion. In gastric ulcers, there is pangastritis with a degree of atrophy. Acid secretion is normal or low.
