During the 18 months January 1975 to June 1976, 25 cases of acute herpetic follicular conjunctivitis and keratoconjunctivitis resembling adenovirus ocular infection presented in the External Eye Disease Clinic, Moorfields Eye Hospital, City Road, London. Herpes simplex virus was isolated in HEp2 cells in 22 patients, and the remaining 3 patients were identified by a minimum 4-fold rise in the level of antiherpes simplex virus antibody in their blood. No adenovirus was isolated from these patients, but complement fixation test for adenovirus was positive in 1 patient with cultural test positive…

Two cases of meningitis are reported in which the etiologic diagnosis was established by isolation of herpes simplex virus from the cerebrospinal fluid. Both patients were afebrile and had bradycardia when the cultures were obtained, and one patient had been thought to have leukemic meningitis. This experience added to that of others in the literature indicates the value of culture for herpes simplex virus in the cerebrospinal fluid.

Macrophages restrict herpes simplex virus replication and can prevent the development of herpetic disease in mice. In an attempt to define the nature of this restriction, an analysis of virus-specified macromolecular syntheses in infected macrophages was undertaken. The significant results were the following: All cells were killed, but the infection was considered to be abortive since the level of infectious virus in macrophage cultures dropped steadily to a level beyond detection by 25 hr after infection. This restriction appeared to be specific for macrophages; the virus replicated efficient…

Herpes simplex encephalitis (HSE) is a severe viral infection of the central nervous system that most commonly affects the temporal lobes and limbic structures. Although most cases are managed medically with intravenous acyclovir, rare cases can rapidly progress to malignant cerebral edema, mass effect, and neurologic deterioration requiring surgical intervention. We report the case of a 57-year-old woman presenting with altered mental status, headache, vomiting, myalgias, and focal neurologic deficits. Initial computed tomography (CT) and CT angiography of the head and neck were unremarkable,…

PURPOSE: This study aimed to characterize the temporal dynamics of corneal immune responses and programmed cell death (PCD) following herpes simplex virus type 1 (HSV-1) infection, and investigate the role of PANoptosis in shaping the immune responses during herpes simplex keratitis (HSK).
METHODS: A murine HSK model was established through HSV-1 infection, and time-course RNA sequencing was performed on murine corneas at 0, 1, 3, 7, and 14 days post-infection. Phenotypic and mechanistic analyses were conducted using quantitative real-time PCR, western blotting, immunofluorescence staining, f…

PURPOSE: Herpes simplex keratitis (HSK) is a leading cause of infectious corneal blindness, yet local host-response programs underlying active disease, recurrence, and stromal involvement remain incompletely understood. This study aimed to establish a deep tear proteomic atlas of HSK and characterize phenotype-associated local host-response states.
METHODS: Data-independent acquisition mass spectrometry was used to profile tear proteins from HSK-affected eyes (n = 20), contralateral unaffected eyes (n = 12), and healthy control eyes (n = 25). Differential abundance testing, co-expression netw…

PURPOSE: Herpes simplex virus 1 encephalitis (HSE) is the most common sporadic infectious encephalitis in Western countries, with a 70% mortality rate and only 9% of survivors free from neurological sequelae. While definitive diagnosis relies on cerebrospinal fluid testing, magnetic resonance imaging (MRI) plays a crucial role in identifying typical acute patterns and features. However, the imaging evolution of encephalitic lesions is not well understood. We aimed to identify and evaluate the prevalence and progression of cortical lesions, as well as the recurrence of these patterns in HSE and…

BACKGROUND: This study was performed to investigate the clinical characteristics and outcome of patients with herpes simplex (HSV) type 1 encephalitis.
METHODS: Nationwide, prospective, population-based cohort study using the Danish Study Group for Infections of the Brain database to identify all adults hospitalized with microbiologically confirmed HSV-1 encephalitis from 2015 to 2023 in Denmark. Modified Poisson regression was used to estimate the relative risk (RR) with 95% confidence intervals (CI) for 6-month mortality, adjusted for age, sex, immunocompromise, altered mental status, imagi…

Cutaneous arteriovenous haemangioma (AVH) is a rare benign vascular lesion, which typically occurs on the head and neck. Its aetiology is unclear but thrombosis, trauma, infection or endocrine triggers have been proposed. We report the case of a 64-year-old female presenting with acquired AVH of the upper lip following oral herpes simplex virus infection. This is the first documented occurrence of AVH as a post-herpetic isotopic response (PHIR).

Herpes simplex virus type 1 (HSV-1), a ubiquitous neurotropic virus, has been increasingly associated with neurodegenerative disorders through mechanisms extending beyond acute encephalitis to include chronic neuroinflammation, proteostatic disruption, and immune dysregulation. Simultaneously, the gut-brain-immune axis has gained recognition as a master regulator of central nervous system (CNS) homeostasis. In this review, we delineate how HSV-1 neurovirulence intersects with gut microbiota dynamics and immune signaling to shape neuropathological trajectories. We explore five interconnected do…

Herpes simplex encephalitis (HSE) patients may develop secondary anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis (NMDARE), associated with worsened long-term neurological outcome. Immunosuppressive treatment can limit NMDAR autoantibody-mediated pathology, but early predictive biomarkers for the risk of NMDARE are lacking. In a multicenter study, we performed unbiased antibody reactome profiling using Phage ImmunoPrecipitation Sequencing (PhIP-Seq). HSE patients with secondary NMDARE (n = 13) versus those without (n = 10) showed enhanced antibody responses against HSV-1, but not HSV-2,…

INTRODUCTION: Herpes simplex keratitis (HSK) is a blinding disease caused by corneal infection of Herpes simplex virus type 1 (HSV-1). Effective clearance of HSV-1 from the infected cornea is crucial for HSK management. Macrophages play an important part in the innate immune defense against viral infections. This study investigates the immunomodulatory role of NLRP12 in macrophage immune response during HSV-1 infection.
METHODS: NLRP12 expression post-infection was assessed in various macrophage cell lines. Overexpression of NLRP12 was achieved by lentiviral transfection, and its effect on HS…

Gene drives are genetic modifications designed to propagate efficiently through a population. Most applications rely on homologous recombination during sexual reproduction in diploid organisms such as insects, but we recently developed a gene drive in herpesviruses that relies on co-infection of cells by wild-type and engineered viruses. Here, we report on a viral gene drive against human herpes simplex virus 1 (HSV-1) and show that it propagates efficiently in cell culture and during HSV-1 infection in mice. We describe high levels of co-infection and gene drive-mediated recombination in neur…

BACKGROUND: Cervicitis, an infectious or noninfectious inflammation of the cervix, encompasses a wide range of clinical conditions, from asymptomatic infections to severe lesions, making its diagnosis difficult. Acute cervicitis may develop into pelvic inflammatory disease. In patients with cervicitis, current guidelines recommend testing for herpes simplex virus when external genital lesions are present. Here, we present the case of a patient with an atypical primary herpes simplex virus 2 infection manifesting as cervicitis without genital lesions.

A 7-year-old female child was referred to the cornea clinic with a painless, ulcerated, and inflamed nodule near the limbus which did not respond to 1-week antibiotic therapy and worsened on starting topical steroids. Clinical examination showed ulcerative nodular episcleritis and raised a suspicion of herpes simplex virus etiology. The patient responded well and resolved completely on topical antiviral therapy alone.
