Bedside Neonatal Intensive Care Unit Correction of Congenital Diaphragmatic Hernia: Is Repair without Compromise?

Objectives This study aims to evaluate the feasibility, safety, limitations, and outcomes of performing different surgical approaches and techniques for the bedside repair of congenital diaphragmatic hernia (CDH) in critically ill patients who cannot be transferred to the operating room. Study Design Between December 1997 and July 2013, medical charts of all neonates operated on at the bedside for CDH while on high-frequency oscillatory ventilation (HFOV) and nitric oxide were reviewed. Demographic data; contributing antenatal, perinatal, and postnatal factors; clinical presentation; associate

Laryngeal cleft repair: the anesthetic perspective.

INTRODUCTION: Laryngeal cleft is a rare congenital malformation that is being reported with increasing frequency. Diagnosis requires suspension microlaryngoscopy under general anesthesia during spontaneous respiration. Repair may be attempted by a minimally invasive endoscopic approach or open surgical repair. The authors report on their experience with total intravenous anesthesia (TIVA) and spontaneous ventilation without an endotracheal tube during suspension laryngoscopy and CO2 laser application for this specific surgical procedure. Of particular interest were the rate at which this techn

A comparison of the Acapella and a threshold inspiratory muscle trainer for sputum clearance in bronchiectasis-A pilot study.

BACKGROUND: Inspiratory muscle training is used to specifically strengthen the respiratory muscles. Controversy exists regarding the use of inspiratory muscle training as a method of facilitating airways clearance. Acapella is already known to be effective in airway clearance. OBJECTIVE: The objective of the study was to compare the effects of the Acapella and a threshold inspiratory muscle trainer as a method of airway clearance in subjects with bronchiectasis and to determine patient preference between the two techniques. METHODS: Thirty patients (10 males, 20 females) mean age of 50.67+/-

Correlation of echocardiographic markers and therapy in persistent pulmonary hypertension of the newborn.

BACKGROUND: Persistent pulmonary hypertension of the newborn (PPHN) causes morbidity and mortality in neonates. High-frequency ventilation (HFV), inhaled nitric oxide (iNO), and extracorporeal membrane oxygenation (ECMO) are used when conventional treatment fails. This study aimed to identify echocardiographic predictors of progression to these therapies before clinical deterioration. METHODS: Echocardiographic parameters were compared for survival and need for ECMO, HFV, iNO, and prolonged mechanical ventilation (MV, >or=10 days). RESULTS: Of 63 neonates, 95% survived, with 14% requiring EC…

Fetal stabilization for antenatally diagnosed diaphragmatic hernia.

BACKGROUND/PURPOSE: Infants with congenital diaphragmatic hernia have pulmonary hypoplasia resulting in persistent pulmonary hypertension of neonates (PPHN), which is the main contributor to both high mortality and morbidity. The pulmonary artery bed in patients with congenital diaphragmatic hernia (CDH) is underdeveloped and is very sensitive to slight stimuli. It is, therefore, vital to avoid any factors that might increase pulmonary vascular resistance during the perinatal treatment of these patients. Recently, fetal anesthesia for perinatal stabilization in patients with CDH has been repor

Selective bilateral bronchial intubation for large, acquired tracheoesophageal fistula.

The anesthetic management of patients undergoing tracheoesophageal fistula repair often involves lung separation, usually selective bronchial intubation with a double-lumen endotracheal tube. However, in patients with airway fistulas arising below the tracheal lumen, selective lung ventilation and separation may require unusual methods of airway management. In the patient described in this report, the airway fistulas involved the distal 3 cm of the trachea, approximately half of the left main bronchus 1.5 cm beyond the carina, and the proximal 0.5 cm of the right main bronchus. To separate and

[Non-invasive mandatory ventilation in extremely low birth weight and very low birth weight newborns with failed respiration].

Early extubation of ELBW and VLBW premature infants treated with IMV results in decreased incidence of tracheal and laryngeal injury, lowers the risk of nosocomial infection, decreases the severity and frequency of bronchopulmonary dysplasia (BPD). Due to prematurity this group of patients is especially susceptible to extubation failure because of apnoe, hypoventilation and atelectasis. In clinical practice attempt was made to provide adequate noninvasive ventilation by the use of nasal intermittent mandatory ventilation in the case of apnoe of prematurity. Advantages of noninvasive nasal IMV

Airway management and smoke inhalation injury in the burn patient.

Smoke inhalation injury, a unique form of acute lung injury, greatly increases the occurrence of postburn morbidity and mortality. In addition to early intubation for upper-airway protection, subsequent critical care of patients who have this injury should be directed at maintaining distal airway patency. High-frequency ventilation, inhaled heparin, and aggressive pulmonary toilet are among the therapies available. Even so, immunosuppression, intubation, and airway damage predispose these patients to pneumonia and other complications.

Life-threatening human metapneumovirus pneumonia requiring extracorporeal membrane oxygenation in a preterm infant.

We present the first report in the literature of a child with human metapneumovirus pneumonia who required extracorporeal membrane oxygenation for survival. This was a 3-month-old premature boy from British Columbia, Canada, who developed severe respiratory failure, experienced failure of high-frequency oscillatory mechanical ventilation, and required extracorporeal membrane oxygenation support for 10 days. This case illustrates the importance of including this newly discovered pathogen among the causes of childhood pneumonia.

Feasibility of surgery for patent ductus arteriosus of premature babies in a neonatal intensive care unit.

Twenty-two premature neonates underwent surgical haemoclip closure of patent ductus arteriosus in a neonatal intensive care unit (NICU). Surgery was performed in the NICU in order to prevent hypothermia and interruption of care, and to avoid accidental vascular access removal and extubation. The results suggest that morbidity can be limited by performing the surgery in an NICU, and by switching from conventional to high-frequency mechanical ventilation in case of severe respiratory failure.

Survey of neonatal respiratory support strategies.

AIM: To survey current practice regarding neonatal respiratory support strategies to determine whether it reflected evidence from randomised trials. METHODS: A questionnaire (in Supplementary Material online) survey of all U.K. neonatal units was undertaken to determine what modes of ventilation, types of endotracheal tube, lung function monitoring and oxygen saturation levels were used. RESULTS: There was an 80% response rate. Most (73%) units used in prematurely born infants (in the first 24 h) the intermittent positive pressure ventilation, and other respiratory modes included: CPAP (2%),

Terrestrial locomotion does not constrain venous return in the American alligator, Alligator mississippiensis.

The effects of treadmill exercise on components of the cardiovascular (heart rate, mean arterial blood pressure, central venous pressure, venous return) and respiratory (minute ventilation, tidal volume, breathing frequency, rate of oxygen consumption, rate of carbon dioxide production) systems and on intra-abdominal pressure were measured in the American alligator, Alligator mississippiensis, at 30 degrees C. Alligators show speed-dependent increases in tidal volume and minute ventilation, demonstrating that the inhibition of ventilation during locomotion that is present in some varanid and i

Response to inhaled nitric oxide in premature and term neonates.

Inhaled nitric oxide (iNO) has emerged as a promising therapeutic agent in the treatment of persistent pulmonary hypertension of the newborn. Several theories exist regarding causes of both response and nonresponse to iNO. Clinical trials differentiate disease entities (primary vs secondary persistent pulmonary hypertension associated with meconium aspiration syndrome, pneumonia or congenital diaphragmatic hernia) and their specific response rates. iNO combined with high-frequency ventilation appears to be superior to inhalation of nitric oxide (NO) during conventional ventilation. Little is k…

Survival in a recent cohort of mechanically ventilated pediatric allogeneic hematopoietic stem cell transplantation recipients.

There is ongoing discussion whether survival improved for children requiring mechanical ventilation after hematopoietic stem cell transplantation (HSCT). We reviewed the outcomes of 150 children who received an allogeneic HSCT between January 1999 and April 2007, in a pediatric university hospital in The Netherlands. Thirty-five of the 150 patients received mechanical ventilation on 38 occasions. None of the recorded risk factors was significantly associated with the requirement of mechanical ventilation. Sixteen admissions resulted in death in the intensive care unit (ICU), giving a case fata

Ventilators Currently Used in Emergency Neonatal Transport: Italian National Survey, Year 2025.

OBJECTIVE: Emergency neonatal transport is well established in many Western countries, with comprehensive frameworks and guidelines for the management of neonatal respiratory distress. The aim of this survey was to provide valuable insights into the ventilators currently used during neonatal transport in Italy. We hope that the results of this survey will help guide and inform future improvements in the quality of care provided during neonatal transport for newborns with respiratory distress. METHODS: Survey period: January 2025 (1-31). A simple questionnaire with 3 key questions was sent by
