Supporting families in their child's journey with neonatal encephalopathy and therapeutic hypothermia.

Neonates and families face challenges in hypothermic therapy, including trauma to parents, extreme emotions, and unfamiliarity with the medical system. Communication is an essential element to supporting parents while their children are in the NICU, and beyond, building the foundation for the ongoing relationship the family has with the medical system. Significant consideration needs to be given to the critical element of integrating the family into the care of a baby being treated with therapeutic hypothermia. Clinicians can promote healing of accumulated traumas of parents through ensuring p

Loneliness among older adults in Europe: time to integrate health and social care.

Loneliness is a major concern for public health in contemporary societies. Older adults are particularly vulnerable to loneliness because of their reduced social connections following life changes, such as retirement. Loneliness is often investigated and discussed together with social isolation. While social isolation implies an actual lack of social connections, loneliness implies a perceived deficit of social relations regardless of the real amount of social contacts. However, since living alone may lead to loneliness, individuals can experience both loneliness and social isolation together.

Clinicians' and patients' perspectives of a mobility technician role in the hospital to promote ambulation: A qualitative study.

BACKGROUND: Hospitalized older patients are predominantly sedentary, risking functional decline. Nurses and physical therapists are primarily responsible for mobility but have many competing demands. OBJECTIVES: To explore clinicians' and patients' perceptions of mobility technicians dedicated to support inpatient ambulation. METHODS: We conducted a qualitative framework analysis informed by Normalization Process Theory (NPT). The four NPT constructs guided the development of an initial analytic framework. Stakeholders involved in the ambulation program from one health system were eligible t

Physiotherapist- and peer-led physical activity counseling for people with spinal cord injury: a cross-sectional study of barriers and facilitators to implementation in the rehabilitation to community transition.

INTRODUCTION: A large decrease in physical activity (PA) is typically observed among people with spinal cord injury (SCI) upon discharge from in-hospital rehabilitation. Physiotherapists and SCI peers are well-positioned to intervene at this critical timepoint; however, the implementation of coordinated PA interventions between these two groups have yet to be studied. PURPOSE: To identify barriers and facilitators that affect the implementation of coordinated PA counseling among physiotherapists and SCI peers during the transition from in-hospital rehabilitation to the community. METHODS: Sem

Feasibility and preliminary effects of an integrated hospital-to-home transitional care intervention for older adults with stroke and multimorbidity: A study protocol.

BACKGROUND: Stroke is a major life-altering event and the leading cause of death and disability in Canada. Most older adults who have suffered a stroke will return home and require ongoing rehabilitation in the community. Transitioning from hospital to home is reportedly very stressful and challenging, particularly if stroke survivors have multiple chronic conditions. New interventions are needed to improve the quality of transitions from hospital to home for this vulnerable population. OBJECTIVES: The primary objective of this study is to examine the feasibility of implementing a new 6-month

Holistic care of complicated tuberculosis in healthcare settings with limited resources.

In recent years, most of the focus on improving the quality of paediatric care in low-income countries has been on improving primary care using the Integrated Management of Childhood Illness, and improving triage and emergency treatment in hospitals aimed at reducing deaths in the first 24 hours. There has been little attention paid to improving the quality of care for children with chronic or complex diseases. Children with complicated forms of tuberculosis (TB), including central nervous system and chronic pulmonary TB, provide examples of acute and chronic multisystem paediatric illnesses t

Etiology of new-onset jaundice: how often is it caused by idiosyncratic drug-induced liver injury in the United States?

BACKGROUND AND AIM: The epidemiology of acute drug-induced liver injury (DILI) in the United States has not been well studied. We conducted a study of adults with new-onset jaundice at a nonreferral community hospital to better understand the epidemiology of acute DILI. METHODS: This is a retrospective study of adult outpatients and inpatients (> or =18 yr) with new-onset jaundice over a 5-yr period (1999-2003) at Wishard Memorial Hospital, Indiana. Patients with new-onset jaundice were identified using our electronic medical record system and individual medical records were reviewed to extra

Positive predictive value of the AHRQ Patient Safety Indicator "Postoperative Sepsis": implications for practice and policy.

BACKGROUND: Patient Safety Indicator (PSI) 13, or "Postoperative Sepsis," of the Agency for Healthcare Quality and Research (AHRQ), was recently adopted as part of a composite measure of patient safety by the Centers for Medicare and Medicaid Services (CMS). We sought to examine its positive predictive value (PPV) by determining how well it identifies true cases of postoperative sepsis. STUDY DESIGN: Two retrospective cross-sectional studies of hospitalization records that met PSI 13 criteria were conducted, one within the Veterans Administration (VA) Hospitals from fiscal years (FY) 2003 to

'Genuine doctor care': Perspectives on general practice and community-based care of Australian men experiencing homelessness.

People with complex health and social needs, including tri-morbidity and homelessness, are challenging for modern healthcare systems. These clients have poor health and social outcomes. They tend to use available health resources inefficiently, with fragmented, uncoordinated use of multiple health and social care services. Increasing access for these clients to well-supported general practice care may be an effective response to these challenges. The aim of this study was to explore client experiences of, and attitudes to, community-based healthcare, and general practice in particular, to iden

Organisational culture and trust as influences over the implementation of equity-oriented policy in two South African case study hospitals.

BACKGROUND: This paper uses the concepts of organisational culture and organisational trust to explore the implementation of equity-oriented policies - the Uniform Patient Fee Schedule (UPFS) and Patients' Rights Charter (PRC) - in two South African district hospitals. It contributes to the small literatures on organisational culture and trust in low- and middle-income country health systems, and broader work on health systems' people-centeredness and "software". METHODS: The research entailed semi-structured interviews (Hospital A n = 115, Hospital B n = 80) with provincial, regional, distri

Risk of redocumenting penicillin allergy in a cohort of patients with negative penicillin skin tests.

BACKGROUND: Even though electronic documentation of allergies is critical to patient safety, inaccuracies in documentation can potentiate serious problems. Prior studies have not evaluated factors associated with redocumenting penicillin allergy in the medical record despite a proven tolerance with a penicillin skin test (PST). OBJECTIVE: Assess the prevalence of reinstating inaccurate allergy information and associated factors thereof. DESIGN: We conducted a retrospective observational study from August 1, 2012 to July 31, 2013 of patients who previously had a negative PST. We reviewed reco

Early warning and response systems for respiratory disease outbreaks: lessons learnt from cluster-associated cases of acute respiratory illnesses in Gilgil subcounty, Nakuru County, Kenya, 2021.

Investigating acute respiratory illnesses (ARIs) is difficult due to non-specific symptoms, varied health-seeking behaviors, and resource limitations; yet early detection is critical to global health security. Kenya's Ministry of Health (MOH) uses the Integrated Disease Surveillance strategy for public health surveillance, incorporating event-based surveillance (EBS) and indicator-based surveillance (IBS) for early warning system. MOH, supported by the US-CDC, established Influenza Sentinel Surveillance (ISS) in 2006 and later launched community EBS (CEBS) and health facility EBS (HEBS) pilots

Scale-up evaluation results of a multi-level approach using mailed immunochemical test (FIT) and patient navigation to increase colorectal cancer screening rates in rural clinic settings.

BACKGROUND: Colorectal cancer (CRC) screening effectively reduces CRC mortality and morbidity, yet screening rates remain below US national targets in rural and low-resourcedd settings. An effective intervention to improve screening is mailed fecal immunochemical tests (FIT) combined with patient navigation. However, these multicomponent interventions have been unevenly adopted across health systems and organizations, and studies of how to spread effective system-level approaches are limited. METHODS: We conducted a mixed-methods evaluation of a multi-component scale-up approach designed to i

Golden hour management in the patient with intraparenchymal cerebral hemorrhage: an Italian intersociety document.

BACKGROUND: Spontaneous intracerebral hemorrhage (ICH) accounts for 9-27% of all strokes worldwide and is associated with high mortality and disability. The main causes include vascular malformations, small- and large-vessel angiopathies, and coagulation disorders. Mortality rates reach approximately 40% at 1 month and 54% at 1 year, largely influenced by early management decisions. Rapid intervention, particularly within the first hour, is crucial, especially for patients initially treated in peripheral hospitals. This consensus document, developed by SIAARTI with the endorsement of multiple

Role of inpatient rehabilitation facility functional measures to predict community discharge after stroke.

BACKGROUND: This study investigated the association between stroke severity, functional status measured by the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI), and community discharge from IRF. OBJECTIVES: Aim one examined the association between National Institutes of Health Stroke Scale (NIHSS) scores measured during the acute care stay and IRF admission functional status, measured by the admission IRF-PAI self-care and mobility functional measures, to deduce if functional measures can serve as a proxy for stroke severity. Aim two investigated the ability of the NI
