Symptomatic medial humeral epicondylar fracture non-union- rare presentation of a relatively common injury.

BACKGROUND: Symptomatic non-union of medial humeral epicondylar fractures is a limited entity. Some studies recommend surgical excision of the fragment, but the results are controversial. The purpose of this study is to evaluate the outcome of open reduction and internal fixation of a medial epicondyle non-union fragment. MATERIALS AND METHODS: A retrospective study was conducted in all the patients, who were operated in our hospital between the year 2010 and 2015 for symptomatic medial humeral epicondyle non-union. Inclusion criteria were open reduction and internal fixation of symptomatic m

The bicipital groove as a landmark for reconstruction of complex proximal humeral fractures with hybrid double plate osteosynthesis.

BACKGROUND: Complex proximal humerus fractures with metaphyseal comminution remain challenging regarding reduction and stability. In most fracture patterns the hard bone of the bicipital groove remains intact. In this case series, we describe a novel technique of hybrid double plate osteosynthesis of complex proximal humerus fractures with metaphyseal comminution. METHODS: In randomly chosen shoulder specimens and synthetic bones, pilot studies for evaluation of the feasibility of the technique were performed. Between 4/2010 and 1/2012 10 patients underwent hybrid double plate osteosynthesis.

Management of humeral fracture nonunion in severe osteoporosis by a combination of locking plating and intramedullary fibular grafting.

Nonunion of the humerus in a severely osteoporotic bone is a likely event especially if the fracture is transverse. The management of such a combination is a challenge. Most of the conventional fixation methods are unlikely to succeed as the bone failure precedes implant failure in osteoporosis. The challenge is further compounded in severe osteoporosis when the cortical thickness is affected more severely. We used a combination of an intramedullary fibula with a locking plate in 5 cases. The results show that it may be a good combination in such situations as the bone strength is augmented an

The perfused, pulseless supracondylar humeral fracture: intermediate-term follow-up of vascular status and function.

BACKGROUND: This study provides intermediate-term follow-up data on the vascular status of a cohort of children with a perfused, pulseless supracondylar humeral fracture who were managed with closed reduction, percutaneous pinning, and observation. METHODS: Pediatric patients who sustained a perfused, pulseless supracondylar humeral fracture in the period from 2007 to 2011 and who had at least six months of clinical follow-up were identified and returned for evaluation. The primary outcome was vascular status as indicated by palpation of the radial pulse, the wrist brachial index, and arteria

Locking plate for proximal humeral fracture in the elderly population: serial change of neck shaft angle.

BACKGROUND: We conducted this radiographic study in the elderly population with proximal humeral fracture aiming to evaluate 1) the serial changes of neck-shaft angle after locking plate fixation and 2) find relationship between change in neck shaft angle and various factors such as age, fracture pattern, severity of osteoporosis, medial support and initial reduction angle. METHODS: Twenty-five patients who underwent surgical treatment for proximal humeral fracture with locking plate between September 2008 and August 2010 are included. True anteroposterior and axillary lateral radiographs wer

New technique of imaging and treatment of pediatric supracondylar humeral fracture without moving the injured limb.

The management of closed, displaced supracondylar fracture of humerus in children can be divided into 3 stages. In stage 1 closed reduction is achieved whose acceptability is confirmed, on table, in stage 2 using an image intensifier. This reduction is maintained in stage 3 by immobilization in slab/cast or by percutaneous Kirschner wire fixation. Although enough literature for proper technique in stage 1 and stage 3 are available, there are certain practical problems encountered in the equally important stage 2, which needs a closer look. Most of the C-arm compatible tables have their metalli

Life-threatening posterior circumflex humeral artery injury secondary to fracture-dislocation of the proximal humerus.

Proximal humeral fractures account for 4% to 5% of all fractures, and most of these are minimally displaced and less prone to neurovascular injuries. This article presents a case of a 65-year-old man who injured the posterior circumflex humeral artery following a fracture dislocation of the proximal humerus leading to a life-threatening hemorrhagic complication during surgical fixation of the dislocated proximal humeral fracture. Preoperative vital signs were normal. Using the deltopectoral approach, the fracture site was exposed and the dislocated head was extracted. Blood pooled and overflow…

Four-part fracture after intrathoracic displacement of the humeral head: a case report and review of the literature.

Displaced four-part fracture dislocation represents the highest risk of osteonecrosis of the humeral head due to a disruption of the blood supply. In a few cases, that dislocation can be intrathoracic which worsens prognosis with life-threatening injuries such as hemothorax or pneumothorax. This paper presents the case of a 77-year-old woman after having fallen down the stairs with an intrathoracic dislocation four-part fracture leaving the head in the thorax. We analyze mechanisms and review the literature in order to highlight this lesion and provide a take home message for the treatment.

Humeral shaft fracture sustained during arm wrestling: report on 30 cases and review of the literature.

Thirty cases of humeral shaft fracture sustained during arm wrestling were analyzed. Twenty eight were men and two were women. Eighteen were fractures of the right arm and 12 were of the left. The patients ranged in age from 15 to 60 years old, averaging 30 years. Although there was no prominent tendency for the injuries to occur at a certain phase of the match, they happened in all cases while full force was being exerted in an effort to win or to change the tide of the contest. All cases were spiral fractures, of which 23% presented with a medial butterfly fragment, and also 23% of which had

[Comparison of Blount's sling and Kirschner wire fixation in the treatment of a dislocated supracondylar humeral fracture in childhood].

From 1979 to 1982 we were treating 63 cases of displaced supracondylar fractures of the humerus. Patients underwent either closed reduction and fixation by collar and cuff immobilization (32 cases), or fixation by means of Kirschner wires following closed or (more often) open reduction (31 cases). Follow-up controls after an average time of three years clearly showed the more favourable results of the conservative method.

[Management of humeral shaft fractures in childhood and adolescence : Consensus report of the Pediatric Traumatology Section of the DGU].

Humeral shaft fractures are rare in childhood, accounting for less than 5% of all pediatric fractures. Indirect trauma predominates in younger children, whereas direct trauma is more common in older patients. To date, no uniform recommendations exist regarding diagnostic evaluation or treatment management. Therefore, during the 13th and 14th scientific meetings of the Pediatric Traumatology Section (SKT), the available evidence was reviewed, the management strategies were critically discussed within an expert panel and a consensus was formulated. The management remains predominantly conservativ
