OBJECTIVES: To evaluate the clinical significance of the grade of hydronephrosis and the tumor diameter in ureteral transitional cell carcinoma.
METHODS: From 1986 to 2004, the medical records of 104 patients who were treated with nephroureterectomy and diagnosed with ureteral transitional cell carcinoma were retrospectively reviewed. The correlation of the grade of hydronephrosis and tumor diameter with the pathologic T stage was investigated. Also evaluated were the effects of the grade of hydronephrosis and tumor diameter on the prognosis.

CA 19-9 is a widely used tumor marker. However, an elevation in serum CA 19-9 can occur in some patients with benign disorders such as cholecystolithiasis in the absence of tumor. We treated a case of acquired ureteral stone-induced giant hydronephrosis with markedly elevated serum CA 19-9 values. After nephrectomy, the serum CA 19-9 level returned to normal. No malignant cells were found in the tissues of the resected kidney. Localization of CA 19-9 was confirmed by immunohistochemical staining of the renal pelvic mucosa. A detailed case report is presented with a review of the literature.

Serum C-reactive protein levels and erythrocyte sedimentation rates were prospectively evaluated in 38 patients with dilated pyelocaliceal systems in an attempt to distinguish simple hydronephrosis from infected hydronephrosis and pyonephrosis. Analysis of a group of 276 patients with congenital hydronephrosis by dysfunction of pyeloureteral function shows that in the most cases the diagnosis has to be established by corroborating the data given by urography, echography, and isotopic renogram. In this episode of Mythbusters the premise that among infants with prenatally identified urinary tract dilation (UTD), voiding cystourethrogram (VCUG) should be performed in those with higher grade UTD but not in those with lower grade UTD is critically examined. It is concluded that severity of dilation is not strongly associated with risk of vesicoureteral reflux or other anomalies diagnosed by VCUG. Therefore, using hydronephrosis grade to decide which infants with UTD should undergo VCUG is not evidence based.
