To avoid the risks of reoperation, we treated 15 uremic patients on regular extracorporeal dialysis and affected by hyperparathyroidism recurring after subtotal parathyroidectomy with ultrasound-guided ethanol injection. Follow-up was extended to 12 months after the last injection and for 11 patients to 24 months. Plasma parathyroid hormone concentration, as measured with a carboxyterminal parathyroid hormone (c-PTH) radioimmunoassay (normal, 0.2 to 2 ng/mL), significantly decreased from a basal value of 19.29 +/- 14.73 ng/mL to 11.19 +/- 9.54 ng/mL at 1 month, 7.45 +/- 4.99 ng/mL at 6 months,…

Primary hyperparathyroidism in childhood is rare. Long-standing hypercalcaemia is reported to result in severe and irreversible renal damage. However, renal complications of hyperparathyroidism, particularly nephrocalcinosis are uncommon. We report a 12-year-old girl presenting after three years with extensive nephrocalcinosis and with rapid recovery of renal function.

Population-based investigations measuring serum calcium levels seem to indicate a prevalence of primary hyperparathyroidism (pHPT) of 0.4-1% in adults with a figure of about 3% in women over 60 years of age. Annual incidence rate varies and is found to be about 25-30 new cases per 100,000 adults with routine use of serum calcium measurements in diagnostic work-up. In women over 60 years of age the incidence approaches 200 instances per 100,000 individuals. Autopsy studies have demonstrated parathyroid disease in about 10%, one third as solitary adenomas and two thirds as hyperplasia. Annual in…

Magnetic resonance imaging is indicated for the localization of the abnormal gland or glands in patients with recurrent or persistent hyperparathyroidism. The accuracy of MRI is equivalent or superior to thallium subtraction scans and high-resolution ultrasonography for identifying the abnormal glands in the neck and superior to other noninvasive imaging techniques for recognizing ectopic glands in the thorax. Most glands have low to medium intensity on T1-weighted images and high intensity on T2-weighted images; however, glands with unusual signal intensity characteristics occur infrequently.…

Osteodystrophy is a common and sometimes debilitating complication of renal failure. Hyperparathyroidism plays a crucial role in the development of this condition. Significant morbidity is also incurred by the effects of calcium deposition in other tissues. We report a series of 27 patients undergoing parathyroidectomy between May 1988 and November 1989. All had biochemical, radiological and clinical evidence of hyperparathyroidism. Surgery was well tolerated leading to an improved quality of life and avoidance of the need for aluminium containing phosphate binders.

Immunohistochemical and ultrastructural studies were performed on cases with hyperparathyroidism. The relationship between histology and cell activity in hyperfunctioning parathyroid glands was studied. Furthermore, the synthesis-secretion process of parathyroid hormone (PTH), which has been more or less elucidated biochemically, was studies by a morphological means. The subjects employed in the present study were 23 cases of primary hyperparathyroidism (PHPT) and 31 cases of secondary hyperparathyroidism (SHPT). Based on the results of the immunohistochemical study using anti-PTH antibody, th…

Parathyroid hyperplasia is second only to adenoma as the most common cause of hyperparathyroidism. Symptoms are most likely to result from elevated serum calcium levels. Symptoms include nephrocalcinosis and nephrolithiasis. Studies that may help confirm a suspected diagnosis include ultrasonography, computed tomography, radionuclide scanning with thallium and/or technetium, and magnetic resonance imaging. Surgical treatment consists of excision of all but part of one parathyroid gland if all of the glands are enlarged.

It is generally accepted that some patients affected by mild asymptomatic primary hyperparathyroidism need not be treated with surgery, but may be medically managed without risk. However, our experience regarding 5 of these cases observed in the last two years, suggests a different approach. These patients, initially diagnosed as having mild hyperparathyroidism based on only moderately elevated serum concentrations of calcium and followed medically for years, were referred to us for a sudden worsening of their clinical course. One 35-year-old man presented hemorrhagic gastritis with severe ane…

Persistent or recurrent hyperparathyroidism after total parathyroidectomy with forearm implant may be caused by hyperplastic grafted tissue, residual parathyroid tissue left in the neck or the presence of a supernumerary gland not found during surgery. A correct assessment of graft function is needed to localize the source of hormone excess and to avoid an unnecessary neck reoperation in cases of graft dependent hyperparathyroidism. In 12 patients with relapsing hyperparathyroidism after total parathyroidectomy with forearm implant, total ischemic blockade of the arm bearing the parathyroid gr…

During recent years the total number of patients undergoing surgery for hyperparathyroidism has markedly increased, but the annual number of cases with substantial hypercalcemia has remained unchanged. Parathyroid carcinoma and water clear cell hyperplasia cause more severe hypercalcemia than other kinds of hyperparathyroidism. Grave hypercalcemia due to hyperparathyroidism is more common among the elderly, but can occur during pregnancy and also among children. Occasionally, a patient with hyperparathyroidism can also have another cause of the hypercalcemia and does not become normocalcemic u…

Two groups of patients with primary hyperparathyroidism, one early group surgically treated operated between 1970 and 1979 (n = 90), and the other late group from 1980 to 1984 were compared concerning signs, symptoms and laboratory parameters in order to find out the changing trends in the clinical spectrum of this disease during these years. The decade of manifestation decreased from the age of sixty to fifty. The most common mode of presentation in the early group were renal (30%), gastrointestinal (27%), and osseous (18%) symptoms of primary hyperparathyroidism, and 21% symptoms due to hype…

The pseudogout syndrome has been suggested to be a diagnostic clue of hyperparathyroidism or a complication after parathyroidectomy that abruptly relieves hypercalcemia. In this report of 20 patients who had pseudogout, eight had intermittent attacks of arthritis associated with chondrocalcinosis that led through the pseudogout syndrome to a confirmed diagnosis of primary hyperparathyroidism. In an additional 12 patients, the diagnosis of pseudogout was made in patients who suffered from acute arthritis after parathyroidectomy that relieved the primary hyperparathyroidism. The postoperative at…

Hypercalcemia caused by primary hyperparathyroidism was believed to be responsible for seizures in a dog. A diagnostic evaluation showed no primary causes of seizures. After surgical excision of the adenomatous parathyroid gland, phenobarbital treatment was discontinued, without recurrence of seizures.

Forty-seven reoperations for persistent or recurrent secondary hyperparathyroidism (HPT) in 29 patients are reported. The patients are part of a group of 181 patients operated on for secondary HPT between 1975 to 1986. Most reoperations were performed in the neck (26 operations in 17 patients). The main cause were remaining parathyroid glands in the mediastinum (9 patients), followed by normally localized glands in the neck, which had not been discovered during previous operations. In 7 patients a hyperplasia of the autografted tissue was the reason for recurrent disease. They were treated by …

Ultrasonography was used preoperatively in 21 patients with primary hyperparathyroidism in an effort to locate enlarged parathyroid glands. It indicated the presence of a solitary mass compatible with a parathyroid adenoma in 14 patients. At operation, adenomas were found at the location indicated by ultrasonography in 13 of the 14 patients (93 per cent). In seven patients, ultrasonography failed to reveal evidence of a mass of the parathyroid gland. At operation, three had diffuse parathyroid hyperplasia and four had a solitary adenoma. Of the four patients with adenomas undetected by ultraso…

Although a cautious approach is essential, there is still no safe and effective chronic medical therapeutic regimen that can substitute for surgical management of primary hyperparathyroidism in the elderly. The primary care physician should recognize that: the natural history of this disease is variable and unpredictable; symptomatic disease in the elderly patient may be easily overlooked; and therefore, older patients who have vague and varied nonspecific neuromuscular, GI, and constitutional complaints or acquired behavior disturbances should be carefully scrutinized. This approach to hyperp…

A 59 year-old patient had lumbago and pain in hip joints, knees, and ribs of long duration. Severe hypophosphatemia and high serum ionized c … cm, weighing 4.0 g was removed. The long standing phosphate depletion and hypophosphatemia, due to hyperparathyroidism causing renal damage with nephrocalcinosis and reduced synthesis of active vitamin D, and milk tolerance due to gastroduodenostomy were probably responsible for producing the clinical picture of normocalcemic hyperparathyroidism complicated with ost …

Controversy exists as to whether primary hyperparathyroidism is associated with an anaemia. Records corresponding to 215 patients operated on at the Karolinska Hospital between 1972 and 1981 were reviewed. An incidence of 7.4% of anaemia was found, but in only 3% of these cases the anaemia could not be explained by any other known cause. The anaemic group was characteristic in presenting a normochromic and normocytic picture, with iron and serum transferrin values below the normal range. No renal or osseous pathology was observed. In these patients, the anaemia disappeared after the parathyroi…

Data on the management of primary hyperparathyroidism during pregnancy are sparse and relatively few parathyroidectomies have been performed during pregnancy. Provided the diagnosis can be established with reasonable certainty, removal of a parathyroid adenoma should be undertaken during the second or at the beginning of the third trimester. Parathyroidectomy in the hands of a surgeon familiar with this operation carries few risks. The complication rate of untreated cases may reach 80 per cent and include spontaneous abortion, fetal death and neonatal tetany. We present two cases of pregnant w…
