BACKGROUND: Congenital hypopituitarism is an uncommon cause of hypophyseal insufficiency It is less common than growth hormone deficiency which has an incidence of 1:4.000 to 1:8.000 live newborns. Early diagnosis of this condition is important to prevent impairment of cognitive function, poor growth and alterations in metabolic profile in these patients.

Central diabetes insipidus secondary to hypophysitis in eosinophilic granulomatosis with polyangiitis (EGPA) is very rare, and this article summarizes one case reported from our site as well as two previously reported patients with EGPA, both of whom had central diabetes insipidus, suggesting that new, rare organ involvement (pituitary gland) may occur in the early stages of EGPA or years later. The aim is to improve our understanding of central diabetes insipidus caused by EGPA involving the pituitary gland and avoid misdiagnosis or missed diagnosis.

An ectopic functioning pituitary in the sphenoid is an extremely rare occurrence, and even rarer is pituitary adenoma causing symptoms of Nelson's syndrome. A case is presented of a young female diagnosed and treated in our clinic. The only functioning hypophyseal tissue was detected inside the sphenoid, as the pituitary gland had been radiated because of Cushing's syndrome 10 years before and imaging studies revealed an empty sella.

Function of the hypothalamo-hypophyseal-adrenal system was investigated in 23 patients with different types of the lactorrhea-amenorrhea syndrome. The basal levels and "stress secretion" of ACTH, cortisol and cyclic nucleotides, the circadian rhythm of ACTH and cortisol levels were determined by RIA. Noticeable disorders in the central and peripheral factors of this system and changes in the blood levels of cyclic nucleotides were observed in patients with the lactorrhea-amenorrhea syndrome.

A man aged 31 with remittent fever, leukopenia, polyuria, and splenomegaly in addition to chronic and recurrent episodes of neurologic abnormalities was examined pathologically. A chronic type of perivenous encephalomyelitis was found in the CNS, and chronic inflammatory lesions were noticed in the infundibulo-hypophyseal system and also in the peripheral nerves. Moreover, hemophagocytic lymphohistiocytosis predominated in the reticuloendothelial system. This seemed to be the first adult case of demyelinating disease with hemophagocytic lymphohistiocytosis.
