We describe a rare case of a 31-year-old woman at 28 weeks of pregnancy presenting with an incarcerated pedunculated fibroid in an umbilical hernia sac. She had a successful myomectomy and hernia repair and proceeded to have spontaneous vaginal delivery at term. Incarceration of a pedunculated fibroid presents a diagnostic puzzle which can be successfully treated by myomectomy.

This study presents the results of a needs assessment survey conducted with 66 incarcerated women in a large Philadelphia county jail during the winter of 1993. Over a 6-year period, 14 infants with a total of 16 inguinal hernias (IH) underwent transperitoneal closure of the internal ring (TPIR). This was performed through a minilaparotomy, using a purse-string suture placed around the internal ring from within. We report a case of incarceration of the right vertebral and basilar arteries in a longitudinal fracture of the clivus. Discussed is a patient with both a mechanical and paralytic ileus as a result of a gangrenous appendicitis with perforation and a circumscribed peritoneum and incarceration of an inguinal hernia on the opposite side. The patient's heart too was suffering a disease. The postoperative course was difficult. It was complicated by pleural hemorrhaging and by lung infiltrations because of embolisms. With an emergency operation involving the entire team, the patient was saved. Although the pathology is rare, the patient was saved.
