Low birth weight (LBW) infants have lower infant mortality in groups in which LBW is most frequent. For example, in 1991, US infants born to smokers had higher risks of both LBW and infant mortality than infants born to nonsmokers. However, among LBW infants, infant mortality was lower for infants born to smokers (relative rate = 0.79). There are competing theories regarding this so-called "paradox." One is that maternal smoking is beneficial for LBW infants. The authors use causal diagrams to show that, even in the absence of any beneficial effect of smoking, an inverse association due to str…

A study of neonatal mortality in Meerut district revealed an infant mortality rate of 50.1 per 1000 live births. Neonatal mortality accounted for 37.8% of infant mortality with a neonatal mortality rate of 19.0 per 1000 live births. 90.5% of these neonates were delivered at home largely by untrained personnel (57.2%). Only 28.6% of these neonates were treated by qualified doctors and only 30.9% of their mothers were fully immunized against tetanus. At least 2/3rd of neonatal mortality was due to exogenous factors with tetanus neonatorum and septicaemia being the principal causes of mortality e…

In the Pittsburgh region, as elsewhere, the Black infant mortality rate, reflecting overall health disparities, is higher than the rate for White babies. This study suggests that the answer to the infant mortality racial disparity question may lie partially in the characteristics and availability of prenatal care as well as broader changes in the social and economic environment. Numerous known risk factors associated with infant mortality have been present for as long as statistics have been recorded. Only wide ranging comprehensive programs will combat such factors as low income, education, t…

Singapore's infant mortality has fallen drastically from a high of 131 per thousand births in 1931 to 56 in 1954 and a low of 7.4 in 1987. Postneonatal mortality used to account for many of the infant deaths but since the 1960s, neonatal mortality has become increasingly important. It now accounts for about three quarters of all infant deaths. This study of neonatal deaths in 1987 showed that sex, birth order, maternal age and social class in terms of possible contributory factors has not changed much over a thirteen year period. The important causes of neonatal deaths being congenital anomali…

BACKGROUND: Maternal postpartum vitamin A supplementation (VAS) provides an opportunity to improve vitamin A nutriture of breast fed infants in developing countries and can possibly prevent infant mortality and morbidity attributable to vitamin A deficiency.

DESIGN: Systematic review, meta-analysis and meta-regression of randomized controlled trials.
DATA SOURCES: Electronic databases and abstracts and proceedings of micronutrient conferences.

BACKGROUND: The potential to reduce child mortality by preventing folic acid-preventable spina bifida and anencephaly (FAP SBA) is inadequately appreciated. To quantify possible reduction in FAP SBA-associated child mortality in low- and middle-income countries, we conducted an analysis to demonstrate in India, a country with more than 25 million births and 1.2 million under-five deaths each year, the decrease in neonatal, infant, and under-five mortality that would occur through total prevention of FAP SBA.
METHODS: We estimated the percent reductions in neonatal, infant, and under-five mort

OBJECTIVE: To compare fetal/infant mortality risk associated with each additional week of expectant management with the infant mortality risk of immediate delivery in growth-restricted pregnancies.
METHODS: A retrospective cohort study was conducted of singleton, nonanomalous pregnancies from the 2005-2008 California Birth Registry comparing pregnancies affected and unaffected by growth restriction, defined using birth weights as a proxy for fetal growth restriction (FGR). Birth weights were subdivided as greater than the 90th percentile, between the 10th percentile and 90th percentile, and l…

OBJECTIVE: To examine the association between birth weight percentile and severe infant outcomes according to gestational age category.
METHODS: We conducted a population-based retrospective cohort study using publicly available U.S. birth certificate data linked to infant death data from 2017 to 2019. Maternal-neonate pairs of singleton live births between 28 0/7 and 41 6/7 weeks of gestation (vaginal or cesarean delivery) were evaluated. We excluded infants with major fetal anomalies, chromosomal disorders, missing data, and birth weight outliers. The primary outcome was infant mortality wi…

All births and infant deaths in 1985 87 and 1992 94 in Canada, except in Ontario and Newfoundland, were analyzed to assess the potential impact of the recent increased use of elective labour induction for post-term pregnancies. The combined fetal and infant mortality declined by 20 30% between 1985 87 and 1992 94 at each gestational week beginning at 37 weeks, with no increased reduction among post-term pre…

Perinatal deaths (fetal or infant deaths from the 28th week of pregnancy up to the seventh day after birth) occur as a result of adverse conditions during pregnancy, labor, and delivery, or in the first few days of life. Placental malaria infection is known to increase the risk of delivery of a low birth weight infant, thus, potentially increasing the risk of perinatal and infant mortality. To better understand the relationship among the adverse events in pregnancy, including placental malaria infection, adverse conditions in labor, and birth weight to perinatal mortality, we investigated the …

OBJECTIVE: To evaluate mortality on the first day of life by minute and hour, and examine changes in major causes of death in the past three decades.

A new low in the infant mortality rate was reached in 1992, at 848.7 deaths per 100,000 live births, a decline of 5% from 894.4 in 1991. Birth, death, and marriage rates were also lower, but the divorce rate inched up to 4.8 per 1000 population, the same level as in 1988. The age-adjusted death rate was 504.9 per 100,000 population, the lowest in US history. Natural increase in the population, excess of births over deaths, decreased from 1,941,389 to 1,907,000, from 7.7 to 7.5 per 1000 population. Births outside hospital were fewer, both in numbers and in proportion to all births. Birth rates …

BACKGROUND: Sudden infant death syndrome (SIDS) makes up the largest component of sudden unexpected infant death in the United States. Since the first recommendations for supine placement of infants to prevent SIDS in 1992, SIDS postneonatal mortality rates declined 55% between 1992 and 2001.
OBJECTIVE: The objective of this analysis was to examine changes in postneonatal mortality rates from 1992 to 2001 to determine if the decline in SIDS was due in part to a shift in certification of deaths from SIDS to other causes of sudden unexpected infant death. In addition, the analysis reviews the c
