Impact of fluconazole prophylaxis on incidence and outcome of invasive candidiasis in a neonatal intensive care unit.

OBJECTIVES: We assessed the impact of intravenous fluconazole prophylaxis (FP) in extremely low birth weight (ELBW [<1000 g]) infants on the incidence of and outcome from invasive candidiasis (IC) in all infants admitted to a neonatal intensive care unit (NICU). STUDY DESIGN: Beginning April 1, 2002, FP was given to ELBW infants aged < 5 days admitted to the NICU of Woman's Hospital of Texas. Infants in NICU in whom IC developed during the first 2 years of FP (FP period) were compared with those with IC during 2000-2001. RESULTS: During 2000-2001 and the FP period, the incidence of IC in ELB

Pharmacokinetics of propofol infusions in critically ill neonates, infants, and children in an intensive care unit.

BACKGROUND: Propofol is a commonly used anesthetic induction agent in pediatric anesthesia that, until recently, was used with caution as an intravenous infusion agent for sedation in pediatric intensive care. Few data have described propofol kinetics in critically ill children. METHODS: Twenty-one critically ill ventilated children aged 1 week to 12 yr were sedated with 4-6 mg. kg(-1).h(-1) of 2% propofol for up to 28 h, combined with a constant morphine infusion. Whole blood concentration of propofol was measured at steady state and for 24 h after infusion using high-performance liquid chro

An outbreak of necrotizing enterocolitis associated with a novel clostridium species in a neonatal intensive care unit.

An outbreak of necrotizing enterocolitis (NEC) occurred in 6 neonates within a 2-month period. Blood cultures from 3 of these neonates grew the same strain of what appears to be a novel clostridial species for which the name "Clostridium neonatale" has been proposed. A point-prevalence survey that used rectal swabs was performed in our intensive-care and intermediate-care nurseries, and it indicated that 20.8% of neonates carried this same "C. neonatale" strain despite having no evidence of NEC. In conclusion, we describe an outbreak of NEC associated with the novel species, and we suggest tha

Evaluation of the Cerebral Function Monitor as a tool for neurophysiological surveillance in neonatal intensive care patients.

BACKGROUND: Cerebral function in critically ill infants is difficult to assess and would certainly require continuous monitoring. Therefore, this study was performed to evaluate the Cerebral Function Monitor (CFM) as a tool for continuous neurophysiological surveillance in the Neonatal Intensive Care Unit (NICU). PATIENTS: A total of 40 neurological risk neonates were included in the study. They were classified on the basis of their primary diagnoses as infants with clinically manifest seizures, suspected seizure activity, intracranial hemorrhage (ICH) and hypoxic-ischemic encephalopathy (HIE

Hypothyroxinemia in sick term neonates and its risk factors in an extramural neonatal intensive care unit: a prospective cohort study.

OBJECTIVE: Thyroid functions in the sick newborns may be altered in the first week of life. Transient hypothyroxinemia has been reported in these babies, which could be due to the immaturity of the hypothalamic-pituitary-thyroid axis or to acute illness. We conducted this study to estimate the incidence of hypothyroxinemia and determine its risk factors in sick term newborns. MATERIALS AND METHODS: We analyzed free T4 (FT4) and thyroid-stimulating hormone (TSH) levels in sick term neonates (≤7 days of life) admitted to the neonatal intensive care unit. FT4 and TSH levels were estimated in the

Therapeutic hypothermia in neonates. Review of current clinical data, ILCOR recommendations and suggestions for implementation in neonatal intensive care units.

Recent evidence suggests that the current ILCOR guidelines regarding hypothermia for the treatment of neonatal encephalopathy need urgent revision. In 2005 when the current ILCOR guidelines were finalised one large (CoolCap trial, n=235) and one small RCT (n=67), in addition to pilot trials, had been published, and demonstrated that therapeutic hypothermia after perinatal asphyxia was safe. The CoolCap trial showed a borderline overall effect on death and disability at 18 months of age, but significant improvement in a large subset of infants with less severe electroencephalographic changes. Surfactant replacement therapy in neonates less than 32 weeks gestation: effect on neonatal intensive care resource utilization.

The effect of synthetic surfactant (Exosurf) replacement on complications from hyaline membrane disease (HMD) in infants < 32 weeks gestation and their resource utilization within a neonatal intensive care unit was studied in 1991-92. A control group was selected from infants admitted to the same unit during the preceding 3 years when Exosurf was not available. The infants were controlled for gestation, weight and severity of HMD. Infants given Exosurf had a significant reduction in the incidence of pulmonary interstitial emphysema (PIE), and a marginal decrease in the incidence of pneumothora

Critical Factors Influencing the Uptake of Breastfeeding Support Interventions in Neonatal Intensive Care Units: A Scoping Review.

Background: Breastfeeding is considered the optimal source of nutrition for infants admitted to Neonatal Intensive Care Units (NICUs). Despite these well-documented benefits, establishing and sustaining breastfeeding in NICU settings remains challenging due to inadequate uptake of breastfeeding support measures. This scoping review aimed to examine the evidence on factors influencing the uptake of breastfeeding support practices in NICUs. Methods: The search was undertaken across four electronic databases: PubMed, MEDLINE, CINAHL, and the Cochrane Library. Primary studies published in English

Artificial Womb Technology: A Systematic Review of Preclinical Evidence and Implications for Neonatal Viability and Intensive Care.

Artificial womb technology (AWT), also known as artificial placenta or ex-vivo uterine environment (EVE) therapy, is designed to mimic intrauterine conditions and support fetal physiology in extremely preterm neonates. Despite significant advances in large-animal artificial womb studies, there remains a critical gap in synthesizing how these findings translate to questions of viability, ethical governance, and regulatory readiness, making it unclear what evidence currently supports the progression toward human clinical trials. A systematic review of MEDLINE/PubMed, Embase, and Scopus using ter

Application of next-generation sequencing to investigate a Serratia marcescens outbreak in a neonatal intensive care unit.

INTRODUCTION: Nosocomial infections in Neonatal Intensive Care Units (NICU) are a major concern due to the vulnerability of premature and immunocompromised infants. Serratia marcescens is an opportunistic pathogen often involved in these infections, contributing significantly to morbidity and mortality. Integrating Next-Generation Sequencing (NGS) into infection control programs can enhance detection, surveillance, and prevention efforts. This study aimed to develop a mapping-based pipeline for strain typing and phylogenetic analysis of nosocomial infections, enabling detailed comparison of mi

[Early management of preterm infants in neonatal intensive care units (excluding respiratory diseases and ulcerative necrotizing enterocolitis)].

Preterm neonates present multiple challenges due to organ immaturity.Cardiovascular:Patent ductus arteriosus (PDA) is common in lower gestational ages and may cause respiratory and circulatory complications. Treatment includes medical (ibuprofen, para-cetamol), surgical, or percutaneous approaches. Bradycardia is frequent and requires continuous monitoring.Digestive/Metabolic:Early enteral nutrition with fortified breast milk is preferred; parenteral nutrition is often required. Gastroesophageal reflux is usually benign. Preventing metabolic bone disease necessitates adequate calcium and phosp
