UNLABELLED: Takotsubo syndrome (TTS) is rarely diagnosed following valvular and nonvalvular cardiac surgery. Only several such cases, including 12 after mitral valve replacement (MVR) or plasty (MVP) and 2 after coronary artery bypass grafting (CABG) have been reported so far.
A CASE REPORT: The authors describe a case of a 75-year-old female in whom TTS occurred on the first postoperative day after elective combined surgery: MVR (with bioprosthesis) and CABG (of the left anterior descending coronary artery). Echocardiography revealed left ventricular (LV) dysfunction in the form of apical ba

Thanks to advancements in percutaneous cardiac interventions, an expanding patient population now qualifies for treatment through percutaneous endovascular procedures. High-risk interventions far exceed coronary interventions and include transcatheter aortic valve replacement, endovascular management of acute pulmonary embolism and ventricular tachycardia ablation. Given the frequent impairment of ventricular function in these patients, frequently deteriorating during percutaneous interventions, it is hypothesized that mechanical ventricular support may improve periprocedural survival and subs

BACKGROUND: Mortality from cardiogenic shock remains high and early recognition and risk stratification are mandatory for optimal patient allocation and to guide treatment strategy. The CardShock and the Intra-Aortic Balloon Counterpulsation in Acute Myocardial Infarction Complicated by Cardiogenic Shock (IABP-SHOCK II) risk scores have shown good results in predicting short-term mortality in cardiogenic shock. However, to date, they have not been compared in a large cohort of ischaemic and non-ischaemic real-world cardiogenic shock patients.
METHODS: The Red-Shock is a multicentre cohort of

BACKGROUND: Shock is a common complication of acute myocardial infarction and cardiac arrest. Internal mammary artery (IMA) injury is a rare cause of post-cardiopulmonary resuscitation (CPR) hemorrhagic shock.
CASE SUMMARY: An 80-year-old woman presenting with inferior ST-segment elevation myocardial infarction underwent percutaneous coronary intervention to the left circumflex artery with intra-aortic balloon pump support after cardiac arrest treated with mechanical CPR. Persistent hypotension prompted computed tomography angiography, revealing a mediastinal hematoma with active right IMA ex

Parallel venovenous (VV) and venoarterial (VA) extracorporeal membrane oxygenation (ECMO) can allow separate titration of respiratory and circulatory support in patients with concurrent severe cardiopulmonary failure. A 64-year-old woman with central hypopituitarism developed cytomegalovirus infection with hemophagocytic lymphohistiocytosis, followed by severe Pseudomonas aeruginosa pneumonia and septic cardiomyopathy. Despite VV-ECMO and intra-aortic balloon pump support, refractory cardiogenic shock required the addition of VA-ECMO. After parallel ECMO was initiated, hemodynamics improved ra

BACKGROUND: Electrical storm in Chagas disease represents a life-threatening scenario, particularly in Latin America.
CASE SUMMARY: A 62-year-old man developed recurrent ventricular tachycardia a decade after implantable cardioverter-defibrillator placement for Chagas cardiomyopathy. Despite optimized therapy, arrhythmia control was not achieved, culminating in refractory electrical storm. He underwent heart transplantation. Severe primary graft dysfunction developed intraoperatively. Peripheral venoarterial extracorporeal membrane oxygenation was initiated, unloading with an intra-aortic bal

A healthy man in his 40s presented with acute anterolateral ST-segment elevation myocardial infarction after sudden-onset chest pain during a coughing episode. Coronary angiography and intravascular ultrasound revealed extensive spontaneous coronary artery dissection (SCAD) of the left anterior descending artery with Thrombolysis in Myocardial Infarction II distal flow. He was managed conservatively, but his course was complicated by severe left ventricular systolic dysfunction, apical thrombus and a ventricular tachycardia storm leading to cardiogenic shock. He required repeated emergency dir

Acute myocardial infarction complicated by cardiogenic shock (AMI-CS) is a heterogeneous clinical syndrome associated with substantial morbidity and mortality. We developed a machine learning-based mortality model to identify features and patient subgroups associated with the largest change in mortality risk when evaluating treatment with Impella devices or intra-aortic Balloon Pump (IABP). Our cohort comprised 369 sites and 15,796 patient visits to the cardiac catheterization laboratory from the National Cardiovascular Data Registry. The estimated population mean excess mortality effect of tr

Extracorporeal membrane oxygenation (ECMO) delivers powerful mechanical circulatory support while simultaneously offering respiratory support; however, it can increase afterload and is associated with potential device-related vascular complications. To date, several randomized controlled trials have failed to demonstrate a prognostic benefit of routine use of ECMO in patients with cardiogenic shock secondary to acute myocardial infarction or in those with out-of-hospital cardiac arrest. Therefore, the routine use of ECMO is not a guideline-recommended therapeutic strategy. However, in real-wor

BACKGROUND: Cardiac T-cell lymphoma is a rare and aggressive malignancy that often presents with nonspecific symptoms and is difficult to diagnose early.
CASE SUMMARY: A 73-year-old man presented with syncope, anorexia, and abdominal discomfort. During hospitalization, he developed hypereosinophilia and rapidly progressed to fulminant myocarditis. Despite steroid and immunoglobulin therapy, cardiac function declined, requiring extracorporeal membrane oxygenation and intra-aortic balloon pump support. Myocardial biopsy confirmed cardiac T-cell lymphoma with lymphocytic infiltration.

The combination of the Impella and peripheral venoarterial extracorporeal membrane oxygenation (Ecmella) is a promising treatment for critically ill patients. We report a single-access Ecmella approach using the brachiocephalic artery. A 65-year-old woman with acute myocardial infarction involving the left main coronary artery underwent intra-aortic balloon pump and peripheral venoarterial extracorporeal membrane oxygenation (VA-ECMO) placement. Given the progression of pulmonary congestion and lower limb ischaemia, and an axillary artery diameter of 4.9 mm, we decided to perform a single Ecme

Surgical approaches for mitral valve exploration vary, with the preferred approach being via the interatrial groove. Data on whether the interatrial groove approach can be used for mitral valve surgery in patients with a previously implanted Amplatzer septal occluder (ASO) (St Jude Medical, an Abbott company) are extremely limited. The authors performed mitral valve surgery using the transatrial septal approach on 2 patients following explantation of an ASO, which significantly impedes the interatrial groove approach. Concomitant surgical procedures with mitral valve replacement, removal of th

Enhanced External Counterpulsation (EECP) is a non-invasive FDA approved therapy for patients with refractory angina pectoris. The EECP mechanism of action is similar to that of an intra-aortic balloon pump (IABP) by administering a vigorous pressure pulse via external blood pressure cuffs during the diastole. The benefit of EECP includes improvement in angina severity, angina stability, maximal walking capacity and generalized improvement in overall health. Seatle Angina Questionnaire (SAQ) is a valid, reliable and sensitive measure of health-related quality of life. It is also a sensitive an

Cardiogenic Shock (CS) is a major challenge in current cardiology. Over the last decade, cardiogenic shock mortality has decreased somewhat, but it still remains high, particularly when associated with ischaemic heart disease. The challenges are numerous and include prevention, accurate diagnosis, prompt management and effective therapies to support a failing heart and prevent multi-organ failure. Despite improvements in the care of Acute Coronary Syndrome (ACS), it remains the most common cause of CS. In addition to existing medical therapy, mechanical circulatory support has been proposed fo

This article gives an update on the management of acute ST-segment elevation myocardial infarction (STEMI) according to the recently released European Society of Cardiology guidelines 2017 and the modifications are compared to the previous STEMI guidelines from 2012. Primary percutaneous coronary intervention (PCI) remains the preferred reperfusion strategy. New guideline recommendations relate to the access site with a clear preference for the radial artery, use of drug-eluting stents over bare metal stents, complete revascularization during the index hospitalization, and avoidance of routine

Anomalous origin of the right coronary artery (AORCA) is a rare congenital anomaly with an incidence of 0.92% during routine cardiac catheterization. Its presence raises an important concern to the anaesthesiologist because it can lead on to myocardial ischaemia manifesting as either angina pectoris or myocardial infarction, or sudden death in young patients with minimal exertion, even in the absence of atherosclerosis. Patients with AORCA may be intolerant to stress and the high cardiac output condition owing to volume loading. Such a therapeutic manoeuvre may be desirable during renal transp

The intraaortic balloon pump (IABP) provides counterpulsation by displacing a 40-50 cc blood volume during diastole augmenting diastolic pressure. The rapid deflation of the balloon timed to the initiation of systole reduces the afterload of ventricular ejection and thus peak systolic pressure. As a direct result, IABP increases mean arterial pressure (MAP) and peak diastolic pressure while reducing systolic pressure and myocardial work. IABP increases coronary flow velocity in non-obstructed vessels, but does not increase flow across a severe obstruction as shown by intracoronary Doppler flow
