Exosomes of male reproduction.

Exosomes are nanosized membrane vesicles secreted by wide variety of cells and found in abundance in biological fluids including semen. They contain cargo of lipids, proteins, microRNAs and mRNAs, and are known to play a major role in intracellular communication. Seminal exosomes mainly include epididymosomes and prostasomes. Most of the proteins associated with the epididymosomes are transferred to the sperm subcellular or membranous domains during their epididymal transit and are involved in the acquisition of fertilizing ability, modulation of motility and protection against oxidative stres

Exercise-Induced Abdominal Wall Muscle Injury Resulting in Rhabdomyolysis and Mimicking an Acute Abdomen.

BACKGROUND: Rhabdomyolysis is characterized by muscle necrosis and release of intracellular constituents, causing muscle pain, weakness, and myoglobinuria. This can be attributed to muscle injury after strenuous exercise. If the abdominal wall is involved, clinical presentation may resemble an acute abdomen. CASE: A 27-year-old woman, gravida 4 para 2, presented with swelling and pain of the mons pubis and abdominal pain after intense powerlifting 2 days prior. A computed tomography scan was performed, revealing abdominal wall inflammation. Although myoglobinuria was absent, there was high su

Aquaporins in Brain Edema and Neuropathological Conditions.

The aquaporin (AQP) family of water channels are a group of small, membrane-spanning proteins that are vital for the rapid transport of water across the plasma membrane. These proteins are widely expressed, from tissues such as the renal epithelium and erythrocytes to the various cells of the central nervous system. This review will elucidate the basic structure and distribution of aquaporins and discuss the role of aquaporins in various neuropathologies. AQP1 and AQP4, the two primary aquaporin molecules of the central nervous system, regulate brain water and CSF movement and contribute to cy

Primary glycogen-rich clear cell squamous cell carcinoma of the mandibular gingiva.

Clear cell squamous cell carcinoma (CCSCC) is a rare variant of squamous cell carcinoma, first reported by Kuo, who described 6 cases of squamous cell carcinoma of the skin of the head and neck. CCSCC is composed of cells with clear cytoplasm, which Kuo attributed to the accumulation of intracellular fluid and not the presence of glycogen, lipid, or mucin. This case describes a 59-year-old woman who presented with an exophytic, hemorrhagic lesion on the posterior mandibular gingiva of 2 months' duration. Histologic examination revealed dysplastic stratified squamous epithelium showing transiti…

Extracellular alveolar proteasome: possible role in lung injury and repair.

The ubiquitin-proteasome system is the major intracellular pathway for protein degradation in eukaryotes, and it also generates oligopeptides for antigen presentation. However, the 20S proteasome is also associated with the cell's outer membrane, and observations indicate its physiologic presence and biological activity in the extracellular alveolar space (i.e., in the epithelial lining fluid). Furthermore, its concentration is increased in the adult respiratory distress syndrome, acute lung injury, and other inflammatory lung disease. While its cellular origin, potential extracellular biologi

Melioidosis in Malaysia.

Melioidosis is an important cause of sepsis in the tropics, is caused by an environmental saprophyte--B. pseudomallei. It affects mainly adults with underlying predisposing condition such as diabetes. The range of symptoms varies from benign and localized abscesses, to severe community-acquired pneumonia to acute fulminating septicaemia with multiple abscesses often leading to death. B. pseudomallei is an intracellular pathogen and some of the virulence mechanisms that govern the complex interaction between the organism and the host have been elucidated. Isolation of B. pseudomallei from bodil

Mycobacterium avium/Mycobacterium intracellulare Complex-Associated Arthritis Masquerading as a Seronegative Rheumatoid Arthritis.

Mycobacterium avium/Mycobacterium intracellulare[cf1] complex (MAC) is a rare cause of tenosynovitis. We describe a case in which a previously healthy patient developed bilaterally symmetric synovitis and was treated with disease modifying anti rheumatic drugs (DMARDs); the synovitis progressed into a bilaterally symmetric chronic granulomatous tenosynovitis that was uncontrollable despite a synovectomy and DMARD therapy. Three years after developing symptoms, and 2 1/2 years after her diagnosis of rheumatoid arthritis, MAC was cultured from synovial fluid. All DMARDS were stopped, and the pat

Molecular cloning of a putative Na(+)-K(+)-2Cl- cotransporter from the Malpighian tubules of the tobacco hornworm, Manduca sexta.

Fluid secretion in insects is controlled, in part, by diuretic hormones which bind to receptors on the surface of Malpighian tubule cells, leading to an increase in intracellular cAMP. Recent evidence suggests that Na(+)-K(+)-2Cl- cotransporters may be involved, upon stimulus by diuretic hormones, in transport of ions from the hemolymph to the lumen of the Malpighian tubules. In this report, I describe the cloning of a putative bumetanide sensitive Na(+)-K(+)-2Cl- cotransporter from the Malpighian tubules of the tobacco hornworm, Manduca sexta. This Na(+)-K(+)-2Cl- cotransporter is a Malpighia

Lipoprotein-incorporated pregnenolone fatty acid esters act as substrate for ovarian progestin synthesis.

Progesterone is of great importance in the normal development of the ovarian follicle and its biosynthesis has been shown to rely on plasma low-density lipoproteins for delivery of cholesterol substrate. However, the pre-ovulatory ovarian follicle has no access to plasma low-density lipoproteins suggesting that other substrates may be involved in pre-ovulatory progestin production. We show in this study using tritiated pregnenolone fatty acid esters incorporated into lipoproteins can give rise to a series of labelled progestins following lipoprotein-mediated internalization. In doing so, proge

Patients with Alzheimer's disease have an increased removal rate of soluble beta-amyloid-42.

Senile plaques, which are mostly composed of beta-amyloid peptide, are the main signature of Alzheimer's disease (AD). Two main forms of beta-amyloid in humans are 40 and 42-amino acid, long; the latter is considered more relevant to AD etiology. The concentration of soluble beta-amyloid-42 (Aβ42) in cerebrospinal fluid (CSF-Aβ42) and the density of amyloid depositions have a strong negative correlation. However, AD patients have lower CSF-Aβ42 levels compared to individuals with normal cognition (NC), even after accounting for this correlation. The goal of this study was to infer deviations o

Management of listeriosis.

Determination of the MIC in vitro is often used as the basis for predicting the clinical efficacy of antibiotics. Listeriae are uniformly susceptible in vitro to most common antibiotics except cephalosporins and fosfomycin. However, the clinical outcome is poor. This is partially because listeriae are refractory to the bactericidal mechanisms of many antibiotics, especially to ampicillin-amoxicillin, which still is regarded as the drug of choice. A true synergism can be achieved by adding gentamicin. Another point is that listeriae are able to reside and multiply within host cells, e.g., macro

Water and electrolyte homeostasis during decongestion in heart failure.

In heart failure, sodium retention generally occurs out of proportion to water retention and may occur disproportionate to chloride retention, requiring excretion of other cations (i.e. potassium, magnesium, and hydrogen). Renal homeostatic mechanisms keep chloride levels in balance, making it the primary regulator of intravascular tonicity. Excess total body sodium stores and potassium depletion increase the vulnerability to dehydration, especially in the intracellular compartment. With decompensation, extracellular volume expansion with sodium and chloride overload occurs, further enhancing

Neurotoxins and neurodegenerative disorders.

There is a hypothesis that excitotoxic pathomechanisms underlie neuronal tissue degeneration in neurological disorders. It is proposed that target neurons are overexcited, the result being energy disturbance and pathobiochemical changes that culminate in nerve cell death. Domoic acid (structural analogue of kainic acid) and beta-N-oxalylamino-L-alanine (BOAA) are powerful excitotoxins. Diminished energy supplies may result either in excessive release of glutamate or an inability of the neuron to restore ionic balance after stimulation. Impairment of intracellular energy metabolism induced by c

Facile fabrication of antioxidative and antibacterial hydrogel films to accelerate infected diabetic wound healing.

Diabetic wounds are accompanied with hemorrhage, excessive reactive oxygen species accumulation, microbial infection, and poor exudate management. Current dressings are insufficient to modulate the complex pathophysiological microenvironment of infected diabetic wounds. Here, we develop a facile-fabricated hydrogel film with potent antibacterial and antioxidative properties to significantly accelerate infected diabetic wounds healing. The hydrogel film demonstrates potent antibacterial efficacy against methicillin-resistant Staphylococcus aureus and Escherichia coli, efficient scavenging of th

A Case of Anti-GAD 65 Autoimmune Encephalitis Associated with Focal Segmental Stiff-Person Syndrome.

Glutamic acid decarboxylase (GAD) antibody-related encephalitis is an autoimmune disease associated with intracellular neuronal antigens. We report on a rare case of GAD antibody-associated encephalitis complicated with focal segmental stiffness-person syndrome (SPS) in a middle-aged woman. The disease course lasted for >10 years, initially presenting with drug-resistant epilepsy, followed by stiffness of the right lower limb, and right upper limb involvement. The patient experienced anxiety and depression symptoms due to long-term illness. During hospitalization, serum and cerebrospinal fluid

Concurrent Ocular and Cerebral Toxoplasmosis in a Liver Transplant Patient Treated with Anti-CD40 Monoclonal Antibody.

Toxoplasma gondii, an obligate intracellular parasitic protozoon, usually causes a mild, acute infection followed by a latent asymptomatic phase with tissue cysts or a chronic form with recurrent retinochoroiditis. However, immunocompromised patients can cause disseminated disease due to the reactivation of the latent tissue cysts or due to a primary infection. Here, we present a rare case of bilateral ocular toxoplasmosis and concurrent subacute toxoplasma encephalitis in a 70-year-old patient on anti-CD40 treatment following his liver transplant. The diagnosis was confirmed by PCR of anterio

Th1/Th2 balance and HTLV-I proviral load in HAM/TSP patients treated with interferon-alpha.

We studied the immunological and virological effects of interferon-alpha (IFN-alpha) therapy in nine patients with HTLV-I-associated myelopathy (HAM/TSP). After therapy, the percentages of CCR5+ cells in CD4+ cells significantly decreased in the cerebrospinal fluid as well as blood. The therapy also significantly lowered the intracellular IFN-gamma+/interleukin-4+ T-cell ratio in blood. Those helper T-cell type 1 (Th1)-related responses tended to be higher and reduce more evidently following therapy in three patients who clinically improved. Also, all the three patients had one or more HTLV-I

Laxative abuse as a cause for ammonium urate renal calculi.

Nine women with laxative abuse and predominantly ammonium urate renal calculi underwent metabolic studies to identify common chemical abnormalities and determine pathophysiology. The 24-hour urine studies demonstrated marked decreases in volume (902 cm.3), sodium (28 mEq.), citrate (116 mg.) and potassium (21 mEq.). A significant elevation in ammonium urate supersaturation was found compared to control subjects when studied by the computer model EQUIL 2. Of the patients 7 had 1 or more urine specimens positive for phenolphthalein. Gastrointestinal loss of fluid and electrolytes allowed for chr

The tumour lysis syndrome. Intensive care aspects of paediatric oncology.

Tumour lysis syndrome occurs when chemotherapy is started in children who present advanced lymphomas and leukaemias. Rapid cell lysis causes life-threatening metabolic derangements because of the release of intracellular potassium, phosphate and uric acid. An alkaline diuresis is commonly established before the start of chemotherapy to minimise uric acid and phosphate deposition within the kidney. Two cases are described where intravenous fluid loading resulted in acute pulmonary oedema, and the intensive care management of such cases is discussed. The improved outcome from increasingly aggres

Regucalcin in the Tumor Microenvironment: From Intracellular Tumor Suppression to Putative Extracellular Signaling.

Regucalcin (RGN), also known as senescence marker protein-30, is a calcium-binding protein without an EF-hand motif that regulates intracellular calcium homeostasis and Ca2+-dependent enzyme activity. Its expression is reduced in tumor tissue relative to matched normal tissue across several cancer types, and restoring RGN in cancer cell lines suppresses proliferation, migration, and invasion through cell cycle arrest, reduced expression of matrix metalloproteinases and epithelial-mesenchymal transition regulators, and transcriptional reprogramming involving p53, p21, Rb, c-myc, and β-catenin.
