Total knee arthroplasty in patients with prior ipsilateral hip arthrodesis.

Only a few articles based on the management of symptomatic knee osteoarthritis in patients with prior ipsilateral hip arthrodesis have been reported, and there are no clear criteria for the best treatment option [to carry out a total knee arthroplasty (TKA)-or to take down the hip fusion and conversion to a total hip arthroplasty-THA, and after that to carry out the TKA]. We report two cases, a 72-year-old male who underwent a left hip arthrodesis at 28 because of a trauma and a 51-year-old woman who underwent a left hip arthrodesis at 9 years because of a congenital dislocation. They presente

Operative treatment of patellofemoral maltracking with torsional osteotomy.

INTRODUCTION: A femoropatellar syndrome may be caused by a deformity in the torsional axis of the femur or the tibia. The two cardinal symptoms are anterior knee pain and patellar instability. METHODS: We retrospectively evaluated 32 torsional osteotomies. The aim of the study was to prove that a torsional osteotomy could bring about significant pain relief and create a stable femoropatellar joint. Besides clinical and radiographic analysis of leg geometry, a computed-tomography-based measurement of the torsional angle was performed. The mean duration of follow-up was 37 months. RESULTS: The

Use of a modified Elmslie-Trillat procedure to improve abnormal patellar congruence angle.

Forty patients underwent 45 modified Elmslie-Trillat realignment procedures (mean followup, 2 years) for refractory patellar instability (34 knees) or painful patellofemoral syndrome with malalignment (11 knees). The postoperative congruence angle (mean, +3.4 degrees) was significantly improved compared with the preoperative value (mean, +21.5 degrees). We considered the "normal" congruence angle average as -8 degrees (range, -20 degrees to +4 degrees). Over time postoperatively, we detected no statistical difference in the congruence angle (5 months, 3.4 degrees; 24 months, 6.3 degrees). Ther

Fixed-bearing versus high-flexion RP total knee arthroplasty (TKA): midterm results of a randomized controlled trial.

BACKGROUND: We compared the midterm results after total knee arthroplasty (TKA) using PFC Sigma RP-F mobile model with PFC Sigma PS fixed model. MATERIALS AND METHODS: In this randomized controlled trial, we analyzed 50 knees that underwent TKA with PFC Sigma RP-F and 60 knees with PFC Sigma PS fixed model. The follow-up period ranged from 76 to 104 months. RESULTS: The knee score, function score, and radiographic evaluation were significantly not different between the two groups at final follow-up. No revisions, subluxations, dislocations, or infections were seen. Also, no radiographic evid

Knee Ligament Instability Patterns: What Is Clinically Important.

The knee dislocation is a severe, complex injury that can be difficult to treat and is fraught with complications. The first step in a successful reconstruction of a multiple ligamentous knee injury is gaining an accurate and thorough understanding of the pattern of instability imparted by the injury. Evaluation begins with a detailed review of radiographic and advanced imaging studies followed by a thorough physical examination, often done under anesthesia, in conjunction with dynamic fluoroscopy. Failure to identify and reconstruct a damaged ligament may place undue stress on adjacent struct…

Conservative Versus Surgical Treatment for Primary Patellar Dislocation.

BACKGROUND: Primary patellar dislocation is often the initial manifestation of patellofemoral instability. Its long-term consequences can include recurrent dislocation and permanent dysfunction of the knee joint. There is no consensus on the optimal treatment of primary patellar dislocation in the relevant literature. The main prerequisite for a good long-term result is a realistic assessment of the risk of recurrent dislocation. METHODS: We carried out a systematic literature search in OvidSP (a search engine for full-text databases) and MEDLINE to identify suitable stratification models wit

[EFFECTIVENESS OF UC ULTRA-CONGRUENT ROTATING PLATFORM PROSTHESIS IN TREATMENT OF KNEE OSTEOARTHRITIS].

OBJECTIVE: To explore the effectiveness of UC ultra-congruent rotating platform prosthesis in the treatment of knee osteoarthritis by comparing with fixed-bearing implant. METHODS: The clinical data were retrospectively analyzed, from 98 patients (98 knees) with knee osteoarthritis undergoing primary total knee arthroplasty between January 2011 and December 2012. The UC ultra-congruent rotating platform prosthesis was used in 56 cases (mobile-bearing group) and fixed-bearing implant was used in 42 cases (fixed-bearing group). There was no significant difference in gender, age, side, weight, d

Anatomical Risk Factors for Anterior Cruciate Ligament Injury Are Not Important As Patellar Instability Risk Factors in Patients with Acute Knee Injury.

To compare in magnetic resonance imaging the anatomical risk factors for anterior cruciate ligament (ACL) injury and patellar dislocation among patients who suffered acute knee injury, 105 patients with acute knee injury resulting in 38 patellar dislocations (patella group), 35 ACL injuries (ACL group), and 32 meniscus or medial collateral ligament injuries (control group) were included. These groups were compared for risk factors for patellar dislocation (patellar height, trochlear dysplasia, and quadriceps angle of action) and for ACL injury (intercondylar width, posterior inclination of tib

Tibial tubercle proximalization as quadriceps lengthening in treating severe habitual patellar dislocation in adults.

BACKGROUND: To describe a novel 'four-in-one' procedure - including tibial tubercle proximalization, extensive lateral release, tibial tubercle medialization, and medial patellofemoral ligament reconstruction - for treating severe habitual patellar dislocation (HPD) in adults, and to report its early clinical outcomes. METHODS: Thirteen patients (13 knees) with severe primary HPD received this procedure. Results of the physical examinations, including apprehension tests and patellar tracking throughout full range of motion, were recorded pre-operatively and at final follow-up. Radiological as

Traumatic anterior hip dislocation with associated bilateral femoral fractures in a child: a case report and review of the literature.

Traumatic anterior hip dislocation is rare, because the hip joint is a highly stable joint. It is extremely rare for the anterior hip dislocation with combined bilateral femoral fracture in children. We present a case of 7-year-old boy with traumatic anterior hip dislocation with associated bilateral femoral fractures. Radiographic examination showed the right femoral head was dislocated anteroinferiorly. The ipsilateral femoral shaft showed a transverse femoral shaft fracture and proximal and distal femoral bifocal fractures of the contralateral femur. The dislocation of the right hip was red

Medial patellofemoral ligament reconstruction for recurrent patellar instability.

The purpose of this study was to determine the value of reconstruction of the medial patellofemoral ligament (MPFL) in the treatment of recurrent patellar dislocation and subluxation. We retrospectively reviewed 40 randomised patients with recurrent patellar dislocation or subluxation, who had undergone realignment surgery from July 1999 to December 2001. Group E consisted of 20 patients who had undergone an Elmslie-Trillat procedure. In Group M, also consisting of 20 patients, the Elmslie-Trillat procedure was combined with reconstruction of the MPFL. Pre and postoperative data were collected

Femoral fracture mimicking acute arthritis in Down's syndrome.

We describe a 38-year-old white woman with Down's syndrome with a history of chronic arthritis and hyperuricemia who presented with acute left knee pain, patellar tenderness and patello-femoral instability. Findings appeared due to a fracture of the lateral femoral condyle, which responded to conservative therapy with a spica cast. Patellofemoral instability in Down's syndrome can be associated with significant morbidity including femoral condyle fracture. Patients with Down's syndrome who present with acute knee arthritis responding poorly to antiinflammatory agents and other conventional the

Surgical management for recurrent patellar dislocations in skeletally immature patients.

BACKGROUND: Due to his multifactorial aetiology, treating patellofemoral instability can be a challenge for the orthopaedic surgeon. The incidence of patellofemoral instability shows a peak during adolescence, especially from 11 to 14 years old. AIM: Several clinical studies focusing on recurrent patellar dislocations in skeletally immature patients have been published, reporting inconsistent or controversial results. Currently, there is a lack of consensus regarding the surgical management of these patients. The purpose of this study is to update current evidence and systematically review in

[Imaging of external instabilities of the patella. State of the art].

External femoropatellar instability is a dynamic abnormality from various origins: osseous, cartilaginous or musculotendinous; X-rays films cannot give a precise enough description of this phenomenon. Attention is drawn by anterior pain or a sensation of instability. Clinical analysis distinguishes between permanent, traumatic or transient dislocations which are now more frequently discovered as part of a femoro-patellar syndrome with or without cartilage involvement. Conventional imaging, CT-scan and MR imaging are based on faultless techniques. Lateral views precisely report femoropatellar a

Survivorship analysis of cementless meniscal bearing total knee arthroplasty.

Four hundred seventy-three consecutive cementless cruciate retaining meniscal bearing primary total knee replacements were done on 375 patients from May 1985 to February 1991. These were observed for a 10-year period (average, 5 years). Seventeen (3.6%) required change of components because of mechanical failure. There were 12 polyethylene fractures or dislocations. There were 5 tibial subluxations secondary to ligamentous instability occurring at an average of 21 months postoperatively. There were 2 component loosenings secondary to bone graft resorption (1 femoral, 1 tibial). There were 5 in

"Missed" or late-diagnosed congenital dislocation of the hip. A clinical entity.

Reported series of neonatal examinations of the hip by trained orthopedic surgeons reveal cases of dislocation of the hip discovered many months after the previous normal examination. The possibility of delayed dislocation of the hip needs further study and its recognition is important for the medical and legal professions. Physicians may be blamed erroneously for failure to diagnose CDH at an early stage. In 10 documented cases, early examinations by qualified experts revealed normal hip findings, although the patients were later discovered to have CDH. In five instances, the physical examina

The press-fit condylar modular total knee system with a posterior cruciate-substituting design. A concise follow-up of a previous report.

The purpose of the present study was to determine the long-term results of a series of 150 consecutive primary posterior stabilized modular knee arthroplasties that had been performed in 118 patients with use of a circumferential tibial insert capture as described in a previous report, published in 1997. The patients were evaluated with use of a patient-administered questionnaire; Knee Society clinical, functional, and radiographic scoring systems; and Kaplan-Meier survivorship analysis. A good to excellent result was confirmed in seventy-six (90%) of the eighty-four patients (105 knees) with
