Preliminary report on amputation versus reconstruction in treatment of tibial hemimelia.

OBJECTIVE: Tibial hemimelia is a rare disorder characterized by the absence or hypoplasia of the tibia with associated rigidity. The aim of this study was to retrospectively evaluate the affectivity of reconstructive surgeries including centralization of the knee-ankle joints and lengthening with Ilizarov principles, as well as physical and functional results of amputation and reconstruction. METHODS: This is an IRB-approved retrospective review of all patients diagnosed with tibial hemimelia who required surgery at a single institution between 1998 and 2011. Charts were analyzed for clinical

CT scanogram for limb length discrepancy in comminuted femoral shaft fractures following IM nailing.

INTRODUCTION: Leg length discrepancy (LLD) following intramedullary nailing of femoral fractures is not uncommon. We designed a prospective study to evaluate the efficacy of routine postoperative computed tomography (CT) scanograms for evaluation of limb length discrepancy in patients with comminuted Winquist III or IV femoral shaft fractures treated with intramedullary nailing. METHODS: The study consisted of 15 patients with Winquist III and 13 with a Winquist IV femoral shaft fracture pattern with an average age of 37 years. The mechanisms of injury were motor vehicle collision (13), gunsh

3D visualized robot assisted reduction of femoral shaft fractures: evaluation in exposed cadaveric bones.

The main problems in intra-medullary nailing of femoral shaft-fractures are leg-length discrepancies and rotational differences with an incidence of 2-18% and 20-40% respectively. These may lead to severe postoperative sequelae such as additional correctional operations and difficult rehabilitation. Insufficient visualization can be considered the main reason for these complications. Finally, retention of the fragments in the correct alignment before nail insertion is difficult. To overcome these problems we established a robotic telemanipulator system to support the reduction process. It was

Parkes-Weber syndrome associated with a congenital short femur of the affected limb.

The association of capillary malformation, high-flow arteriovenous fistulas, and limb hypertrophy corresponds to Parkes-Weber syndrome. Most of cases are sporadic, although a first familial case has been recently reported. We report the first observation of a Parkes-Weber vascular anomaly with an underlying congenital short femur.

Reconstruction with callus distraction for nonunion with bone loss and leg shortening caused by suppurative osteomyelitis of the femur.

We present a retrospective study of 27 patients treated by callus distraction using a unilateral external fixator of our own design for nonunion with bone loss and shortening of the femur caused by suppurative osteomyelitis. The unilateral external fixator was used either alone or in combination with an intramedullary nail. The mean age of the patients was 13.6 years (8 to 18). The fixator was used alone in 13 patients and with an intramedullary nail in 14. The bone results at a mean follow-up of 88 months (37 to 144) were excellent in 16 patients and good in 11. The functional results were ex

Postaxial hypoplasia of the lower extremity.

Congenital deficiency of the fibula may present a variety of patterns. In a series of 20 patients with this diagnosis, we have observed limb-length inequality and a spectrum of musculoskeletal anomalies involving the ipsilateral hip, femur, knee, tibia/fibula, ankle, and foot. Considering the frequently associated abnormalities of the lower extremity, the term postaxial hypoplasia may be more descriptive than the traditional terms fibular hemimelia or fibular a/hypoplasia. By raising the awareness of associated deformities, the clinician is better prepared to advise patients and to intercede a

[Acceleration of epiphyseolysis capitis femoris lenta as a complication of growth hormone therapy in hypophyseal insufficiency].

Slipped capital femoral epiphysis is a rare complication of growth hormone therapy. We report on a young man with pituitary insufficiency, diagnosed and treated with growth hormone at the age of 14 9/12. The patient withdrew from treatment after 6 months of growth hormone therapy without significant catch-up growth or complications. At the age of 21 8/12 years slipping of the left femoral capital epiphysis became apparent 2 1/2 months after treatment with growth hormone had been resumed in combination with low dose testosterone. Young adult patients with unfused epiphyses undergoing growth hor

The results of 39 fractures complicated by major segmental bone loss and/or leg length discrepancy.

Thirty-nine fractures with segmental bone loss and/or limb length discrepancy with defects of 1.8 to 12.0 cm were reviewed retrospectively. Most patients presented with an acute open fracture with segmental bone loss or a late reconstructive problem with a nonunion, frequently complicated by infection, segmental bone loss, and adjacent soft-tissue disruption. One hundred forty-two weeks was the average length of time of treatment before referral of a patient to us. Eighty-eight per cent of the patients received a bone graft that included either autologous cancellous bone, bank bone, a vascular

Rapid rehabilitation programme following sacral stress fracture in a long-distance running female athlete.

Stress fractures occur in normal bone due to mechanical overload by cyclic stress increasing the osteoclastic activity, thus facilitating weakening leading to fracture of bones. Long-distance running may lead to stress fractures of the mid- and distal tibia and of the metatarsal bones. Stress fractures to the sacrum are rare. Certain factors for stress fractures in runners have been identified, such as leg-length inequality, a high longitudinal arch of the foot, forefoot varus, and menstrual irregularities in case of female athlete triad. We report on a 22-year-old female runner (usually train

Electromyographic and nerve conduction changes after tibial lengthening by the Ilizarov method.

Clinical observation of several hundred Ilizarov patients has demonstrated prolonged muscle weakness in the involved limb after treatment. The etiology may be either neuropathic or myopathic. Direct intraoperative neural injury is unusual but generally is detected immediately postoperatively. We wished to determine the etiology of this clinical problem. Six consecutive patients completing tibial lengthening by the Ilizarov method were examined by electrodiagnostic methods. All patients were clinically normal. Electrodiagnostic testing showed abnormalities in six of six deep peroneal nerves. Fi

Length and offset restoration in partial hip arthroplasty (PHA) performed by resident surgeons: Comparison between preoperative planning and intraoperative X-ray.

Restoring of leg length and femoral offset in partial hip arthroplasty (PHA) performed by residents (level one of experience surgery) in the neck femoral fractures has a significant role in the clinical outcome. The aim of this study is to show the statistically significant differences in restoring hip geometric parameters using preoperative planning vs intraoperative X-ray. Authors compared the use of pre-operative planning with intra-operative radiography to restore the limb length and femoral offset, focused on the measurement of femoral offset and leg length discrepancy (LLD). Based on the

'Rail and Nail' bifocal management of atrophic femoral nonunion.

AIMS: The management of a significant bony defect following excision of a diaphyseal atrophic femoral nonunion remains a challenge. We present the outcomes using a combined technique of acute femoral shortening, stabilized with a long retrograde intramedullary nail, accompanied by bifocal osteotomy compression and distraction osteogenesis with a temporary monolateral fixator. PATIENTS AND METHODS: Eight men and two women underwent the 'rail and nail' technique between 2008 and 2016. Proximal locking of the nail and removal of the external fixator was undertaken once the length of the femur ha
